[Fowler's Syndrome: diagnostic and therapeutic challenge].

Urinary retention in women is not a common complaint. The main causes are structural or functional, which means that urological and neurological reviews are mandatory. Fowler's Syndrome has been described in 1988 and has stablished an association between urinary retention, electromyographic abnormality of the striated urethral sphincter and polycystic ovaries pathologies. Even though its phisiopathology is not completely understood, this disorder seems to be due to a primary failure of relaxation of the striated urethral sphincter. Not only a typical clinical history, but also the evaluation o…

Endoscope-assisted radical perineal prostatectomy.

Dissection of the bladder neck is particularly important in patients who have previously had prostate surgery due to hyperplasia. We describe an endoscope-assisted radical perineal prostatectomy (RPP) technique that facilitates the dissection of the prostate-vesical junction. The technique was employed in four patients with a history of transurethral prostate resection. Before dissecting the prostate from the bladder neck during RPP, we circumscribed the bladder neck perurethrally with a Collins knife. The remaining of the RPP procedure was performed via the traditional approach. The incision …

Unusual cause of failure of end-to-end anastomotic urethroplasty for traumatic obliterative bulbar urethral stricture.

End-to-end anastomotic urethroplasty (EEA) for traumatic bulbar urethral stricture has excellent long-term success rate. We encountered a patient with obliterative bulbar stricture and persistent phlegmon in perineum resulting from straddle injury. Phlegmon-excision and EEA was performed, which ended up in recurrence. Thorough excision of recurrent nodules & scar with stage-I urethroplasty was performed using 'scrotal shutter flap'. Histopathology of phlegmon of first surgery and nodules & urethral margins of second revealed amyloidosis. Stage-II urethroplasty was performed 3-months later afte…

Complicated urethroplasty: a guide for surgeons.

Although straightforward male urethral stricture disease is commonly encountered in the scope of general urologic practice, complex urethral strictures are less common and require a more systematic approach. Complex urethral stricture surgery for long and panurethral strictures, after failed hypospadias repair, and for recurrent posterior urethral distraction defects requires a dynamic treatment paradigm. A multistaged urethral reconstruction is often necessary owing to hostile urethral tissue, especially after multiple previous procedures. A perineal urethrostomy sometimes offers improved qua…

Traumatic urethral injury without pelvic fracture in an adult female.

A 23-year-old female was involved in a motor vehicle collision with multiple injuries, including a right acetabular fracture, but no pelvic fracture. Urology consultation was obtained due to difficulty placing a urethral catheter. Examination revealed a longitudinal urethral tear with vaginal laceration extending 2 cm from the urethral meatus proximally toward the bladder neck. The longitudinal urethral tear was repaired primarily. Traumatic female urethral injury in the absence of a pelvic fracture is an exceedingly rare occurrence.

Stricture recurrence after urethroplasty: a systematic review.

PURPOSE: Urethroplasty remains the gold standard for the management of urethral stricture disease with acceptable long-term success. However, the standard by which stricture recurrence is defined and evaluated after urethral reconstruction remains widely variable. We conducted a systematic review of the urological literature to determine how stricture recurrence is defined and evaluated. MATERIALS AND METHODS: A systematic review was conducted on all contemporary urethroplasty articles published between 2000 and 2008. Using the term "urethroplasty" 302 articles were identified and evaluated. …

Posterior urethral valves.

The most common cause of lower urinary tract obstruction in male infants is posterior urethral valves. Although the incidence has remained stable, the neonatal mortality for this disorder has improved due to early diagnosis and intensive neonatal care, thanks in part to the widespread use of prenatal ultrasound evaluations. In fact, the most common reason for the diagnosis of posterior urethral valves presently is the evaluation of infants for prenatal hydronephrosis. Since these children are often diagnosed early, the urethral obstruction can be alleviated rapidly through catheter insertion a…

[Urethoplasty in balanitis xerotica obliterans. Technical variation].

OBJECTIVE: We propose a modification to urethroplasty for stricture due to lichen sclerosus (balanitis xerotica obliterans). METHODS: We combine two-stage bucal mucosa graft and onlay ventral island flap. RESULTADOS/CONCLUSIONES: This technique offers enlargement of the graft with the island flap and removal of the pathological skin.

[Buccal mucosa urethroplasty in anterior urethral strictures].

UNLABELLED: Buccal mucosal graft can be used for succesfull repair in both pendulous and bulbar strictures. MATERIAL AND METHODS: We present our experience with buccal mucosal graft repair in 8 patients with onlay patch that varies from 4 to 16 cm. in length. Three pendulous, two bulbar and three panurethral strictures were repaired. These patients were observed for 36 to 60 months. RESULTS: No stricture recurrences were observed. Only one patient had lower lip paresthesia for six months.

Concordant posterior urethral valves in male monochorionic twins with secondary prune belly syndrome.

BACKGROUND: Posterior urethral valves (PUVs), the most common congenital cause of lower urinary tract obstruction, have been described to occur in identical and nonidentical twins. Until now, reports have been published on 15 cases of PUVs. AIM: We report a new case of concordant PUVs in one set of male monochorionic twins with secondary Prune Belly Syndrome. CASE REPORT: The twins were born by elective cesarean section at 38 weeks of gestation to a 36-year-old mother, gravida 6, para 6. On ultrasound perfomed at 18 weeks's gestation, both fetuses showed signs of PUVs. At birth, physical exa…

Normal anatomy of the external urethral meatus in boys: implications for hypospadias repair.

UNLABELLED: Both papers in this section relate to the always difficult subject of hypospadias repair. One of them describes the anatomy of the external urethral meatus, and the other a technique for repairing coronal or subcoronal hypospadias. OBJECTIVE: To investigate the normal external urethral meatal anatomy in boys, and to examine the proportional relationship between meatal length and degree of ventral glans closure. PATIENTS AND METHODS: In all, 92 boys with presumed normal penile anatomy were considered eligible for the study; 17 were not assessed because either the boy or parents de…

[Hypospadias and congenital curvature of the penis in children and their surgical treatment].

Hypospadias is a congenital defect of the penis in which the urethral orifice is located on the ventral aspect of the glans penis or, more proximally, on the shaft or scrotum. Some type of hypospadias occurs in approximately 1 in 200 boys. The defect can be minimal or so severe that the sex of the newborn is difficult to determine. Many boys with hypospadias also have a congenital curvature of the penis that needs correction. Corrective surgery is done preferably between the ages of 6 and 12 months. The aim of surgery is to obtain a functionally and cosmetically normal penis while limiting the…

Partial bladder outlet obstruction induces urethral smooth muscle hypertrophy and decreased force generation.

PURPOSE: PBOO leads to increased urinary frequency, decreased void volume, hypertrophy of the detrusor SM, and alterations in contractile and regulatory proteins. This study was done to determine whether PBOO induced increases in urinary frequency and detrusor SM hypertrophy are associated with an alteration in the contractility and expression of myosin isoforms in urethral SM. MATERIALS AND METHODS: PBOO was surgically induced in male New Zealand White rabbits, and sham operated rabbits served as controls. After surgery, rabbits were kept 12 days, and prior to sacrifice, urine output and voi…

[Bladder leiomyoma treated by transurethral endoscopic resection].

We report a case of intravesical bladder leiomyoma in a 47-year-old patient presenting with recurrent macroscopic haematuria. The diagnosis was strongly suspected on CT and cystoscopy. Treatment consisted of transurethral resection of the mass. No recurrence has been observed with a follow-up of two years. In the light of this case, the authors discuss the diagnosis and treatment of bladder leiomyoma.

Novel everting urologic access sheath: decreased axial forces during insertion.

BACKGROUND: Advancement of urologic instruments through the genitourinary tract is associated with significant axial forces that likely contribute to patient discomfort, even after injection of a local anesthetic, and may lead to mucosal trauma, postprocedural dysuria and hematuria, and increased susceptibility to infection and strictures. Placing an everting urethral sheath prior to instrumentation may decrease these problems. MATERIALS AND METHODS: Two 7-cm-long, 5-mm diameter urethral luminal models were created, one with and one without an artificial stricture. We measured the forces gene…

Congenital pouch colon with prune belly syndrome and megalourethra.

Congenital pouch colon (CPC), an unusual high anorectal malformation in which a varying length of colon is replaced by a dilated pouch, has appeared in a number of case reports and series published in the English literature. There is only one case report of its association with prune belly syndrome (also known as triad syndrome, Eagle-Barrett syndrome, and abdominal muscle deficiency syndrome). We report the description and management of this rare association in a male neonate with CPC, prune belly syndrome, and congenital megalourethra.
