Infant Social Avoidance Predicts Autism but Not Anxiety in Fragile X Syndrome.

Objective: Autism spectrum disorder (ASD), attention-deficit/hyperactivity disorder (ADHD), and anxiety are three of the most common childhood psychiatric disorders. Early trajectories of social avoidance have been linked with these psychiatric disorders in previous studies, but it remains unclear how social avoidance differentially predicts comorbid disorders in a high-risk genetic subgroup. Here, we delineate the association between trajectories of social avoidance from infancy and subsequent ASD, ADHD, and anxiety outcomes at preschool in children with fragile X syndrome (FXS), a well-chara

Motor and speech disorders in classic galactosemia.

Purpose To test the hypothesis that children with classic galactosemia and speech disorders are at risk for co-occurring strength and coordination disorders. Method This is a case-control study of 32 children (66% male) with galactosemia and neurologic speech disorders and 130 controls (50% male) ages 4-16 years. Speech was assessed using the Percentage of Consonants Correct (PCC) metric from responses to the Goldman-Fristoe Test of Articulation-2 and from a 5-min recorded speech sample, hand and tongue strength using the Iowa Oral Performance Instrument, and coordination using the Movement As

Unraveling the Link between Sleep Disturbances and Parkinson's Disease: A Narrative Review.

Parkinson's disease (PD) is characterized by both motor and nonmotor symptoms, many of which originate from the pathophysiology of the disease and its progression. Nonmotor symptoms are diverse and can often be more debilitating for patients than motor symptoms. Among these, sleep disorders are particularly significant, as they can occur several years before the onset of motor symptoms and have profound implications for the patient's quality of life (QoL). Sleep problems typically manifest at all stages of the disease and are frequently regarded as a separate entity. This analysis aims to expl

Premonitory urges in Tourette's syndrome.

OBJECTIVE: Tourette's syndrome traditionally has been viewed as a hyperkinetic movement disorder characterized by involuntary motor and phonic tics. Many patients, however, describe their tics as a voluntary response to premonitory urges. This cross-sectional study evaluated premonitory urges and related phenomena in subjects with tic disorders. METHOD: A total of 135 subjects with tic disorders, aged 8 to 71 years, completed a questionnaire concerning their current and past tic symptoms. Subjects were asked to describe and, if possible, localize their premonitory urges. The Yale Global Tic S

How is EMDR best conducted and adapted for trauma-focused work within bipolar disorder? A Delphi study.

Background: Individuals with bipolar disorder are at increased risk of exposure to traumatic events and associated psychological distress, including affective, cognitive, physiological, and mood-related difficulties. Eye movement desensitisation and reprocessing (EMDR) targets unprocessed trauma memory networks underlying these difficulties; however, guidance on adapting EMDR for this population is limited. This study aimed to establish practitioner consensus on adaptations to enhance the accessibility and effectiveness of EMDR for trauma in individuals with bipolar disorder. Methods: A three-r

Child Neurology: Creatine Biosynthesis Disorder in an Adolescent With Capgras Syndrome and Reduplicative Paramnesia.

This case report and discussion pertain to a 17-year-old female patient with developmental delay and acute onset of psychosis, diagnosed with guanidinoacetate methyltransferase (GAMT) deficiency, a rare creatine biosynthesis disorder. The patient experienced auditory hallucinations, paranoia, and delusional misidentification syndromes (Capgras syndrome and reduplicative paramnesia), following a viral respiratory illness. The neurologic examination was nonfocal but included intermittent negative myoclonus, generalized stiff movements, and a mechanical gait, which initially suggested catatonia o

Hysterical motility disorders during the First World War: cases from the orthopaedic department of the Red Cross Hospital in Zagreb.

PURPOSE: This paper examines how Vladimir Ćepulić at the orthopaedic department of the Red Cross Hospital in Zagreb described, understood and treated hysterical motility disorders between 1915 and 1917. METHODS: We analysed Ćepulić's article from 1919 on 107 cases of motility disorders among World War I veterans. Our analysis involved a formal examination of historical records, articles, and contemporary literature on conversion disorders, as well as a contextual analysis of Ćepulić's work. RESULTS: Ćepulić's report is a rare record of patients with conversion disorder treated within an orth

Clinical, cognitive, and morphometric profiles of progressive supranuclear palsy phenotypes.

The International Parkinson's and Movement Disorder Society (MDS) criteria for progressive supranuclear palsy (PSP) have broadened the clinical spectrum of the disease and established phenotypic characterization according to the predominant manifestation at onset. The objective of this study is to describe clinical/cognitive and imaging features of a monocentric cohort of PSP patients, highlighting different patterns of functional disability according to the assigned phenotype. We retrospectively reviewed clinical/imaging data of 53 PSP patients diagnosed with probable PSP according to the MDS

Evidence and Recommendation for Guanidinoacetate Methyltransferase Deficiency Newborn Screening.

Guanidinoacetate methyltransferase (GAMT) deficiency is an autosomal recessive disorder of creatine biosynthesis due to pathogenic variants in the GAMT gene that lead to cerebral creatine deficiency and neurotoxic levels of guanidinoacetate. Untreated, GAMT deficiency is associated with hypotonia, significant intellectual disability, limited speech development, recurrent seizures, behavior problems, and involuntary movements. The birth prevalence of GAMT deficiency is likely between 0.5 and 2 per million live births. On the basis of small case series and sibling data, presymptomatic treatment

Coexistence of Gait Disturbances and Chorea in Experimental Huntington's Disease.

Huntington's disease (HD) is an autosomal dominant neurodegenerative disease caused by an expanded CAG repeat. The clinical features are progressive motor dysfunction, cognitive deterioration, and psychiatric disturbances. Unpredictable choreic movements, among the most characteristic hallmarks, may contribute to gait disturbances and loss of balance in HD individuals. In this study, we aimed to investigate and characterize the gait abnormalities and choreic movements in a transgenic rat model of HD (tgHD). TgHD presents typical neuropathological, neurophysiological, and behavioral aspects mim

Persistent toe walking in autism.

The records of 954 ambulatory children presenting for initial evaluation to a university developmental pediatrician were reviewed for the prevalence of persistent toe walking and associated tight heel cords. The incidence of persistent toe walking (20.1%) and tight heel cords (12.0%) were found to be higher in 324 children with an autistic spectrum disorder but lower (10.0%/3.0%) in 30 children with Asperger syndrome. These results confirm the previously reported high incidence of toe walking in children with autism and with language disorders and also raise the possibility of a secondary orth

Response interruption and redirection for vocal stereotypy in children with autism: a systematic replication.

This study systematically replicated and extended previous research on response interruption and redirection (RIRD) by assessing instructed responses of a different topography than the target behavior, percentage of session spent in treatment, generalization of behavior reduction, and social validity of the intervention. Results showed that RIRD produced substantial decreases in vocal stereotypy. Limitations of this study were that behavior reduction did not generalize to novel settings or with novel instructors and that appropriate vocalizations did not improve.

'Study of several involuntary functions of the apparatus of movement, gripping, and voice' by Jean-Marc Gaspard Itard (1825).

Itard's 1825 paper, written while he was Chief Physician at the National Institute for Deaf-Mutes in Paris, demonstrates his empiricist approach to medicine. That is, Itard founded his medical practice on sense and experience rather than on surgery and medication. If all knowledge came through the senses, Itard reasoned, those lacking knowledge or social abilities could be improved by appropriate sensory stimulation. This concern with senses and society, along with his different approaches to men and women, his references to contemporary cures and his comparisons between humans and animals, do

Chronic anosognosia: a case report and theoretical account.

Unawareness of motor disorders (anosognosia) has often been reported after brain lesions, and it has been considered a temporary condition common in the acute and post-acute phases. The presence of anosognosia in a chronic phase (i.e. lasting more than few weeks) is a rare occurrence, thought to be the result of reasoning deficits which prevent patients from performing an adequate check of reality. Although this assumption is widely shared amongst researchers, only a few studies have actually addressed this issue. We report on the case of a patient (NS) who was still showing anosognosia for he

An akinetic-rigid syndrome, depression, and stereotypies in a young man.

A young man is presented who developed an akinetic-rigid syndrome shortly after a minor illness. Rituals and stereoptypies were prominent. At its most severe he was unable to feed himself. There was no response to L-dopa/cardopa treatment. A course of ECT was followed by a marked improvement in his condition. Attempts to stop ECT for more than a week have led to recurrence of his bradykinesia.

Alien hand syndrome of the dominant hand and ideomotor apraxia of the nondominant hand.

Following an extended infarction in the territory of the left anterior cerebral artery including the anterior corpus callosum, a 66-year-old right-handed man exhibited an alien hand syndrome on the right side and an ideomotor apraxia of the left hand. In addition to other typical symptoms of frontal lobe lesions, he presented with an interesting difference in the manifestation of perseveration in either hand. In the right hand, the perseveration was part of the alien hand syndrome, whereas in the left hand perseveration showed in the inability to switch promptly from one movement to another on

Cognitive impairment in amyotrophic lateral sclerosis and its relation to motor disabilities.

The performance of patients with amyotrophic lateral sclerosis (ALS) on selective neuropsychological tests was examined in regard to the applicability of such examinations to diagnosis. Eighteen patients with ALS, and 15 age- and education-matched controls were given a battery of tests designed to assess motor and intellectual functions. The ALS group displayed significantly lower scores on all tests than those in the control group. Correlation analyses on the several motor and neuropsychological results in ALS group revealed that there was a significant negative correlation between upper moto
