Editorial: Combatting the Intergenerational Trauma of Structural Racism Through Practice, Policy, and Research.

Several recent studies have investigated the mental health consequences of structural racism. Structural racism has been defined as "macro-level societal conditions that limit opportunities, resources, and well-being of less privileged groups on the basis of race/ethnicity and/or other statuses, including but not limited to, gender identity, sexual orientation, disability status, social class or socioeconomic status (SES), religion, geographic residence, national origin, immigration status, limited English proficiency, physical characteristics, or health conditions."1 Researchers have hypothes

Pharmacotherapy of comorbid mood, anxiety, and substance use disorders.

Mood and anxiety disorders commonly co-occur with substance use disorders. Exploration of the neurobiology of substance use disorders and mood and anxiety disorders have found that the neural circuitry in mood, anxiety, and substance use disorders is clearly overlapping. These discoveries have encouraged the exploration of a number of pharmacotherapeutic agents in the treatment of co-occurring mood, anxiety, and substance use disorders. In this article, recent data on the pharmacotherapeutic treatment of mood and anxiety disorders in individuals with substance use disorders are reviewed. Some

Posttraumatic stress disorder in eating disorder patients: The roles of psychological distress and timing of trauma.

Exposure to traumatic events may be a risk factor for subsequent development of an eating disorder (ED). In a previous study, we showed that trauma exposure impacted symptom load in ED patients. We also saw an effect of trauma on general psychological distress. The aim of the present study was to investigate the association between Posttraumatic stress disorder (PTSD) and ED severity, to focus on the mediating role of psychological distress for the association, and to assess the role of timing of trauma in relation to emergence of ED. Participants were Swedish adult ED patients with a history

Trauma-Focused Art Therapy in the Treatment of Posttraumatic Stress Disorder: A Pilot Study.

Research showed that more than 30% of patients with Posttraumatic Stress Disorder (PTSD) do not benefit from evidence-based treatments: Trauma-Focused Cognitive Behavioral Therapy (TF-CBT) or Eye Movement Desensitization and Reprocessing (EMDR). These are patients with prolonged and multiple traumatization, with poor verbal memory, and patients with emotional over-modulation. Retelling traumatic experiences in detail is poorly tolerated by these patients and might be a reason for not starting or not completing the recommended treatments. Due to lack of evidence, no alternative treatments are r

Parent- and child-reported anxiety disorders differentiating major depressive disorder and dysthymic disorder in children and adolescents.

OBJECTIVE: To date, specific parent- and child-defined anxiety disorders associated with dysthymic disorder (DD; DSM-5 persistent depressive disorder equivalent) with and without major depressive disorder (MDD) have not been investigated in children and adolescents. METHOD: In a cross-sectional study, we compared point prevalence rates of parent- and child-reported anxiety disorders in DD alone (N = 154), MDD alone (N = 29), comorbid DD and MDD (N = 130) and anxiety disorders alone (N = 126) groups. RESULTS: DD alone and MDD alone did not differ with respect to comorbid anxiety disorders fro

Mental disorders and mental health problems among recruit trainees, U.S. Armed Forces, 2000-2012.

Annual counts and rates of incident diagnoses of mental disorders or mental health problems have increased in the U.S. military active component since 2000, but less is known about recruit trainees. From 2000 to 2012, 49,999 active component recruit trainees were diagnosed with at least one mental disorder, and 7,917 had multiple mental disorder diagnoses. Annual incidence rates of at least one mental disorder decreased by approximately 37.4 percent over the last 13 years. Approximately 80.5 percent of all incident mental disorder diagnoses were attributable to adjustment disorders, depression

Hypothesis on PTSD Pathophysiology: Role of CRH, Noradrenaline, and Glucocorticoid Receptors in an Amygdala-Centered Closed-Loop System.

Post-traumatic stress disorder (PTSD) is a severe condition triggered by traumatic exposure, characterized by symptoms like trauma re-experiencing, avoidance, mood alterations, hypervigilance, and sleep disturbances. While its exact mechanisms remain uncertain, PTSD involves dysregulation across neurobiological systems underlying fear conditioning, threat appraisal, executive control, and stress response. Although research highlights the sympathetic-adreno-medullary (SAM) system and the hypothalamic-pituitary-adrenal (HPA) axis, findings on stress-related mediators remain inconsistent regardin

Randomization tests in clinical trials with multiple imputation for handling missing data.

Randomization-based inference is a useful alternative to traditional population model-based methods. In trials with missing data, multiple imputation is often used. We describe how to construct a randomization test in clinical trials where multiple imputation is used for handling missing data. We illustrate the proposed methodology using Fisher's combining function applied to individual scores in two post-traumatic stress disorder trials.

Patient perspectives on stress after ICU and a short primary care based psychological intervention - results from a qualitative sub‑study of the PICTURE trial.

BACKGROUND: Approximately 20-25% of patients who survive medical treatment at an intensive care unit (ICU) develop post-traumatic stress symptoms. There is currently a gap in follow-up care for them. As part of the PICTURE study, general practitioners (GPs) carried out a brief interview-based intervention. The aim of this sub-study is to record the most distressing memories of ICU treatment from the patient's perspective and their evaluation of a GP-based brief psychological intervention. METHODS: Participants were recruited from the intervention group of the main PICTURE study using selectiv

War, diet, and PTSD in Ukrainian youth.

The ongoing war in Ukraine has exposed young adults to sustained psychological stress, elevating their risk of developing post-traumatic stress disorder (PTSD). In a case-control study of 698 individuals, we investigated associations between PTSD, dietary patterns, disordered eating behaviours, and hematological parameters. PTSD was associated with greater adherence to restrictive diets-including ketogenic, low-fat, and intermittent fasting patterns-as well as higher scores for emotional, external, and uncontrolled eating. Conversely, adherence to a Mediterranean diet was associated with a red

Nightmares or a crippling reality? A review on sleep paralysis.

Sleep Paralysis (SP) is a state that occurs during the hypnagogic or hypnopompic period in which one finds themselves unable to move despite being conscious. During an episode of SP, the individual experiences muscle atonia, as they would during normal sleep, but they remain conscious. SP is also largely associated with three types of hallucinations - the 'intruder', the 'incubus', and an out of body experience. It can induce tremendous fear in the individual experiencing it, and cause significant distress. SP has been found to co-occur with chronic conditions such as hypertension and obstruct

Traumatic experiences, dissociative symptoms, and alexithymia in patients with alopecia areata.

Although genetic, environmental, autoimmune, and psychological factors are believed to play a role in the onset of alopecia areata (AA), the exact cause remains unknown. This study aimed to investigate whether there are differences in traumatic experiences, dissociative symptoms, and alexithymia levels between groups. Fifty eight patients diagnosed with AA, 58 individuals with dermatological diseases thought to have a low psychosomatic component, and 58 individuals not diagnosed with any chronic disease were included in the study. All participants were assessed using the Childhood Trauma Quest

Narrative Exposure Therapy versus treatment as usual in a sample of trauma survivors who live under ongoing threat of violence in Rio de Janeiro, Brazil: study protocol for a randomised controlled trial.

BACKGROUND: One in three individuals who live in Rio de Janeiro experience a traumatic event within a period of 12 months. In the favelas particularly, trauma exposure is ongoing. Psychological sequalae include posttraumatic stress disorder (PTSD), depression and other mental disorders. Trauma-focused therapy approaches have emerged as the treatment of choice when the dangerous events are over, but symptoms have remained for an extended time period. Ideally, the victim is in a safe context during treatment. However, frequently, survivors cannot escape from situations characterised by ongoing t
