A retrospective analysis of patients deemed ineligible for surgery in a multidisciplinary complex spine conference.

PURPOSE: Assess patient experiences and outcomes following a multidisciplinary conference recommendation against surgery, including the proportion who later pursued surgery elsewhere and their outcomes. METHODS: We performed a retrospective analysis of patients advised against surgery during our internal multidisciplinary ASD conference. A phone-based survey gathered on patient satisfaction, effectiveness of alternative treatments, rates of operations at outside hospitals, and subsequent rates of complications, reoperations, and readmissions among those who underwent surgery. RESULTS: One hu

Is Hip and Lumbar-Spine Pain Related to Physiological Characteristics in Young Elite Gymnasts?

PURPOSE: Gymnastics performances entail complex and extensive movements that require specific body morphology, strength, and flexibility that may lead to hip and lumbar-spine injuries. The present study aimed to evaluate the prevalence of hip and lumbar-spine pain and their associated physiological characteristics in young gymnasts. METHODS: Two hundred seventy-four female rhythmic, acrobatic, and artistic gymnasts age 9 to 16 years were physically examined for hip and lumbar-spine pain and reported pain experienced during training on a visual analog scale. In addition, gymnasts were assessed…

Digital twin for the analog scoliometer: advancements in telehealth for paediatric spine deformity care.

In scoliosis care, a spine specialist will measure the lateral curvature of the spine from a radiograph and measure the torso rotation non-radiographically with an analog scoliometer directly on the patient at every surveillance appointment. While radiographs can be taken at any imaging centre and transferred to the treating spine clinic, the patient must visit in-person for monitoring the progression of structural rotation. Until now, distance measurement of the Angle of Trunk Rotation (ATR) has not been possible as no "virtual" scoliometer exists. This study describes the development of the …

Segmental asymmetry and multiplanar motion of the lumbar spine: Cadaveric insights into spinal pathologies.

The lumbar spine plays a critical role in supporting multiplanar motion and distributing loads during daily activities. While global spinal symmetry is often assumed in biomechanical assessments, segmental asymmetries may exist and contribute to conditions such as low back pain (LBP). The present study investigates the multiplanar kinematic behavior and torque asymmetry of the lumbar spine using cadaveric specimens, focusing on identifying segmental asymmetries that may be masked in global motion analysis. A novel experimental setup combining a 6°-of-freedom robotic system with optical metrolo…

The importance of the posterior osteoligamentous complex of the lumbar spine: dogma changing biomechanical insights.

BACKGROUND: During full flexion of the spine, the paraspinal muscles are largely inactive. This suggests that passive structures like the posterior osteoligamentous complex (POLC), consisting of interspinous and supraspinous ligaments and the spinous processes, play a key role in spinal stability and protection of the spinal column. The POLC, however, is often resected or damaged during spinal decompression surgeries, whereas the biomechanical implications of this resection or damage are not yet fully understood. METHODS: A stepwise reduction study was performed on three fresh frozen cadaveri…

Impact of kyphosis exceeding 30° on lumbar spine in patients with post-traumatic kyphosis.

This study investigated the impact of post-traumatic kyphosis (PTK) on the lumbar spine using a local Cobb angle of 30° as the cutoff point. The measurements included the local Cobb angle, lumbar lordosis angle (L1-L5), intervertebral space angle (IVSA), lumbar disc degeneration, and posterior wall height loss (PWHL). Patient characteristics such as sex, age, disease duration, American Spinal Injury Association (ASIA) grade, fracture site, bone mineral density (BMD), Oswestry Disability Index (ODI), and visual analogue scale (VAS) score were also recorded. Patients were divided into two groups…

Are Mildly Disabled Patients Appropriate for Spine Bundles? An Application of the Operative Value Index.

BACKGROUND: Many studies have evaluated the effect of preoperative disability status on functional outcomes following spine surgery. However, no research has compared the "value" (outcomes per dollar spent) of surgery for patients with different levels of diagnosis-specific disability. METHODS: We retrospectively reviewed 429 patients who underwent neurosurgical anterior cervical discectomy and fusion. Time-driven activity-based costing was used to calculate total intraoperative costs. Neck Disability Index (NDI) scores were recorded at baseline and 3 months postsurgery. Patients were categor…

[Results of manual therapy in degenerative spine diseases with and without radiculopathy. (Literature review)].

The prevalence of degenerative diseases of the spine, including neck and low back pain, which reduce the ability to work and lead to disability, is increasing. We have studied the materials of clinical studies, meta-analyses and systematic reviews devoted to the evaluation of the results of manual therapy in patients with degenerative spine diseases, including those who underwent surgical treatment. The analysis (38 sources were analyzed, including 15 meta-analyses and 21 systematic reviews) revealed the ability of manual therapy to influence the level of acute and chronic pain (local and proj…

A unique geometrical representation of the cervical spine based on meta-analysis-derived descriptors.

This study was aimed at developing a unique geometrical model of the cervical spine, representative of a global population, for use in designing spinal fixation instrumentation. The purpose of the work is to (i) develop a geometrical model of the cervical spine representing a global population of subjects and (ii) demonstrate the utility of the model for design and manufacture of spinal fixation instrumentation. Several methods for quantifying the cervical lordosis exist such as Jackson's physiological stress line method, Harrison's posterior tangent method and Ishihara's cervical curvature in…

The effect of smartphone texting on cervical spine sagittal alignment in healthy young adults.

BACKGROUND: Never before has technology been so universally accepted as the modern-era cell phone, the smartphone. Spine surgeons have noticed a rise in patients in their offices complaining of neck and upper back pain. Many patients are of the younger age group, and one thing they all seem to have in common is prolonged smartphone use. While using a smartphone, the cervical spine demonstrates flexion angles ranging from 15 to 60 degrees, with more pronounced deviations from the neutral position observed during sitting compared to standing, and while texting compared to web browsing or video w…

Imaging in Pott's spine: A review of typical and atypical imaging features and diagnostic challenges.

Pott's spine is associated with high morbidity, long-term disabling sequelae and even mortality. The classical paradiscal form, where there is destruction of end plates of adjacent 2-3 vertebral bodies along with intervening disc involvement and associated paravertebral abscess is well recognized. However, due to its varied clinical and radiological presentations, it remains a diagnostic challenge often resulting in delay in diagnosis. This is especially true of countries where tuberculosis has been almost eliminated but now are witnessing increase in incidence both due to immigration and HIV …
