Computer simulation of glenohumeral and patellofemoral subluxation. Estimating pathological articular contact.

Analytic stereophotogrammetry and an interactive computer graphics program were used to obtain first order assessments of joint contact patterns in patellofemoral and glenohumeral joints, simulating normal and abnormal articulations. Precise (90 microns accuracy) computer graphic representations of the humeral head, glenoid, patella, and femoral articular surfaces were obtained from cadaver knees and shoulders. These surface representations were then manipulated into an articulated position, and joint contact areas computed by a proximity criterion. Pathologic states were then simulated, and c

Isolated and painless (?) atrophy of the infraspinatus muscle. Left handed versus right handed volleyball players.

The suprascapular nerve originates from the upper trunk of the brachial plexus or less frequently from the root of C5. It runs a short way and crosses the suprascapular notch. It innervates the supraspinatus muscle and the acromioclavicular and glenohumeral joints. Then, it crosses the lateral edge of the spine of the scapula passing through the spinoglenoid notch, and innervates the infraspinatus muscle. These are potential sites of injury to the suprascapular nerve. Three cases of suprascapular nerve entrapment causing an isolated infraspinatus muscle atrophy in volleyball players were studi

[Therapeutic possibilities of shoulder arthroscopy at the orthopedic clinic of the Charité and the surgical clinic Berlin-Weissensee].

We report on arthroscopic procedures of 279 shoulder joints which were done in two hospitals of East-Berlin. We stress out the intention to arthroscopic diagnostic and simultaneous surgery. Arthroscopic surgery was mainly performed as resections and exstirpations. In cases of luxation and severe rotator cuff tears the open revision was necessary to perform.

Isolated avulsion fracture of the lesser tuberosity of the humerus: report of two cases.

Isolated avulsion fractures of the lesser tuberosity of the humerus are exceptionally rare. Two cases are reported here, each involving a 12-year-old boy. The mechanism of injury appears to be a strong external rotatory force applied while the arm is at maximum external rotation and approximately 60 degrees of abduction. In children and adolescents the fracture most likely occurs through the apophyseal plate of the lesser tuberosity (traction epiphysis). In adults, clinical differentiation from the far more common calcific peritendinitis of the rotator cuff can be difficult. Whereas large, dis

[Electromyographic study associated with rotator cuff tears].

To determine the pathophysiological condition of rotator cuff tear, 23 cases were observed using electromyogram of the shoulder girdle muscles during elevation of the arm. The correlation between the action potential of these muscles and the angle of the arm was analyzed using computer in order to identify the pattern of that. The pattern of the electromyogram of the affected side with the ability to elevate the arm above 90 degrees was similar to that of the sound side. Differences were seen between the affected side with the inability and the sound side in the electromyogram of trapezius, de

Acquired isolated axillary neuropathy: an unusual complication of quadriplegia.

This report describes a rare case of acquired axillary neuropathy in a 17-year-old C-5 quadriplegic man who developed right axillary neuropathy after sleeping for six hours in a far-right lateral decubitus position. Two days after the onset of shoulder weakness, his shoulder abduction strength was found to have decreased 1 1/2 grades from measurements made 2 weeks before. Electrodiagnostic studies demonstrated no voluntary activity of the deltoid and no deltoid evoked response on stimulation at Erb's point, while shoulder nerve conduction latencies to other C-5 and posterior cord innervated mu

Symptomatic shoulder instability due to lesions of the glenoid labrum.

Lesions of the glenoid labrum can be associated with and cause shoulder instability and symptomatology by (1) allowing the shoulder to dislocate recurrently (anatomical instability); (2) allowing the shoulder to subluxate (anatomical instability); and (3) allowing the shoulder to click, catch, and lock secondary to partially attached fragments becoming interposed between the articular surfaces (functional instability). The latter two clinical entities are usually associated with "lesser" labral damage and diagnosis can be difficult. Six illustrative cases are presented and salient diagnostic a

Hemi-Hamate Arthroplasty in Delayed Proximal Interphalangeal Joint Fracture-Dislocations: Technical Considerations and Early Outcomes From a Case Series.

PURPOSE: To evaluate the early clinical and radiographic outcomes of hemi-hamate autograft (HHA) reconstruction for unstable dorsal proximal interphalangeal joint (PIPJ) fracture-dislocations with severe comminution involving more than 50% loss of the volar articular surface. METHODS: A retrospective case series of 4 patients treated with HHA reconstruction between June and December 2025 was performed. All injuries involved highly comminuted fractures that were unsuitable for conventional open reduction and internal fixation. The surgical technique aimed to restore the volar buttress of the m

Innovative intramedullary nail system with internally locking: A safe alternative fixation technique for humerus shaft fracture that reduces the risk of the iatrogenic radial nerve injury and surgical time.

PURPOSE: Humeral shaft fractures may be managed conservatively or operatively; however, increasing non-union and complications after non-operative treatment have renewed interest in intramedullary fixation. Innovative intramedullary nails with distal internally locking, requiring neither fluoroscopy nor external guidance, aim to reduce surgical time, simplify the procedure, and minimise the risk of iatrogenic radial nerve injury. This study evaluates the clinical and radiological outcomes of a humeral nail utilising distal internally locking. MATERIAL AND METHOD: A retrospective review includ

Endoscopy for neurogenic thoracic outlet syndrome: effectiveness and safety assessments.

BACKGROUND: Thoracic outlet syndrome (TOS) is a complex and multifactorial condition characterized by the compression of neural, arterial, or venous structures. Traditional surgical approaches use open dissections presenting substantial recurrence rates of 15-20%. This study aimed to explore the safety and efficacy of endoscopic surgery for neurogenic TOS treatment, given the evolution of surgical techniques toward minimally invasive approaches. METHODS: A study was conducted involving 30 patients who were resistant to conservative treatment, presented with objective images of neurogenic comp

Outcomes of reverse total shoulder arthroplasty vs. other surgical fixation methods for proximal humerus fractures in elderly patients.

BACKGROUND: Proximal humerus fractures (PHFs) are among the most common osteoporotic injuries in elderly patients, typically resulting from low-energy mechanisms such as ground-level falls. Multiple surgical options exist, including reverse total shoulder arthroplasty (rTSA), open reduction and internal fixation (ORIF), and hemiarthroplasty (HA), but the optimal treatment remains debated. This up-to-date systematic review and meta-analysis aimed to evaluate the comparative effectiveness of rTSA, HA, and ORIF for treating PHFs in the geriatric population. METHODS: A systematic review and meta-

Radiographic pseudosubluxation of the shoulder in pediatric proximal humeral fractures.

This study was undertaken to determine the incidence, need for intervention, and time to resolution of pseudosubluxation of the shoulder in pediatric proximal humerus fractures. One hundred and ninety-nine radiographs (199 x-rays) were analyzed for pseudosubluxation of the shoulder following pediatric proximal humeral fractures. Pseudosubluxation occurs when the center of the humeral head aligns with the inferior one-fourth of the glenoid. Fourteen patients met the inclusion criteria for pseudosubluxation. The nonoperative cohort consisted of 100 females and 93 males and the operative cohort c

Oral Methadone for Patients With Neuropathic Pain Due to Neoplastic Brachial Plexopathy.

Background: The brachial plexus nerves originate from the cervical (C5-C8) and first thoracic (T1) spinal nerves, and innervate muscles and skin of the chest, shoulder, arm and hand. Brachial plexus injuries can occur as a result of shoulder trauma and inflammation. Malignant tumors can also cause neoplastic brachial plexopathy (NBP), and refractory neuropathic pain is the most common symptom of NBP. Methadone is a synthetic opioid with high efficacy as an opioid-receptor agonist, and its inhibitory effects on N-methyl-D-aspartate (NMDA) may play a role in pain relief. However, there is a need
