Outcome of Subsequent Therapies After 177Lu-Vipivotide Tetraxetan for Metastatic Castrate-Resistant Prostate Cancer: A Tertiary Cancer Center Experience.

BACKGROUND: 177Lu-vipivotide tetraxetan (177Lu-PSMA-617, LuPSMA) improves overall survival in patients with metastatic castration-resistant prostate cancer (mCRPC) after at least one taxane chemotherapy and androgen receptor pathway inhibitor. There are limited data on the clinical course and outcomes of patients with mCRPC after receipt of LuPSMA. METHODS: We queried an IRB-approved prospectively maintained registry of all patients with mCRPC who received standard-of-care LuPSMA at our institution between June 2022 and January 2024. Clinical data about LuPSMA and subsequent therapies were ex

Therapeutic evaluation of [212Pb]Pb-AB001 and [177Lu]Lu-PSMA-617 in a mouse model of disseminated prostate cancer.

BACKGROUND: Metastatic castration-resistant prostate cancer (mCRPC) frequently leads to bone and soft tissue metastases, leading to poor prognosis. The beta-emitting radioligand [177Lu]Lu-PSMA-617 targets the prostate-specific membrane antigen (PSMA) and may be less efficient against micrometastatic disease. The alpha-emitting radioligand [212Pb]Pb-AB001 could offer enhanced treatment by delivering high energy over a short range. This study compared the efficacy of [212Pb]Pb-AB001 and [177Lu]Lu-PSMA-617 in a mouse model of disseminated prostate cancer. METHODS: Binding and internalisation of

Olanzapine/Samidorphan (Lybalvi) and Buprenorphine: Considerations for a Contradictory Combination: A Case Report.

INTRODUCTION: Samidorphan is an opioid receptor antagonist and naltrexone analogue that has been harnessed in unique ways. Samidorphan has been paired with olanzapine in the new oral agent Lybalvi, approved by the FDA in 2021 for the treatment of schizophrenia and bipolar I disorder. The addition of samidorphan to olanzapine has the intention of reducing olanzapine's metabolic side effects (ie, weight gain), however, samidorphan's antagonism of the mu opioid receptor can lead to complications in patients with opioid use disorder on agonist therapy. These complications include not just precipit

Validation of the collapsed-cone superposition for whole-body patient-specific dosimetry in [177Lu]Lu-PSMA-617 radionuclide therapy.

BACKGROUND: Patient-specific dosimetry in radiopharmaceutical therapy (RPT) offers a promising approach to optimize the balance between treatment efficacy and toxicity. The introduction of 360° CZT gamma cameras enables the development of personalized dosimetry studies using whole-body single photon emission computed tomography and computed tomography (SPECT/CT) data. PURPOSE: This study proposes to validate the collapsed-cone superposition (CCS) approach against Monte Carlo (MC) simulations for whole-body dosimetry of [177Lu]Lu-PSMA-617 therapy in patients with metastatic castration resistan

Single-time-point tumor dosimetry using population-based model selection and nonlinear mixed-effects modeling in [177Lu]Lu-PSMA-617 therapy.

BACKGROUND: Molecular radiotherapy with [177Lu]Lu-PSMA-617 is an effective treatment for metastatic castration-resistant prostate cancer. Accurate dosimetry is essential for maximizing therapeutic efficacy while minimizing toxicity. However, standard dosimetry requires multiple imaging sessions, posing logistical challenges. Single-time-point (STP) dosimetry offers a practical alternative but remains challenging for tumor kinetics due to high inter-patient variability. Nonlinear mixed-effects (NLME) modeling, combined with population-based model selection (PBMS), has demonstrated potential for

Case Report of Treatment of Polymicrobial Bacteremia Caused by Carbapenem-Resistant Klebsiella pneumoniae, and Bacillus Species in an Immunocompetent Patient With Meropenem-Vaborbactam.

Klebsiella pneumoniae carbapenemases (KPCs) are the most common carbapenemases in the United States. The Infectious Disease Society of America (IDSA) recommends meropenem-vaborbactam, ceftazidime-avibactam, or imipenem-cilastatin-relebactam as preferred options with cefiderocol as an alternative, all of which are also alternative options for CTX-M Enterobacterales. Bacillus species, comprising 22 different species, are not commonly pathogenic, but can cause invasive nosocomial infections typically in immunocompromised hosts commonly treated with vancomycin or carbapenems. Comparative efficacy of biologics and small molecule drugs in treating pediatric atopic dermatitis in patients aged 2-18 years: A 12-16 week network meta-analysis of randomized controlled trials.

BACKGROUND: Atopic dermatitis (AD) predominantly manifests before age five, with significant phenotype variations across age groups. Despite increasing systemic treatments for pediatric AD, head-to-head comparisons in network meta-analyses focused on children are scarce. METHODS: Through systematic searches of PubMed, Embase, Web of Science, and Cochrane Library up to March 2024, we identified randomized controlled trials (RCTs) evaluating systemic treatments for moderate-to-severe AD in children aged 2-18 years. From 900 screened articles, 8 RCTs (n = 2636) met inclusion criteria, comparing

The ENHANCE-3 study: venetoclax and azacitidine plus magrolimab or placebo for untreated AML unfit for intensive therapy.

Patients with acute myeloid leukemia (AML) ineligible for intensive chemotherapy (IC) have limited treatment options. The phase 3 ENHANCE-3 study aimed to determine whether magrolimab (magrolimab arm) was superior to placebo (control arm) when either was combined with venetoclax and azacitidine. Adults with previously untreated AML who were ineligible for IC were randomized to receive magrolimab (1 mg/kg on days 1 and 4, 15 mg/kg on day 8, 30 mg/kg on days 11 and 15, then weekly for 5 weeks, and then every 2 weeks) or placebo, venetoclax (100 mg on day 1, 200 mg on day 2, and 400 mg daily ther

Augmenting the Anti-Leukemic Activity of the BCL-2 Inhibitor Venetoclax Through Its Transformation Into Polypharmacologic Dual BCL-2/HDAC1 and Dual BCL-2/HDAC6 Inhibitors.

Motivated by the anti-leukemic synergy between histone deacetylase (HDAC) inhibitors and the FDA-approved BCL-2 inhibitor venetoclax, coupled with our interests in polypharmacology, we sought to bolster the anti-leukemic efficacy of the clinical drug by grafting HDAC1-selective or HDAC6-selective inhibitor motifs onto a solvent-accessible domain of venetoclax. We discovered multiple polypharmacological agents that both retained the potent BCL-2 inhibitory activity of venetoclax and effectively inhibited either HDAC1 or HDAC6 with excellent (up to 80-fold) selectivities for the desired HDAC iso

SIRT5 inhibition impairs mitochondrial metabolism and enhances venetoclax-induced elimination of acute myeloid leukemia cells.

Metabolic reprogramming is a key focus of targeted therapies in acute myeloid leukemia (AML). The mitochondrial sirtuin SIRT5 removes succinyl groups from specific lysines and impacts cell metabolism, but its role in AML tumorigenesis has not been extensively explored. A recent study highlighted that SIRT5 regulates AML cell activity by modulating glutamine metabolism, but its molecular targets in AML remain unclear. This study aims to identify the substrates of SIRT5 in AML. It was found that a total of 83 proteins with 121 lysine (K) residues showed increased succinylation after SIRT5 knockd

Objectively measured physical function predicts mortality in older adults with acute myeloid leukemia treated by lower-intensity therapy: A prospective cohort study.

INTRODUCTION: In older adults with acute myeloid leukemia (AML) treated with intensive chemotherapy, the Short Physical Performance Battery (SPPB) and the short form of the Geriatric Depression Scale (SGDS-K) predict survival outcomes. However, their role in lower-intensity therapy remains unclear. We evaluated their prognostic role in older adults with AML treated with hypomethylating agents (HMA) with or without venetoclax (VEN). MATERIALS AND METHODS: In this single-center, prospective cohort study, patients with AML aged ≥60 years receiving HMA or HMA/VEN therapy were included. SPPB and S

Clinical activity of venetoclax and azacitidine in children with de novo or secondary multiple relapsed/refractory acute myeloid leukemia: a real-world experience.

Acute myeloid leukemia (AML) remains a major therapeutic challenge, particularly in relapsed or refractory patients, where prognosis is poor. The combination of venetoclax and azacitidine (ven/aza) has demonstrated significant efficacy in adults, yet evidence in pediatric populations is still limited and warrants further investigation. We conducted a multi-center retrospective analysis of 38 pediatric patients with relapsed/refractory (R/R) AML, including patients with de novo and secondary AML, treated with compassionate use ven/aza in four European countries between 2017 and 2023. Dual activation of 5-HT1A and μ-opioid receptors mediates dezocine's antidepressant effects in mice with comorbid pain and depression.

BACKGROUND: Pain-depression morbidity affects millions, but there are no effective treatments available. Dezocine is a clinically used opioid analgesic with complicated pharmacological mechanisms. Given this unique pharmacological profile and the crucial involvement of noradrenergic/serotonergic systems in the pathophysiology of depression, dezocine might be able to relieve pain-depression comorbidity. This possibility has not been tested and is the focus of this study. However, this potential has not yet been empirically validated, and investigating it constitutes the primary aim of this stud
