Outcomes of Microsurgical Reconstruction of Post-Burn Joint Contracture-Systematic Review and Meta-Analysis.

BACKGROUND: Contracture recurrence is a common setback to burn reconstruction, especially for severe or large-area contractures. Flap-based burn reconstruction has been shown to result in lower recurrent contracture rates. This study aims to summarize and evaluate the outcomes of flap-based techniques used for post-burn joint contracture reconstruction. METHODS: A systematic review was performed following PRISMA guidelines. Databases searched included PUBMED, EMBASE, Scopus, and Web of Science. Articles that described the use of flaps with a known blood supply to reconstruct post-burn contrac

Editorial Commentary: How to Pass Go After Medial Patellofemoral Ligament Reconstruction.

Medial patellofemoral ligament reconstruction (MPFLR) has been a reliable procedure to allow patients with patellar instability to regain function and minimize subsequent instability. However, although much of the prior literature has focused on recurrent patellar instability as a primary outcome after MPFLR, this may not accurately capture how a patient feels he or she has recovered after surgery. A more nuanced approach, taking into account patient-reported outcome scores, specifically the Kujala score, as well as the minimal clinically important difference, substantial clinical benefit, and

A modification of the De Castro neophalloplasty: The continuous search for improvements.

INTRODUCTION: We want to present a 9 months old boy with congenital aphallia treated with the latest refinements and modifications in the technique, avoiding a full quadrangular lower abdominal flap but with some new design in the flap. METHODS: Basically, the lower abdominal flap resembles two joined bottles with 3 triangular deflections in each side to produce a zetaplasty in the dorsal aspect of the neophallus and avoid ventral retractions. The new format flap design allows an easier and more aesthetical conformation of the glans, neomeatus at the tip of the neo glans and the zetaplasty to

Testicular lodges creation during scrotal reconstruction by ALT (anterolateral thigh) island flap after Fournier gangrene: A case report.

INTRODUCTION: Fournier's gangrene is a severe infection of the perineum and genital regions, with high mortality rate. Treatment is medico-surgical and multidisciplinary, usually requiring extensive debridement of scrotal tissue and distant surgical reconstruction. Regarding scrotal reconstruction, numerous techniques have been described, however, to our knowledge, none has focused on testicular repositioning. CASE: We present the case of a 47-year-old patient treated for secondary Fournier's gangrene on post-traumatic colic fistula following a road traffic accident. We performed scrotal reco

Biodegradable temporizing matrix in small-scale reconstruction of malignant resections in functional areas.

Biodegradable temporizing matrices (BTMs) are temporary, biocompatible, and resorbable scaffolds designed to temporarily occupy a defect site, facilitating tissue regeneration and neodermal growth in a surgical wound or defect site while minimizing the need for extensive tissue transfers. Spiradenomas are mostly benign skin tumors developing from sweat gland cells that typically require surgical excision as treatment, which can result in scarring and functional impairment, especially in functional joint areas like the ankle. Though the application of BTMs in small- to moderate-sized defects re

Double-Paddle Use of the Superficial Circumflex Iliac Artery Perforator Flap for Fournier's Gangrene: A Case Report.

Fournier's gangrene often leads to extensive perineogenital soft tissue loss, necessitating complex reconstruction. The superficial circumflex iliac artery perforator (SCIP) flap is known for its thin pliable skin and low donor site morbidity. We present a case of a 65-year-old male with Fournier's gangrene affecting both the mons pubis and scrotum. After multiple debridements, a double-paddle SCIP flap based solely on the superficial branch of the SCIA was designed with two independent skin paddles to reconstruct these adjacent but anatomically distinct areas. Preoperative CT angiography and

The Superficial Inferior Epigastric Artery Fascia Flap for Nerve Reconstruction in Rabbits.

The addition of surgical angiogenesis to reconstructed peripheral nerves is argued to improve nerve regeneration. Surgical angiogenesis can be performed by wrapping a vascularized flap around the nerve graft, which is believed to enhance nerve regeneration following reconstruction. The protocol aimed to outline the surgical procedure and report long-term viability and patency for the superficial inferior epigastric artery fascia (SIEF) flap in a rabbit peroneal nerve defect model to add vascularization and enhance nerve regeneration. In this article, step-by-step instructions on the harvest an

Reconstruction of Large Full-Thickness Medial Lower Eyelid Defects Using a Medially Based Tarsoconjunctival Pedicle Flap.

Reconstruction of large full-thickness medial lower eyelid margin defects often requires centrally based tarsoconjunctival pedicle flaps from the ipsilateral upper eyelid in combination with anterior lamellar grafting. However, centrally based flaps may result in temporary obstruction of the visual axis in the affected eye. The authors present a novel modified surgical technique using a medially based tarsoconjunctival pedicle flap from the upper eyelid for the reconstruction of large full-thickness medial lower eyelid defects. A tarsoconjunctival flap was mobilized and hinged medially by the

Modified Uvular Repair in Children Undergoing Palatoplasty.

BackgroundPaucity exists in the literature on uvular repair while documenting palatoplasty techniques for children with cleft palate.SolutionWe propose a modified approach without losing any soft tissue structures, gaining better cosmetic outcome, and possibly having functional gains postoperatively. What we did that is newThe proposed technique provides better cosmesis for the structures postoperatively and enhances the palatoglossal arch, which may prevent the spillage of saliva to the oropharyngeal region.

Anterolateral thigh flap harvested from paralyzed limbs in post-polio syndrome for tongue reconstruction-a case report and a review of the literature.

BACKGROUND: Poliomyelitis is an acute infectious disease caused by poliovirus in childhood. After the onset of acute poliomyelitis, weakness of limbs occurs 15 to 30 years later, which is defined as post-polio syndrome. Poliomyelitis accompanying head and neck cancer is rare. Microvascular reconstruction is a well-established procedure following extensive tumor resection. Donor-site selection for flap harvest is a very important issue in paralyzed patients, particularly in patients with post-polio syndrome. To date, limited information is available regarding flap harvest in paralyzed patients.

Reconstruction of Skin Defects in the Medial Canthal Region Using the Novel Hug Flap.

Various methods of reconstructing skin defects in the medial canthal region have been reported. We report 2 skin flaps for the upper and lower eyelids designed to reconstruct soft tissue defects in the medial canthal region based on the approach used for the orbit during surgeries for facial bone fractures. The skin flap was elevated without tension until it reached the defect. The skin flaps of the upper and lower eyelids were moved around the defect as rotational flaps and sutured. Two patients with skin defects in the medial canthal region after basal cell carcinoma resection were treated w

3D analysis of condylar and mandibular remodeling one year after intra-oral ramus vertical lengthening osteotomy.

OBJECTIVES: Among the existing techniques for the correction of mandibular posterior vertical insufficiency (PVI), the intra-oral ramus vertical lengthening osteotomy (IORVLO) can be proposed as it allows simultaneous correction of mandibular height and retrusion. This study assessed the 3D morpho-anatomical changes of the ramus-condyle unit and occlusal stability after IORVLO. MATERIALS AND METHODS: This retrospective analysis compared immediate and 1-year post-operative 3D CBCT reconstructions. The analysis focused on the condylar height (primary endpoint) and on the changes in condylar (co

Prevalence of free flap failure in mandibular osteoradionecrosis reconstruction: a systematic review and meta-analysis.

Our study aimed to estimate the prevalence of total free flap failure following free flap reconstruction for mandibular osteoradionecrosis (mORN) and assess the impact of potential moderators on this outcome. A comprehensive systematic literature search was independently conducted by two reviewers using the Medline, Scopus, Web of Science and Cochrane Library databases. Quality assessment of the selected studies was performed, and prevalence estimates with 95% confidence intervals (CI) were calculated. Outlier and influential analyses were conducted, and meta-regression analyses was employed t

Laryngotracheal and esophageal functional reconstruction using a single anterolateral thigh flap.

The main goal for treatment of locally advanced thyroid carcinoma invading the laryngotracheal and esophageal complex, is radical surgical excision and reconstruction of a functional aerodigestive tract. We describe a challenging surgical technique in which a wide laryngotracheal and esophageal defect was functionally reconstructed using a single anterolateral thigh flap. This can represent a good option for a tailored functional reconstruction of extended defects of the aerodigestive tract.

Jejunum free flap for head and neck reconstruction, a step-by-step surgical technique videos.

Circular pharyngolaryngectomy for oncologic resection requires a tubular reconstruction. Different options can be proposed to the patient: digestive free flap, fasciocutaneous flap, or musculocutaneous flap. The jejunum free flap is a tubular flap commonly used in esophageal and pharyngeal reconstruction with good functional outcomes and an acceptable rate of complications. Reconstruction with a jejunum free flap is an ideal choice. Patients at Gustave Roussy Institute (Villejuif, France) were offered a jejunum flap free flap for all circular pharyngolaryngectomies. The surgical technique is e

A road map for the management of a pregnancy complicated by maternal bladder exstrophy.

Bladder exstrophy (BE) is a congenital genito-urinary malformation where there is a defect in the abdominal wall resulting in a protruding open bladder with exposed mucosa (Resnik R.P. et al. Creasy and Resnik's maternal-fetal medicine: principles and practice. Elsevier, 2019). Several reconstructive procedures are required to correct the anomalies, resulting in an ileal conduit which is an alternate urinary reservoir reconstructed from the terminal ileum (Madersbacher S, et al. J Urol 169(3):985-90, 2003). We describe the care of a pregnant woman with BE and outline the principles of manageme
