Anterior cruciate ligament functional instability. A static intra-articular and dynamic extra-articular procedure.

The static and functional results in a series of 50 patients evaluated at least two years after surgery were gratifying. None had any episodes of functional instability. Forty-six of the 49 who participated in sports prior to operation returned to all desired sports, including six patients who returned to intercollegiate and national team sports. The overall subjective and objective results were 30, excellent; 17, good; one, fair, and two, failure. Of the two failures, one had excellent static stability but significant pain due to pre-existing chondromalacia patella, and the other had a postsu

Chondromalacia patellae and the painful knee.

Chondromalacia patellae is a pathologic diagnosis. The initial diagnosis is patellofemoral pain syndrome, unless actual evidence of chondromalacia is available. The success of a nonoperative treatment program is correlated with improvement of thigh musculature balance. An arduous muscle-strengthening program is part of the treatment protocol. Only eight in a series of 100 patients required surgery. The physician's attitude and the patient's confidence are important elements in the success rate.

[Chronic traumatic arteriovenous fistulae (author's transl)].

In two patients with femoral arterio-venous fistulae angiography could show arterial and venous collaterals with differing circulation phases. When angiography shows retrograde flow in the artery distal to the fistula, the volume of arterial collateral circulation is unrelated to the peripheral blood-supply. Venous collateral circulation has to be included in the fistular circulation. Angiography permits distinguishing a venous by-pass circulation for the return of blood from the periphery. In one patient a large aneurysm of the iliac vein was found. Special features of angiography, particular

Weak ankles. A study of common peroneal entrapment neuropathy.

Twenty-three patients were seen with entrapment neuropathy in a two-and-a-half-year period. Symptoms consisted of pain, paresis, and paraesthesia in the distribution of the common peroneal nerve. Some degree of paresis was often present, which in five patients was severe enough to cause drop foot. In 20 patients decompression of the entrapped nerve at the neck of the fibula was quickly and completely successful. It is suggested that the ankle weakness which frequently follows sprains and other forced inversion injuries may often be at least partially due to entrapment of the common peroneal ne

Endoscopic Repair of a Partial Undersurface Proximal Hamstring Tendon Tear.

BACKGROUND: Proximal hamstring tendons are commonly injured among athletes, and when tendon tears are indicated for surgery, the gold standard has been open repair with suture anchors. However, endoscopic repair techniques like the technique described in this study for hamstring undersurface concealed crescentic injuries (HUCCIs) have been shown to have high patient satisfaction and limit perioperative complications. INDICATIONS: General indications for surgery for hamstring tears are 2-tendon, partial tears with >2 cm of retraction or 3-tendon complete tears that have been recalcitrant to no

Evaluating the physical examination and diagnostic workup for occult Neck Of Femur fractures in a district general hospital.

BACKGROUND: Occult Neck of Femur (NOF) fractures are radiographically elusive injuries. Undiagnosed, they lead to delayed treatment, displacement, and increased morbidity. While MRI is the gold standard for detection, CT is frequently used when MRI is unavailable. Physical examination manoeuvres, specifically the hip log-roll (HR) and straight leg raise (SLR), are often employed to guide decisions regarding cross-sectional imaging. This study explores the utility of these physical examinations in guiding the workup for occult NOF fractures. METHODS: A prospective service evaluation was perfor

Modified Percutaneous Endoscopic Interlaminar Discectomy through the Near-spinous Process Approach for L4/5 Disc Herniation: A Retrospective Clinical Study.

OBJECTIVE: Compared with traditional open surgery, percutaneous endoscopic lumbar discectomy (PELD) has the advantages of less trauma, faster recovery, and less postoperative pain, so it has been widely used in the field of spinal surgery. However, it still has the defect of intraoperative fluoroscopy occurrences, complications, and even the risk of damage to the spinal cord and nerve. This study aims to compare the clinical efficacy of modified percutaneous endoscopic interlaminar discectomy (MPEID) with percutaneous endoscopic transforaminal discectomy (PETD) in treating L4/5 lumbar disc her

High-load strength training compared with standard care treatment in young adults with joint hypermobility and knee pain: study protocol for a randomised controlled trial (the HIPEr-Knee study).

INTRODUCTION: Patients with generalised joint hypermobility, including knee hypermobility (GJHk), often experience knee pain and are typically managed with low-intensity strength training and/or proprioceptive training as part of standard care. However, not all patients experience satisfactory outcomes. High-load strength training may offer additional benefits, such as increased muscle cross-sectional area, neural drive and tendon stiffness, which may reduce pain and improve active knee joint stability during movement tasks and daily activities. So far, no randomised controlled trials (RCTs) h

Steep Posterior Tibial Slope and Excessive Anterior Tibial Translation Are Predictive Risk Factors of Primary Anterior Cruciate Ligament Reconstruction Failure: A Case-Control Study With Prospectively Collected Data.

BACKGROUND: Steep posterior tibial slope (PTS) and excessive anterior tibial translation (ATT) have been identified as important anatomic risk factors for anterior cruciate ligament (ACL) injury, which have raised concerns about clinical outcomes after primary ACL reconstruction (ACLR). PURPOSE: To investigate anatomic risk factors of primary ACLR failure and to determine the cutoff values of PTS and ATT for predicting primary ACLR failure. STUDY DESIGN: Case-control study; Level of evidence, 3. METHODS: Between November 2015 and May 2017, a total of 215 consecutive patients with clinically

History and Evolution of Lower Extremity Salvage.

There has been a paradigm shift in the management of lower extremity injury from amputations to more complex limb salvage techniques. Despite advancements in orthopedic surgery, trauma surgery, plastic surgery, and vascular surgery, not every patient is a candidate for complex lower extremity limb salvage. For this reason, novel advancements in plastic and reconstructive surgery have targeted not only limb salvage but also functional amputations. Here we review the history and advancements in microsurgical limb salvage and lower extremity amputation to discern how today's surgeon can better ai

Pathological Rupture of the Quadriceps Tendon in a Patient With Osteogenesis Imperfecta With Leg-length Discrepancy: A Case Report.

Quadriceps tendon rupture is typically caused by severe trauma, such as a fall, leading to substantial restriction of knee joint movements. However, pathological ruptures (one third of all quadriceps ruptures) can occur spontaneously or caused by minor trauma. Pathological ruptures are usually caused by chronic systemic diseases, such as systemic lupus erythematosus, chronic renal failure, and rheumatoid arthritis, and rarely by osteogenesis imperfecta (OI), a genetic disorder that affects the musculoskeletal system. Defective type 1 collagen formation in OI leads to bone fragility, ligamentou

Quality of Life after Open Extremity Trauma (QUINTET) study: An international, multicentric, observational, cohort study of quality of life following lower extremity open fractures.

INTRODUCTION: Open lower limb fractures are severe injuries with long-lasting consequences. Limited research has been conducted on the impact of these injuries on quality of life (QoL), internationally. METHODS: The Quality of Life after Open Extremity Trauma (QUINTET) study was designed as an international, multicentric, observational, cohort study of patients presented with open lower limb fractures. Demographic and clinical information was collected, along with repeated validated QoL measures. Primary outcomes were SF-12 and EQ-5D-3L, and secondary outcomes were soft tissue infection, deep
