Thrombocytopenic purpura during the incubation of hepatitis B.

A 6-year-old girl presented with acute thrombocytopenic purpura. Hepatitis B surface antigenaemia was present at the time of this illness, and 8 weeks later she developed acute icteric hepatitis B. Screening for hepatitis B virus should be considered in children with apparently idiopathic thrombocytopenic purpura.

Cluster of fulminant hepatitis B in crack users.

Fulminant hepatitis occurs in only 1% of acute hepatitis B patients, requiring hospitalization, but coinfection with delta virus increases the incidence. Hepatitis B and D infection are commonly associated with intravenous drug abuse, but there have been no previous reports of an association with nonparenteral cocaine. Crack use, via sexual promiscuity, is associated with an increased risk for human immunodeficiency virus infection, but has never been associated with viral hepatitis. We report four fatal cases of fulminant hepatitis B including, one with delta virus coinfection and one with hu…

[Hepatitis B infection and primary glomerulonephritis in children].

Out of 165 children presenting with renal pathology, HBV-infection was detected by RIA in 69.7% of children suffering from glomerulonephritis and in 30.8% of children with pyelonephritis. The infection manifested by the carriage of HBs-antigen and persistent hepatitis. Unlike the control group (without HBv-infection), in patients with HBv-infection, glomerulonephritis was characterized by the development of a prognostically unfavourable mixed form there of in children under 7 years, by rare cases of the onset of a complete remission consequent to immunosuppressive therapy, a decline of the lev…

Protective antibodies to hepatitis B virus in haemophiliacs.

Haemophilic patients are at increased risk from hepatitis B virus infection because of their need for blood product therapy. They are potentially poor responders to hepatitis B vaccine due to immunological abnormalities resulting from two causes: infection with the human immunodeficiency virus and treatment with clotting factor concentrates. The protective antibody response to hepatitis B virus in vaccinated haemophiliacs was investigated using a competitive enzyme-linked immunosorbent assay which employs a monoclonal antibody, RF-HBs-1, that recognises a virus-neutralising epitope on HBsAg. S…

[Two-dimensional pulse Doppler echography in chronic hepatitis].

A separate estimation of blood flow in the portal and hepatic veins using ultrasonic two-dimensional tomography and impulse Doppler echography of the liver was tried in 30 chronic sufferers with hepatitis versus 31 healthy controls. Two-dimensional impulse Doppler echography recorded the blood flow spectrum in the right hepatic vein at the site of its entering of vena cava inferior as well as in the portal vein at the portal fissure simultaneously with ECG. The index of portal blood flow and that of right hepatic vein flow in systole are proposed. The above method warrants a separate estimatio…

Hepatocellular carcinoma associated with autoimmune chronic active hepatitis.

Primary hepatocellular carcinoma, one of the most common malignancies in the world, develops in chronic liver diseases of different etiologies. Hepatitis B and non-A, non-B infections, alcoholic cirrhosis, hemochromatosis, contamination with aflatoxins, and oral contraceptives are just a few of the conditions that have been associated with this carcinoma. To our knowledge, few cases of autoimmune chronic active hepatitis and hepatocellular carcinoma have been reported in the literature. We describe a patient who developed hepatocellular carcinoma 21 years after the onset of the autoimmune hepa…

Transfusion-transmitted diseases other than AIDS and hepatitis.

Although the major diseases transmitted by transfusion today are AIDS and hepatitis, many others also are known. These include CMV, syphilis, Chagas disease, babesiosis, parvovirus B19, malaria, Epstein-Barr infection, and many others that have been reported only once or twice. Reducing the risk of transfusion-transmitted diseases is a problem for donor centers where donor screening and laboratory testing for possible carriers is undertaken. Physicians should be aware that the potential for disease transmission is always present when transfusions are administered.

Hepatitis B virus infection and health care workers.

Hepatitis B virus (HBV) infection is the major occupational risk of health care workers, among whom HBV infection risk is 2 to 10-fold elevated above that of the general population. Risk is most clearly related to the degree of direct contact with human blood and body fluids, and to frequency of traumatic exposures in the workplace. Most exposures result from unsuspected asymptomatic HBV carriers, who represent 1 to 2% of hospitalized patients. Risk of HBV infection also varies with the frequency of HBV carriers in the patient population. In the USA, the annual number of new cases has been con…

[The seroepidemiological observation on hepatitis delta virus infection].

To assess the relationship between hepatitis delta virus (HDV) infection and HBV and reveal the distribution of HDV infection and the feature of epidemic, we collected 271 cases of HBV infected people in Shijiazhuang area and tested anti-HD with EIA from April 1987 to October 1988. This study found the prevalence of HDV infection was as high as 12.92% (35/271), and male prevalence of anti-HD was 14.06% (27/192) and female 10.13% (8/79), but there was no significance difference (P greater than 0.05). This suggested that the area of Shijiazhuan was a spot where HDV infection was high. Among thes…

[Reaferon treatment in acute HBsAG-positive viral hepatitis].

Clinical, biochemical and serological examinations of 412 patients with acute HBsAg-positive virus hepatitis were conducted to assess therapeutic efficacy of Soviet recombinant alpha 2-interferon (reaferon). There were 309 cases of acute virus hepatitis B, 103 of delta infection (71--coinfection, 32--superinfection). The study and control groups were assigned randomly. Reaferon i.m. administration started on jaundice day 1-5 and lasted for 10-11 days. The treatment proved effective in acute hepatitis B running a moderately severe and severe course up to the development of acute hepatic encepha…

Hepatitis B infection in infants after neonatal immunization.

A double-blind randomized placebo-controlled study to prevent hepatitis B infection in 235 babies born to chronic hepatitis B, HBeAg carriers was carried out. Babies in three treatment groups all received heat-inactivated hepatitis B vaccine. In addition multiple doses of HBIG and a single dose of HBIG were given in groups I and II respectively. After three years of follow-up, 4/60 (Group I), 3/64 (Group II), and 1/64 (Group III) developed chronic infection. For those who escaped chronic infection, other hepatitis events also occurred. They were transient HBs-antigenaemia, anti-HBc conversion …

Autoimmune chronic active hepatitis and primary biliary cirrhosis.

Autoimmune CAH is important to recognize, since it is highly responsive to treatment which undoubtedly prolongs life. Autoimmune CAH can rarely be cured; complete withdrawal of treatment leads to relapse in over 80% of patients. Prednisolone and azathioprine are the major drugs of choice, the former inducing remission while the latter maintains remission, either alone or in combination with prednisolone. Since both drugs are associated with substantial side-effects which tend to be dose-related, the object of treatment must be to induce and maintain remission with the minimum risk of relapse t

[Clinical forms and prognostic criteria in alcoholic hepatitis].

The biological and clinical features and prognostic factors of 65 patients affected by alcoholic hepatitis were studied. All patients had an ethanol intake higher than 80 gr/day during at least 3 years. 22 patients were female and 43 male with a mean age of 45 +/- 11.7 years. 19 had acute hepatitis (29.2%), 2 had acute hepatic insufficiency (3%), one had acute cholestasis (1.5%), 14 had chronic hepatopathy (21.5%). 29 patients had the diagnosis (44.6%) confirmed by histologic analysis. All patients had liver enlargement, 25 had jaundice and 4 had fever. The hepatic biopsies showed steatosis in…

Study of reactivation of chronic hepatitis delta infection.

Five patients with chronic hepatitis delta virus (HDV) infection suffered spontaneous episodes of liver enzyme elevation on a background of otherwise biochemically stable liver disease. In all five patients these episodes were accompanied by a rise in serum levels of anti-HDV IgM, HDV antigen and HDV RNA. These episodes of increased HDV replication accompanied by biochemical evidence of liver injury are reminiscent of reactivation in chronic hepatitis B. Surges of increased HDV replication may be important in the progression of liver disease in chronic HDV infection.
