Mycetoma of the foot caused by Cylindrocarpon lichenicola in an immunocompetent traveler.

Cylindrocarpon is a cosmopolitan soil fungus, which rarely causes human disease. It has infrequently been reported as causing keratitis, mycetoma, osteomyelitis and peritonitis in chronic peritoneal ambulatory dialysis patients and disseminated infection in leukemic neutropenic hosts. This report describes a case of invasive infection caused by Cylindrocarpon lichenicola, localized in the right foot of an otherwise immunocompetent traveler.

Aeromonas hydrophilia Infection in an Immunocompromised Host.

Aeromonas hydrophila is an anaerobic, gram-negative rod-shaped bacterium that is commonly found in aquatic environments. This organism can cause a wide range of infections in humans, including gastroenteritis, septicemia, and skin and soft tissue infections. We herein describe a case of bacteremia due to Aeromonas hydrophila secondary to a soft tissue infection of the fingers that resulted in osteomyelitis in an immunocompromised patient.

Primary zygomycosis of the pericardium in an immunocompetent host.

The fungi in the order Mucorales commonly target immunocompromised hosts, especially diabetics and produce fulminating respiratory and/or central nervous system infections, which are often fatal. We report primary zygomycotic pericarditis accompanied by osteomyelitis of the sternum in an immunocompetent female: a rare site and a rare setting. The disease was clinically thought to be of tuberculous aetiology and had evaded diagnosis for a year.

Marble bone disease: a review of osteopetrosis and its oral health implications for dentists.

Osteopetrosis is one cause of osteosclerosis and may result in such serious oral complications as osteomyelitis and exposed necrotic bone. Dentists should be aware of patients with the disease because of its effect on osteoclast function, which results in impaired wound healing. The purpose of this paper is to review the causes, pathogenesis and differential diagnosis of osteopetrosis and to provide guidance to dentists on the management of patients with osteopetrosis.

Scapholunate dissociation caused by gouty arthritis of the wrist. Case report.

Gouty arthritis of the wrist is rare, and may be associated with scapholunate dissociation. To our knowledge, only two cases have been reported so far. In this report, we describe a 40-year-old patient with scapholunate dissociation caused by acute gouty arthritis of the wrist. His clinical findings and radiographs mimicked infectious arthritis or osteomyelitis of the carpal bones.

[SAPHO syndrome. Case description of 3 patients with acne conglobata and osteoarticular symptoms].

We report three new cases of the SAPHO syndrome. This acronyme consists of synovitis, acne, pustulosis, hyperostosis and osteitis. Symptoms of this syndrome, which may not all be present, are pustulotic skin diseases (pustulosis palmoplantaris or severe acne) associated with osteoarticular lesions (mainly sternoclavicular hyperostosis, spondylarthropathies or chronic recurrent multifocal osteomyelitis). The dermatological aspects of this syndrome are discussed in detail.

Charcot arthropathy of the first metatarsophalangeal joint.

The authors present an unusual case study of a Charcot joint of the first metatarsophalangeal articulation as well as a successful tibial sesamoidectomy for an associated chronic ulceration. Serial radiographs document the destructive disease process. The authors also address the etiologies of peripheral neuropathy, diagnostic tests to differentiate between Charcot joint and osteomyelitis, and both conservative and surgical management of Charcot joints.

A transient granulocyte killing defect secondary to a varicella infection.

A varicella infection in a previously healthy young girl was complicated by bacterial sepsis, arthritis, and osteomyelitis in multiple locations. This secondary complication caused by Staphylococcus aureus was associated with a transient defect in granulocyte function and an alteration in the representation of CD4 and CD8 positive lymphocyte subpopulation. The mechanism responsible for secondary bacterial infections following varicella may be due to transient defects in granulocyte function.

Indolent ulcer and discharging sinus on the eyelid of healthy Indian children and an adult: Orbital tuberculosis, a spot-diagnosis.

Five cases of tuberculous osteomyelitis of the fronto-zygomatic (F-Z) region presented with a non-healing ulcer or discharging sinus in the eyelid skin in healthy children and an adult. Lack of awareness about peri-ocular manifestations of extra-pulmonary tuberculosis and delayed referral to specialists, along with poor compliance to long-term ATT, could be the reason for its underreporting in India.

A linear fracture and meningitis associated with non-infected cephalohematoma in a neonate.

We present a neonate with cephalohematoma complicated by a linear skull fracture and Staphylococcus epidermidis meningitis. Clinicians, especially neonatologists, should be aware that a cephalohematoma in the newborn infant with a history of vacuum-assisted delivery could be the origin or trigger point of the infection either as sepsis, meningitis or osteomyelitis. The utmost importance of screening studies should be emphasized in order to be aware of the pathogenic potential of cephalohematomas.

[Treatment of an infected pseudoarthrosis defect by callotasis of the bone fragments].

The authors demonstrate a method of filling a large total bone defect caused by resection of the bone on account of chronic osteomyelitis. The defect was filled with a regenerate prepared by calotassis of the fragment which was distracted from the proximal part of the bone to its distal part. It proved thus possible to fill a 16 cm long defect. The cosmetic, anatomical and functional result is excellent.

Radionuclide bone scintigraphy in pediatric orthopedics.

Radionuclide bone scintigraphy is highly sensitive and specific for diagnosing the musculoskeletal disorders of childhood. Conditions such as neonatal osteomyelitis, septic arthritis, diskitis of childhood, Legg-Calvé-Perthes disease, the osteochondroses, the "toddler's fracture," sports injuries, spondylolysis, myositis ossificians, and reflex sympathetic dystrophy are readily defined. High-quality state-of-the-art scintigraphy is essential in infants and young children.

Septic arthritis in childhood.

BACKGROUND: The purpose of the present study was to determine whether there was a difference between septic arthritis (SA) combined with osteomyelitis and SA alone with regard to clinical and laboratory findings, such as symptoms on admission, age, sex, joint involvement and isolated micro-organisms, and a relationship between age and joint involvement in SA. In addition, we also aimed to determine the prognostic factors in SA. METHODS: The clinical and laboratory findings of 40 patients who were diagnosed with SA in our hospital were reviewed retrospectively. The diagnosis of SA was made acc…

Pott's Puffy Tumor: A Potentially Deadly Complication of Sinusitis.

Pott's puffy tumor (PPT) is a rare but potentially deadly complication of bacterial sinusitis that consists of subperiosteal abscess and osteomyelitis of the frontal bone. It is best diagnosed with computerized tomography (CT) with intravenous contrast or magnetic resonance imaging and treated with early broad-spectrum antibiotics and surgical intervention. We describe a case of PPT in a 7-year-old patient and present the CT images.

Pott's puffy tumor following an insect bite.

Pott's puffy tumor, a feature of osteomyelitis of the frontal bone, is a rare entity, especially in adults. Sir Percival Pott originally described this condition as a complication of trauma to the frontal bone. This is also a recognized complication of fronto-ethmoidal sinusitis. We present a rare case of Pott's puffy tumor caused by an insect bite presenting initially as a preseptal cellulitis and explore its pathogenesis and management.
