[Therapeutic strategies in urinary tract infections in women].

Simple acute cystitis is cured by single-dose or 3-day treatment. Complicated acute cystitis requires clinical, bacteriological and imaging examinations. Treatment must be continued for 10 days and controlled. Acute pyelonephritis in women requires bacteriology, ultrasonography and plain x-rays and must be rapidly treated by fluoroquinolone or cephalosporin. High-risk acute pyelonephritis justifies admission to hospital, more elaborate examinations and active treatment.

Necrotizing soft tissue infections. Surgical or conservative treatment?

BACKGROUND: Both surgeons and dermatologists are increasingly challenged with the prompt diagnosis and management of severe soft tissue infections. Although early surgical intervention appears to be for life-saving in many patients, especially those diagnosed as necrotizing fasciitis, some patients recover well with only conservative treatment. Because most of these infections have similar initial clinical presentations, there remains a need to find reliable clinical and/or laboratory parameters that can predict the prognosis and to accordingly judge the necessity and timing of operation. MET…

Opportunistic infections occurring during highly active antiretroviral treatment.

OBJECTIVE: To analyse the characteristics of opportunistic infections in patients receiving highly active antiretroviral treatment (HAART). DESIGN AND METHODS: A retrospective study performed in seven hospitals, included all patients starting treatment by ritonavir or indinavir between 26 March and 31 December 1996. Patients were evaluated for the development of AIDS-defining events. Clinical evaluation, plasma HIV-1 RNA quantification, CD4 cell count were recorded at baseline and at the onset of the event. RESULTS: Four hundred and eighty-six patients were included: 44.2% had a CD4 cell cou…

Molecular biology and pathogenesis of human polyomavirus infections.

The two human polyomaviruses JC and BK are ubiquitous in the human population. Primary infection leads to lifelong persistence in the kidney, the CNS and in lymphoid cells. Virus is shed into the urine and is transmitted at least in part by the oral route. Under limited changes of the immunological state persistent polyomavirus infection is activated to an asymptomatic virus production. However, in severe long-lasting immunosuppression, highly effective virus multiplication can be accompanied by extended cytolytic damage of viral target cells leading to fatal disease. Whereas BKV is associated

Patient referral outcome in gonorrhoea and chlamydial infections.

OBJECTIVE: To describe the outcome of patient referral at the STD clinic of the University Hospital Rotterdam. To study characteristics of heterosexual index patients and partnerships related to referral outcome. METHODS: In 1994, patients with gonorrhoea and chlamydia were referred to public health nurses for interview and patient referral. Referral outcome was classified as "verified" if partners attended the STD clinic and as "believed" if partners were said to have attended elsewhere. RESULTS: Of 454 patients, 250 (55%) participated in the study. The outcome of patient referral for the 5

Detection of mycobacterial infections using the Dieterle stain.

Retrospective review comparing the modified Dieterle stain with standard acid-fast stains was performed on seven surgical pathology cases that contained culture-positive mycobacteria infections. Tissues examined comprised cervical and submandibular lymph nodes and soft tissues of the face and chest. Modified Dieterle staining was performed on paraffin-embedded tissue sections, and the results were compared with those of hematoxylineosin stains and auramine-rhodamines and carbolfuchsin acid-fast stains. The acid-fast stain showed organisms in three of seven cases on initial review and five of s…

CNS infections with free-living amebas: neuroimaging findings.

OBJECTIVE: The purpose of this report is to describe the clinical history, treatment, pathology, and imaging in two cases of rare CNS infection caused by free-living amebas. The Naegleria fowleri and Acanthamoeba species cause primary amebic meningoencephalitis and granulomatous amebic encephalitis, respectively. We describe the neuroimaging findings of a case involving a nonspecific cerebral edema pattern in primary amebic meningoencephalitis and a case involving focal enhancing lesions in granulomatous amebic encephalitis. CONCLUSION: Primary amebic meningoencephalitis and granulomatous ame

Serious complications of endodontic infections: some cautionary tales.

While endodontic (dentoalveolar) abscesses can cause significant morbidity, in susceptible individuals they can pose life-threatening problems. This paper provides an overview of the more serious sequelae of endodontic abscesses, and provides examples of 'high risk' situations in practice in which these serious complications are more likely to occur.

Clinical trials on therapies for peri-implant infections.

Peri-implant infections are pathological conditions which are normally localized in the soft tissues surrounding an oral implant. They may range from a rather localized mucositis lesion reflecting the host response to a bacterial challenge to a more advanced lesion "termed peri-implantitis" when previously osseointegrated oral implants have been partially disintegrated; i.e. have lost alveolar bone around the implant. Usually these lesions start as a result of plaque accumulation and show similar progression as observed around teeth. Muscositis seems to be a prerequisite for the following peri…

Serological diagnosis of Schistosoma japonicum infections in China.

This paper reports the results of the dot enzyme-linked immunosorbent assay (dot-ELISA) for the serological diagnosis of 50 acute and 50 chronic Schistosoma japonicum-infected patients employing keyhole limpet haemocyanin (KLH) and soluble egg antigen (SEA) as the specific antigens. The sensitivity of both antigens for the detection of both forms of this infection was 100% and 90%, respectively, using a test serum dilution of 1:100. When the serum dilution was adjusted to 1:500, the sensitivity of the KLH dot-ELISA was 82% and 15%, respectively. Thus, the different manifestations of this disea

Fungal infections in patients with acquired immunodeficiency syndrome.

The incidence and severity of fungal infections appear to increase with progression of HIV disease. Because of the significant morbidity and mortality associated with the mycoses discussed, knowledge of the clinical syndromes, early diagnosis, and prompt institution of therapy are crucial for a favorable outcome. For disseminated or invasive fungal infections, suppressive therapy must be continued to prevent relapse.

Total knee arthroplasty infections associated with dental procedures.

Total knee arthroplasties are at risk for hematogenous seeding secondary to procedures that create a transient bacteremia. To define the risk of infection associated with dental surgery, a retrospective review of the records of 3490 patients treated with total knee arthroplasty by the authors between 1982 and 1993 was performed. Sixty-two total knee arthroplasties with late infections (greater than 6 months after their procedure) were identified, and of these, seven infections were associated strongly with a dental procedure temporally and bacteriologically. These seven cases represented 11% o…

[Results of early intervention in acute finger infections].

In the examination of 66 patients treated for acute felons, we classified the infections of fingers as (1.) cutaneous/subcutaneous felons; (2.) paronychia/subungual felons, (3.) thecal whitlow; (4.) articular/bony felons. Primarily, the therapy was based on early aggressive surgical treatment, and secondly on antibiotics. With the management described we achieved a good to very good result in 76%.

Immunoglobulin isotypes and IgG subclasses in recurrent infections.

The frequency of major immunoglobulin isotype and IgG subclass deficiencies among 74 children aged six months to 14 years with recurrent infections was studied. All children had at least five to six episodes of respiratory tract infections, while recurrent diarrhea had occurred in eight. Two selective and six partial IgA deficiencies were detected. IgG4, IgG3 and IgG2 deficiencies, either isolated or combined, were found in 13, nine and one patient respectively. Among these there were two combined deficiencies of IgA + IgG4, three of IgA + IgG3, one of IgA + IgG2 + IgG4, one of IgG1 + IgG3 and…

Opportunistic infections of the eye in immunocompromised patients.

All opportunistic infections of the eye have their origin in the suppression of the immune system of the host. The immunosuppression can be acquired through the human immune deficiency virus or as a result of immunosuppressive therapy in solid-organ transplant recipients with a maintenance therapy of cyclosporin and prednisone or during chemotherapy in patients with myeloproliferative disorders. Generally, in less than 2% could severe opportunistic infections of the eye, like cytomegalovirus (CMV) or herpes virus infection, be found in transplant and cancer patients with chemotherapy. About 35…

Is screening for genital infections in pregnancy necessary?

In recent decades, cervical screening for gonorrhea has been an integral part of antenatal care. Today, in most countries in the "western world", other microorganisms commonly found in the vagina are important causes of premature labor and are associated with perinatal and puerperal infections. These include Chlamydia trachomatis, Group B streptococci, herpes simplex virus, genital mycoplasmas and bacterial vaginosis. This paper discusses current strategies to prevent complications of these infections in pregnancy and at birth.

Bloodstream infections in liver transplant recipients receiving tacrolimus.

The microbial origin, timing, risk factors, and outcome of bloodstream infections (bacteremia and fungemia) were prospectively analyzed in 130 consecutive liver transplant recipients receiving tacrolimus-based immunosuppression; median followup was 3 yr. 22% (29/130) of the patients developed 36 episodes of bloodstream infections (0.28 episodes/patient). Bloodstream infections accounted for 36% (36/100) of all major infections. 81% (29/36) of bloodstream infections were due to bacteremia and 19% (7/36) due to fungemia (candidemia 14% and cryptococcemia 5%). Intravascular catheters were the mos…
