BACKGROUND: Combined ipsilateral femoral neck and shaft fractures are an uncommon type of fractures. A number of different implant options are available for the management of this injury. Two-device procedures were suggested because of the higher rate of malunion by single-device treatment. However, surgical treatment using a cephalomedullary nail is still an alternative option that provides better mechanical advantage and minimal invasion. This study details the technique of treating these pattern fractures with proximal femoral nail anti-rotation II (PFNA-II) to achieve an acceptable reducti

PURPOSE: Condylar head fractures (CHFs) are increasingly treated by open reduction and internal fixation (ORIF). However, there are no reports on the three-dimensional postoperative volumetric transformation of the condyle, especially with regard to fragmented cases. Protruding hardware can lead to severe complications, so the goal of this study was to examine the amount of condylar bony resorption occurring after ORIF.
METHODS: Included were surgically treated CHFs with eligible cone beam computer tomography (CBCT) datasets immediately after ORIF (T1) and after implant removal (T2), plus fra

Periprosthetic femoral fractures around the femoral stem of a hip prosthesis constitute a serious challenge. The number of these fractures, associated with a high failure rate and a high number of overall complications, including higher mortality, has been on an increase worldwide. Stable femoral stems are indicated for osteosynthesis, while in case of loosening the method of choice is the replacement by a revision implant. The aim of osteosynthesis of periprosthetic femoral fractures is a stable fixation with soft tissue preservation which results in faster union allowing the patient to retur

Vertical Pauwels Type III fractures of the neck of femur in young patients are difficult fractures to treat. These are usually as a result of high energy trauma which are associated with a significant degree of fracture comminution in the neck of femur. This makes the fracture reduction and fixation difficult. Many fixation methods and implants have been described for use in these fractures but are not without reported complications such as non-union and varus collapse. In this article, we highlight the incidence of fracture comminution in both the anterior and posterior cortices of the femora

OBJECTIVE: To investigate the effectiveness of percutaneous compression plate (PCCP) fixation for femoral neck fracture.
METHODS: A clinical data of 100 patients with femoral neck fractures who were treated with internal fixation were analyzed retrospectively. The fractures were fixed with the cannulated screws (CS) in 55 patients (CS group) and with the PCCP in 45 patients (PCCP group). There was no significant difference in gender, age, the cause of injury, the fracture type, complications, and disease duration between the two groups ( P>0.05). The quality of fracture reduction, bone resorp

The aim of the present technical note is to describe the posterior transiliac plating method. Acute-phase clinical and radiological results in a continuous series of 10 vertically unstable fractures are presented. Reduction was good or excellent in 70% of cases, and clinical results were good or excellent in 80% on Hannover and Majeed scores, with mean Majeed score of 71.8±17. Complications comprised 3 cases of early postoperative sepsis requiring surgical lavage, and 4 cases of hardware removal due to discomfort. At last follow-up, consolidation was systematic. Reduction was more difficult wi

Patella fractures comprise 1% of all fractures. Treatment options vary based on fracture displacement, classification, and patient factors. Traditionally, nonoperative treatment has been reserved for nondisplaced fractures. Many operative treatments are available with differing indications and levels of success. Tension band constructs have been the most commonly employed approach to fixation, with cerclage wiring for comminuted fractures. Recently, plate fixation of patella fractures has become more popular. Plating constructs offer a low-profile design with stable fixation, allowing for earl

We describe a novel technique in correcting distal radius fractures deformed with significant shortening or coronal plane translation in both the acute or chronic setting. The technique involves using a modified push-pull device to assist the surgeon in correcting length and/or translation of the articular block without the use additional hardware outside of the volar plate.

Ankle fractures are common and may be treated nonoperatively or by reduction and internal fixation. Both treatments may lead to nonanatomic healing, called malunions. Malunions lead to joint instability and pathologic load transfer. Arthritic joint destruction may be the consequence. Corrective osteotomies for malunited malleolar fractures have to restore anatomic conditions. Osteotomies of the medial malleolus, lateral malleolus, posterior malleolus (Volkmann's triangle), and the posteromedial corner or combinations thereof described. Corrective osteotomies for malleolar malunions show good r

OBJECTIVE: To compare the biomechanical properties of dual bone fixation (DBF) constructs to radial locking compression plating (LCP) in an ex vivo feline antebrachial fracture gap model.
STUDY DESIGN: Ex vivo study.
SAMPLE POPULATION: Cadaveric feline antebrachii (n=12 pairs).
METHODS: Antebrachii were radiographed to confirm normal skeletal appearance and maturity. After creation of a 5 mm radial and ulnar ostectomy, each antebrachium received 1 of 3 constructs using an incomplete randomized block design (n=8 per group). All groups received a 10 hole 2.0 mm radial LCP. DBF groups received

Coronoid fractures commonly occur in a part of unstable elbow fractures. Stable coronoid fracture fixation is important for early elbow motion. It may be difficult to fix multifragmented coronoid process fractures with loose capsular attachment. Thus, we demonstrated the endobutton suspensory technique to fix the anterior coronoidal fragments due to its suspensory effect providing capsular attachment. This technique allows stable fixation to the small multifragmented coronoid fractures with early range of motion.

Bicondylar Hoffa's fractures of distal femur are rare. We report the case of an 18-year-old male who sustained a closed complex fracture of both femoral condyles of the left femur on the sagittal plane (Hoffa fracture), combined with distal metaphyseal fracture. He was treated using parallel plates and interdigitating screws, with our technique based on an extrapolation of the principles of distal humerus fixation. The fracture united clinically and radiologically at 16 weeks. Twelve months postoperatively, the range of movement in the knee was 5°-115°. In this complex case, our technique prov

The rate of nonunion is estimated to be 1.1% to 10% following closed treatment of proximal humerus fracture and 5.5% following closed treatment of humeral shaft fracture. Surgical management should be considered for fractures that demonstrate no evidence of progressive healing on consecutive radiographs taken at least 6 to 8 weeks apart during the course of closed treatment. In the case of proximal humerus nonunion, recent series have demonstrated union in >90% of patients treated with reconstruction using locking plates and autogenous bone graft. Shoulder arthroplasty is reserved as a salvage

Combined forearm fractures are identified according to their location as Galeazzi, Monteggia or Essex-Lopresti injuries. The feature common to these three forms is the combination of a forearm fracture with instability of the distal or proximal radio-ulnar joint. The combination of Galeazzi and Monteggia fractures in the same extremity is an exceedingly rare occurrence. It has been reported in eight cases including two pediatric patients worldwide. In this case report the rare occurrence of the combination of these injuries and the possibility of pitfalls in the operative treatment are present
