We report on a 65-year-old woman having suffered from generalized morphea since 28 years. Chronic leg ulcers within atrophic areas have been known since 1960. Since 10 months, the ground of one of these large ulcerated scars has increasingly turned to cauliflower-like vegetation with its edge progressively heaped up. Biopsies confirmed the diagnosis of squamous cell carcinoma. We discuss possible ways of carcinogenesis of squamous cell carcinoma in generalized morphea.

A curious monozygous pair of twins producing sudden and vehement nose blowing and/or sneezing during/after eating and drinking, when their stomach had achieved a certain stage of fullness, has been observed. The sneezing reflex could be registered in 4 male members of the family. Since there were neither neurological disorders nor significant alterations in their electroencephalographic activity, the phenomenon may be regarded as a special type of hereditary vegetative sensitivity. The trait seems to follow either an autosomal dominant or perhaps a Y-linked mode of inheritance.

Using the clinico-catamnestic method the authors examined residual neurotic states in 36 patients with a history of obsessional neurosis. A 6-year-old boy with a congenital bicuspid aortic valve presented with finger clubbing and hypertrophic osteoarthopathy, and subsequently he developed severe hypertension. The hypertension was successfully treated by nephrectomy, at which a thrombosed mycotic aneurysm of the renal artey was found. Echocardiography showed the presence of aortic valve vegetations. Blood cultures were sterile, but high antibody titres to the phase 1 and 2 antigens of Coxiella burneti strongly suggested Q fever infection. We believe this is the first reported case of Q fever endocarditis in early childhood.

Thirty Ménière patients were treated with betahistine hydrochlorid for an average period of 17 months. The majority of them had been resistent to every previous treatment. We report a case of culture-negative infective endocarditis in a 64-year-old hemodialysis patient who presented with fever and dyspnea. Despite empirical antibiotic therapy and negative blood cultures, transesophageal echocardiography later revealed aortic valve vegetation and destruction. The patient developed cardiogenic shock and died before surgery. This case illustrates the diagnostic challenges of culture-negative infective endocarditis, especially in high-risk populations. Early blood culture collection and continuous reassessment of surgical timing are crucial to improve outcomes.

Peri-ictal water drinking (PIWD) is a rare vegetative manifestation of temporal lobe epilepsy without a definite lateralisation value. We report a case of PIWD in a 22-year-old Omani male patient with post-concussion syndrome and epilepsy presented to a tertiary care hospital in Muscat, Oman, in 2021 for evaluation of paroxysmal events. His behaviour of PIWD was misinterpreted by his family until characterised in the epilepsy-monitoring unit as a manifestation of epilepsy that was treated medically. To the best of the authors' knowledge, this is the second reported case in the region.

Intracardiac masses adhering to the tricuspid valve can occur as a result of right-sided infective endocarditis, malignancy, clot formation in the right atrium, or clots-in-transit passing through the right atrium. Early surgical intervention is recommended for tricuspid valve vegetation in some patients, although open heart surgery is not always an option. Treatment options for right heart thrombi include anticoagulation, thrombolysis, surgical embolectomy, or mechanical aspiration.
