Adolescent SBIRT Practices Among Pediatricians in Massachusetts.

OBJECTIVES: Adolescent Screening, Brief Intervention, and Referral to Treatment (SBIRT) in primary care is a key strategy to prevent, identify, and respond to substance use problems and disorders, including opioid and other drug addictions. Despite substantial investment in recent years to increase its implementation, few studies have reported on recent levels of SBIRT implementation among pediatricians. We aimed to assess self-reported use of the SBIRT framework with adolescent patients among Massachusetts pediatricians, and describe trends since an earlier survey. METHODS: We analyzed respo

One Size Does Not Fit All: Choosing Practical Cognitive Screening Tools for Your Practice.

Every year, millions of patients worldwide undergo cognitive testing. Unfortunately, new barriers to the use of free open access cognitive screening tools have arisen over time, making accessibility of tools unstable. This article is in follow-up to an editorial discussing alternative cognitive screening tools for those who cannot afford the costs of the Mini-Mental State Examination and Montreal Cognitive Assessment (see www.dementiascreen.ca). The current article outlines an emerging disruptive "free-to-fee" cycle where free open access cognitive screening tools are integrated into clinical

Progress in Primary Aldosteronism 2019: New Players on the Block?

Primary aldosteronism (PA) is characterized by hypertension caused by inappropriately high adrenal aldosterone secretion, consecutively low plasma renin, and an elevated aldosterone to renin ratio. It is nowadays the universally accepted main cause of endocrine hypertension. According to the most recent epidemiological data, PA is present in 5.8% of unselected hypertensives in primary care, 6-12% of hypertensives treated in hypertension centers, and up to 30% in subjects with resistant hypertension 1. Despite this high prevalence, a recent survey demonstrated that screening for PA is not unive

Performance characteristics of screening strategies to identify Lynch syndrome in women with ovarian cancer.

BACKGROUND: For women with ovarian cancer (OC), the optimal screening strategy to identify Lynch syndrome (LS) has not been determined. In the current study, the authors compared the performance characteristics of various strategies combining mismatch repair (MMR) immunohistochemistry (IHC), microsatellite instability testing (MSI), and family history for the detection of LS. METHODS: Women with nonserous and/or nonmucinous ovarian cancer were recruited prospectively from 3 cancer centers in Ontario, Canada. All underwent germline testing for LS and completed a family history assessment. Tumo

Identification, Referral, and Services for Individuals With Serious Mental Illness Across Multiple Jails.

Identification of serious mental illness (SMI) among those entering jail is the first step in diversion or appropriate services in jail. Although best practices guidelines for identifying SMI exist, many jails do not employ these standards. Researchers describe identification of SMI in the "practice as usual" and compare/contrast the results with a validated screening instrument for 2,961 individuals across eight jails. Overall, 20% scored positive on the screening instrument, and staff identification yielded an additional 16%. While the instrument was consistent in identifying the proportion

Correlates of sexually transmitted infection testing following women's release from jail.

Given incarcerated women's frequent transitions between jail and community, it is important to seize opportunities to provide comprehensive health care. A potential time to provide care might be when getting tested for sexually transmitted infections (STIs). Our objective was to determine the proportion of women receiving STI testing and correlates, following jail release. This secondary analysis was of one-year follow-up data from women who participated in a jail-based cervical health literacy intervention in three Kansas City jails from 2014 to 2016. Most (82%) completed the survey in the co

Strengthening the predictive power of screening for adverse childhood experiences (ACEs) in younger and older children.

BACKGROUND: There is increasing interest in routine screening for Adverse Childhood Experiences (ACEs) to help identify high-risk children who would benefit from interventions. However, there has not yet been sufficient research concerning which particular set of ACEs would be most predictive as a potential screening tool. OBJECTIVE: This study compared 40 Adverse Childhood Experiences (ACEs), covering 11 different conceptual domains, in their ability to predict trauma symptoms in childhood. PARTICIPANTS AND SETTING: The current study uses pooled data from three National Surveys of Children'

Making the Most of Clinical Encounters: Prevention of Child Abuse and Maltreatment.

Child abuse is a public health concern with great costs to children, families, and society. Prevention of child abuse and maltreatment is an important clinical skill. Providers can take advantage of the opportunity to offer prevention interventions in the health care setting. Identification of risk factors and signs and symptoms of abuse, referral to local resources, parenting education, and application of the public health prevention framework should be integrated into clinical encounters. Identification of sentinel injuries enables tertiary interventions to save lives. Primary interventions

Screening for Autism Spectrum Disorder in Premature Subjects Hospitalized in a Neonatal Intensive Care Unit.

Considering that the average age for diagnosis of autism spectrum disorder (ASD) is 4-5 years, testing screening methods for ASD risk in early infancy is a public health priority. This study aims to identify the risks for development of ASD in children born prematurely and hospitalized in a neonatal intensive care unit (NICU) and explore the association with pre-, peri- and postnatal factors. Methods: The children's families were contacted by telephone when their child was between 18 and 24 months of age, to apply the Modified Checklist for Autism in Toddlers (M-CHAT). The sample consisted of

[A summary of data of screening of the lower limb peripheral arterial diseases in the region of Northern Hungary].

INTRODUCTION: The screening tool for diagnosing lower extremity arterial disease (LEAD) is the assessment of the ankle-brachial index (ABI). In patients at risk for LEAD, the purpose of screening is to avoid major adverse limb events, such as amputation. However, resting ABI can easily produce a false negative result. AIM: In light of this, our goal was to test the usefulness of an easily performed, fast and cost-effective screening method and to determine the proportion of subjects without definitive diagnoses among patients screened in general practice (with special attention to groups havi
