Volume-pressure relationships during development of mineralocorticoid hypertension in man.

Serial measurements of urinary sodium excretion, sodium space, plasma volume, and plasma renin concentration were made during the development of hypertension in patients who were exposed to an excess of endogenous or exogenous mineralocorticoid activity. Five patients with primary aldosteronism due to adenoma were followed during spironolactone treatment, for 35-55 days after the drug had been stopped, and finally, after surgery. Blood pressure rose continuously after stopping spironolactone. Sodium balance, however, showed an initial phase of sodium gain, followed by a phase of gradual sodium…

Sodium, renin, aldosterone, catecholamines, and blood pressure in diabetes mellitus.

Interrelations among plasma renin activity (PRA), aldosterone and cortisole levels, 0lood volume, exchangeable sodium, urinary catecholamines, and blood pressure were studied in 35 normal subjects and 60 age-matched non-azotemic patients with diabetes mellitus (60% with hypertension, 15% with orthostatic hypotension). Basal PRA, plasma aldosterone, cortisol, blood volume, plasma potassium, and urinary electrolytes were comparable in diabetic and normal subjects. Diabetic patients, however, had a 10% increase in body sodium (P less than 0.01), and 8% of them showed normal postural PRA responses…

Prolonged continuous monitoring of intracranial pressure in severe Reye's syndrome.

Evidence from both clinical and pathological investigations suggests that increased intracranial pressure (ICP) is a significant factor in the mortality of patients with Reye's syndrome. This, coupled with a critical review of our previous 22 patients, which failed to document the efficacy of exchange transfusion, led us to include continous ventricular pressure monitoring in the supportive care of children with Reye's syndrome. To date, three children, ages 3 to 5 years, have been managed with continuous ICP monitoring. All had stage IV coma by both clinical and electroencephalographic criter…

Lower esophageal sphincter pressure in women using sequential oral contraceptives.

Lower esophageal sphincter pressure, basal gastric pH, and fasting plasma gastrin were measured sequentially in female volunteers who were using oral contraceptives. No difference in basal gastric pH or fasting plasma gastrin was observed during any of the three selected periods studied. Lower esophageal sphincter pressure was the same during menses (20.8 +/- 1.7) when the volunteers took no medication during the phase of the cycle when the volunteers were ingesting ethinylestradiol (18.3 +/- 1.7). Lower esophageal sphincter pressure decreased significantly (P less than 0.01) to 9.4 +/- 1.2 du…

Prognostic value of central venous pressure in acute myocardial infarction.

The investigations were carried out in 296 patients with myocardial infarction (238 men and 58 women) aged 24-91 years, admitted to an intensive care unit on the 1st or 2nd day of the disease. In all patients the catheter for determination of central venous pressure was introduced into the right atrium through the cephalic vein the the first 100 cases, and through the subclavian vein in 196 cases. The catheter was kept in the atrium for 3-4 days. Values of c.v.p. between 50 and 120 mm Hg were accepted as normal.

Systemic arterial blood pressure and its relevance to optometric practice.

The measurement of systemic blood pressure is a very significant detection test that can provide a great deal of information for the optometrist. The blood pressure readings can be critical in aiding the practitioner's evaluation of both the patient's visual health and related systemic condition and in indicating the possible need for referral. This paper presents a general survey of the benefits blood pressure measurements can provide in the optometric practice.

[Nonaneurysmal vascular pressure lesions of the cranial nerves (author's transl)].

Four cases of facial spasm and 1 case of oculomotor paresis are described. The source of the disorder in all 5 cases is mostlikely not an aneurysm of the vessels of the base of the brain. The literature is discussed and thereby it is shown that mechanical disturbances of other cranial nerves (II, V, VI, IX, XII) can be caused by similar vascular (nonaneurysmal) abnormalities.

Oxygen plus pressure plus time: the etiology of bronchopulmonary dysplasia.

Ten infants developed the chronic pulmonary syndrome designated bronchopulmonary dysplasia, following artificial (assisted) ventilation using a volume-regulated positive-pressure respirator. Despite previous reports implicating elevated oxygen concentrations for prolonged periods of time, only two infants required more than 80% oxygen for more than 24 hours, and only one infant required 60% oxygen for more than 100 hours. "Immature" lungs when exposed to inspired oxygen concentrations over 40% for as little as three days via positive-pressure ventilation may develop bronchopulmonary dysplasia.…

Normal-pressure hydrocephalus: diagnosis and patient selection for shunt surgery.

Currently accepted modes of clinical and radiologic evaluation were analyzed retrospectively in 55 patients with "normal-pressure" hydrocephalus on whom a cerebrospinal fluid shunting procedure was done. When applied alone, each criterion neither reliably differentiated normal-pressure hydrocephalus from cortical atrophy nor indicated in a significant number of cases which patients would benefit from shunting. Therefore, future prospective evaluations should include clinical history, physical and neurologic examination, skull radiography, echoencephalography, psychometric testing, brain scanni…
