Outcomes following distal humeral fracture fixation with an extensor mechanism-on approach.

BACKGROUND: Distal humeral fractures have traditionally been managed with surgical approaches that disrupt the extensor mechanism. We hypothesized that an extensor mechanism-on approach for operative fixation of distal humeral fractures with parallel or orthogonal plate constructs would allow excellent healing, a motion arc of the elbow exceeding 100°, and maintenance of extensor mechanism strength. METHODS: Distal humeral open reduction and internal fixation (ORIF) was performed with either orthogonal or parallel plate constructs in seventy-nine elbows. Thirty-seven elbows were fixed via an

Precision of patient specific screw holes locating surgical guide and pre-bent plates osteosynthesis versus classical workflow in management of class III mandibular fracture.

BACKGROUND: Several treatment modalities have been reported in the management of mandibular fractures using an alternative computer-guided approach through the utilization of different designs of guiding devices. However, these computer-guided methods do not always guarantee accurate anatomical bone reduction. This study aimed to assess the reduction precision of the computer-guided mandibular fracture and internal fixation using screw holes locating surgical guide, as presented earlier in the orthognathic surgery field in various studies to be applied in the field of mandibular traumatology,

Triplicate post-traumatic sciatic nerve palsy: evoked potentials in the diagnosis.

Three episodes of sciatic nerve palsy occurred after open reduction and internal fixation of a fracture-dislocation of the left hip and pelvis in a 20-year-old female injured in a motor cycle accident. There were also ipsilateral open fractures of the tibia and fibula and an open knee injury. When the palsy first developed the patient was in a hip spica plaster cast extending from the costal margin to encase the whole left lower limb. At the time of the second and third episodes of palsy she was in a left below-knee cast and it was not possible to fully assess the function of involved muscles

Early versus delayed treatment of severe ankle fractures: a comparison of results.

A retrospective review of 202 closed Weber B bimalleolar or bimalleolar equivalent ankle fractures treated using open reduction and internal fixation (ORIF) from January 1, 1991, through January 1, 1994 was completed at our institution. A total of 105 ankles (52%) were treated using ORIF within 5 days of injury (early group), with a mean 1.5 days from injury to surgery. A total of 97 ankles (48%) were treated after 5 days (delayed group), with a mean of 13.6 days from injury to surgery. Operative time, length of hospital stay, postoperative range of motion, adequacy of reduction, wound and oth

Gas gangrene following implant removal after the union of a tibial plateau fracture: a case report.

BACKGROUND: Gas gangrene is an invasive, fatal anaerobic infection that is characterized by acute, rapid evolution and high mortality. Gas gangrene is often secondary to open fractures with deep wounds but is extremely rare in the patients undergoing elective surgery. Implant removal is a common elective operation in orthopedics after the union of fractures, and the complications of this surgery include infection, nerve injury and re-fracture. However, to the knowledge of the authors, there is no report in the literature on gas gangrene following implant removal. Here, we present a case study

Use of a synthetic bone void filler to augment screws in osteopenic ankle fracture fixation.

INTRODUCTION: Sufficiently stable constructs may be difficult to obtain with ankle fractures in patients with severe osteopenic bone. Augmentation of the osteosynthesis with a new synthetic bone void filler may help to solve this problem, and it can improve the clinical outcome. MATERIALS AND METHODS: A prospective, open-label study was performed in two surgical clinics in Norway. In 37 of 42 selected patients with Weber type B ankle fractures showing clinical and radiologic signs of osteopenic bone, at least one screw was found to be stripping during open reduction and internal fixation (ORI

[Recent fractures of the base of the 1st metacarpal bone. A study of a series of 138 cases].

Fractures on the base of the first metacarpal are uncommon lesions, affecting young subjects and have major social repercussions (an average of 3.5 months off work). The authors have analysed the clinical and radiological results of a series of 138 recent fractures of the base of the first metacarpal with a mean follow-up of 7 years (12 month-14 years). They used a classification into 5 types (Bennett's with a large fragment = 22%, Bennett's with a small fragment = 20%, Rolando = 15%, extraarticular fractures = 36%, Comminuted fractures = 6%) which are easy to recognize on standard x-rays, or

Anterior Approach for Fixation of Acetabular Fractures Using Anatomically Designed Plates: Maintenance of Reduction and Functional Outcomes at a Minimum Five-Year Follow-Up.

Background Acetabular fractures with quadrilateral plate involvement have been shown to have a high rate of complications. Anatomic suprapectineal plating systems have been developed to manage these injuries with good short-term outcomes. However, long-term maintenance of anatomical reduction and functional outcomes has yet to be established. The aim of this study is to maintain reduction and functional outcomes at a minimum of five years of follow-up. Materials and methods This is a retrospective cohort study from a prospective database examining patients aged over 16 years following fixation

Outcomes associated with secondary implant removal and arthrolysis in patients with a painful stiff shoulder after proximal humeral fracture fixation.

AIMS: Open reduction and plate fixation (ORPF) for displaced proximal humerus fractures can achieve reliably good long-term outcomes. However, a minority of patients have persistent pain and stiffness after surgery and may benefit from open arthrolysis, subacromial decompression, and removal of metalwork (ADROM). The long-term results of ADROM remain unknown; we aimed to assess outcomes of patients undergoing this procedure for stiffness following ORPF, and assess predictors of poor outcome. METHODS: Between 1998 and 2018, 424 consecutive patients were treated with primary ORPF for proximal h

Knee Fracture Increases TKA Risk After Initial Fracture Treatment and Throughout Life.

BACKGROUND: Knee fractures may lead to post-traumatic knee osteoarthritis and subsequent TKA in some patients. However, absolute risk estimates and risk factors for TKA in patients with knee fractures compared with those of the general population remain largely unknown. Such knowledge would help establish the treatment burden and direct patient counseling after a knee fracture is sustained. QUESTIONS/PURPOSES: (1) What is the short-term risk of TKA after knee fracture? (2) What is the long-term risk of TKA after knee fracture? (3) What are the risk factors for TKA in patients with knee fractu

[Long-term results of the treatment of Lisfranc fracture dislocation].

We conducted an ambispective cohort study of 83 patients with a diagnosis of Lisfranc fracture dislocation from 1993 to 2008. The lesions were classified into two groups: pure dislocations and fracture dislocations of the Lisfranc joint using the Hardcastle-Reschamer classification. The results included the following data: sociodemographic and epidemiologic variables, lesion-related variables, clinical parameters, and the following clinical and functional assessment scales: Baltimore Painful Foot Score, Creighton-Nebraska Health Foundation, American Orthopaedic Foot and Ankle Society (AOFAS),

Pediatric Mandibular Fracture Management: Intraoperative Thermoplastic Splint Fabrication and Circummandibular Wires, without Maxillomandibular Fixation.

Management of mandibular fractures often involves the use of maxillomandibular fixation (MMF) to attain immobility of the fractured segments. This can be used as a primary treatment modality or as an adjunct in fracture management. This technique, however, has its drawbacks due to the great burden of care imposed on patients. In the following case, fixation of a pediatric open mandibular body fracture was attained without the use of MMF, and bone union was achieved. Due to age, safety concerns, long-distance travel, and parent's preference, the routine management of this type of fracture with

Anabolic agents for treating postmenopausal osteoporosis.

The main efficacy criterion for drugs against osteoporosis is protection against fractures. Many resorption-inhibiting agents meet this criterion, including estrogens, alendronate, risedronate, raloxifene, calcitonin, and calcium-vitamin D supplements). Conversely, among anabolic agents, only parathyroid hormone (PTH) is known to reduce the fracture risk, the mechanism being increased bone matrix production by osteoblasts with no alterations in the mechanical properties of bone. Although fluoride salts induce a marked increase in bone mineral density (BMD), there is no evidence that this prote

Irreducible Lisfranc Injury Caused by Tibialis Anterior Tendon Entrapment: Surgical Strategy and Literature Review.

Lisfranc injuries are difficult to treat, particularly when reduction is blocked by soft-tissue interposition. Tibialis anterior tendon entrapment is a rare cause of irreducible Lisfranc dislocation, with only nine cases reported since 1950. We report the tenth case in a 65-year-old woman who sustained a Lisfranc fracture-dislocation after a trampoline accident. Emergency treatment consisted of closed reduction and temporary percutaneous fixation, followed by staged open reduction and internal fixation after soft-tissue swelling subsided. At definitive surgery, the tibialis anterior tendon was

Mechanical jaw restriction after zygomaticomaxillary complex fracture: an unusual coronoid-zygomatic relationship.

Displacement of an intact mandibular coronoid process lateral to the zygomaticomaxillary complex (ZMC) is a rare complication of ZMC fractures. We report a case of a woman in her early 30s with malar flattening and inability to achieve full mouth closure following a fall from an electric scooter. Her CT scan revealed a comminuted right ZMC fracture with the ipsilateral intact coronoid process displaced laterally to the ZMC. This rare injury pattern produced mechanical restriction of mandibular movement in both opening and closure. Urgent operative management of the ZMC fracture allowed for spo

Patient-specific approach to the correction of a post-traumatic zygomatic deformity.

Fractures of the zygomaticomaxillary complex (ZMC) represent one of the most frequent midfacial injuries and, if untreated, can result in significant functional and aesthetic sequelae. While open reduction and internal fixation is the standard of care for acute fractures, late presentations remain challenging due to bone remodelling and altered anatomical landmarks. Secondary corrective options include skeletal repositioning via zygomatic osteotomy or implant-based camouflage. We present the case of a young female patient with a displaced ZMC fracture left untreated following urgent cranioplast

Extraoral approach to a bilateral unfavourable mandibular fracture under local anaesthesia: A case report.

OBJECTIVE: To present a feasible treatment method for fractures of severely atrophic mandibles in high-risk patients undergoing general anaesthesia (GA). CASE REPORT: A 77-year-old man presented with bilateral unfavourable fractures in an atrophic mandible. Pre-anaesthetic consultation revealed that the patient was at high risk for GA due to significant medical comorbid conditions of the patient, such as advanced age, chronic obstructive respiratory disease, diabetes mellitus and atherosclerosis. Thus, a modified treatment, consisting of open reduction and internal fixation using a combinatio
