A history of partial complex seizures in a 30-year-old woman.

Dysembryoplastic neuroepithelial tumour is an uncommon lesion of the brain characterised by a heterogeneous population of neurons, astrocytes and oligodendroglia-like cells. Most patients are young adults with a long history of drug-resistant seizures. We report a case of a 31 year-old woman with a history of severe epileptic attacks. Cerebral imaging showed a left temporal tumour measuring 4 cm in its greater dimension. After surgical intervention, histopathological examination showed a tumoural proliferation with both glial and neuronal components that was confirmed by immunohistochemistry. …

Brain stem encephalitis due to varicella-zoster virus in a patient with AIDS.

We describe a patient infected with human immunodeficiency virus (HIV) who had localized brain stem encephalitis due to varicella-zoster virus (VZV) and no cutaneous eruption. Diagnosis of the infection was based on the presence of Cowdry type A intranuclear inclusions in neurons, astrocytes, and oligodendrocytes positive for VZV (as shown by immunochemical staining). Although this infection is rare, we demonstrate the need for clinicians to include VZV infection in the differential diagnosis of rapidly progressive multiple cranial nerve palsies in HIV-infected patients, particularly because s…

Neuroprotective effects of alpha-dihydroergocryptine against damages in the substantia nigra caused by severe treatment with 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine.

The effects on the substantia nigra of alpha-dihydroergocryptine (DEK), a drug with strong dopaminomimetic activity, were tested with a severe 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) treatment in monkeys. Compared with monkeys treated with MPTP alone, the animals which received DEK plus MPTP showed reduced neuronal death in the substantia nigra. The reactive astrocytes were increased in number. Moreover, several axons which were immunopositive to phosphorylated neurofilament proteins and with features similar to those of control animals were seen in the pars compacta. The findings …

Unilateral hemispheric cerebral changes similar to Creutzfeldt-Jakob disease in a case of hemiconvulsion.

A 77-year-old man suffered intermittent hemiconvulsions of unknown etiology on the left side for a period of about 5 weeks. At the autopsy, there was marked neuronal loss, severe proliferation of astrocytes and spongiform changes in the right cerebral cortex. The cerebral white matter showed loosening with astroglial proliferation in areas on the same side. These neuropathological changes were slight or absent in the left cerebral hemisphere. Histopathological changes were similar to those seen in unilateral Creutzfeldt-Jakob disease (CJD). Although unilateral CJD can not be ruled out, these u…

Unilateral Creutzfeldt-Jakob disease.

A 73-year-old woman had progressive right hemiparesis, aphasia, and focal motor seizures. EEG showed periodic discharges on the left. She died 8 weeks after onset. At autopsy, there was marked spongiform change, neuronal loss, and severe proliferation of astrocytes predominantly on the left and most prominently in the insular and centroparietal cortex. The changes were consistent with Creutzfeldt-Jakob disease (CJD), but pathology was slight or absent on the right side. This case appears as the first report of what might be called unilateral CJD. Such a condition should be included within the …

A type of adult polyglucosan body disease.

Polyglucosan bodies in the nervous system and other viscera are the main findings at autopsy of a 64-year-old woman who had a chronic neurologic disorder of 20 years' duration. The clinical features included muscle weakness, sensory disturbances, neurogenic bladder, dementia, and cataracts. Light and electron microscopy of the nervous system showed numerous polyglucosan bodies in the processes of neurons and astrocytes but not in neuronal perikarya. A similar clinico-pathologic presentation has been described in six previous cases. It is suggested that this is a type of adult polyglucosan body…

An autopsy case of Creutzfeldt-Jakob disease with kuru-like neuropathological changes.

An autopsy case of Creutzfeldt-Jakob disease with Kuru-like neuropathological changes which revealed clinically extrapyramidal, pyramidal and psychic symptoms is presented in this report. On microscopic examination, status spongiosus, neuronal degeneration, proliferation of hypertrophic astrocytes and numerous plaques were observed in the cerebrum and cerebellum accompanied with widespread demyelinization. These plaques which suggested Kuru plaques measuring 10 to 60 micron were strongly PAS positive and had a dense central core surrounded by a halo of fine radially arranged fibrils. As for th…

Neuroinflammation in Alzheimer's disease: From glial cell states and molecular networks to biomarkers and precision therapeutics.

Alzheimer's disease (AD) is now recognized as a biologically heterogeneous syndrome in which amyloid-β pathology, Tau-related alterations, and neuroinflammation are key interconnected processes. Microglia, astrocytes, oligodendrocytes, complement, inflammasomes, neurovascular cells, and peripheral immunity are implicated as both adaptive and disease-promoting contributors to the pathophysiology of AD. Here, we highlight mechanisms by which innate immunity and peripheral immune cells influence amyloid β-peptide metabolism, Tau pathophysiology, excitatory synaptic function, oxidative stress resp…

Emerging pathological mechanisms of Alzheimer's disease pathogenesis: from neuroimmune interactions to intercellular communication.

Alzheimer's disease (AD) research has transcended the traditional paradigm centered on amyloid-beta (Aβ) shifting toward a neuroimmune netwo … hora of pro-inflammatory factors, exacerbating neuroinflammation and neuronal damage. Concurrently, astrocytes lose their homeostatic support functions and acquire neurotoxic properties. Intercellular communication molecules play pivotal roles as key mediators. The cytokine/chemokine network sustains a chronic inflammatory milieu; extracellular vesicles (EVs …

Metformin-Loaded Schwann Cell Exosomes Reprogram Macrophages and Enhance Neurogenesis in Spinal Cord Injury Through PI3K/AKT Activation.

Spinal cord injury (SCI) creates a hostile microenvironment characterized by persistent inflammation and glial scarring, which severely limi … ficantly inhibited the secretion of pro-inflammatory cytokines (TNF-α, IL-1β, and IL-6) by reactive astrocytes. Functionally, the treatment significantly improved motor recovery, restored electrophysiological conduction, and ameliorated bladder dysfunction in SCI mice. Alexander Disease: A Literature Review for Clinicians.

Alexander disease (ALEXD; MIM 203450) is a rare leukodystrophy caused by dominant mutations in the GFAP (Glial Fibrillary Acidic Protein) gene, which encodes a key structural protein of astrocytes. First described in 1949, ALEXD is now recognized as a clinically heterogeneous disorder with a broad phenotypic spectrum spanning neonatal, infantile, juvenile, and adult-onset forms. Clinical manifestations vary according to age at onset and disease subtype, ranging from early developmental impairment and progressive neurologic decline to later-onset presentations dominated by bulbar dysfunction, a…

A 3D bioprinted in vitro contusion injury platform for mechanobiological studies of spinal cord injury.

Spinal cord injury (SCI) models are crucial for understanding injury mechanisms and evaluating potential therapeutic strategies. However, most in vitro SCI models rely on chemical or scalpel-induced damage and do not adequately reproduce the controlled mechanical loading associated with traumatic injuries. This study aimed to develop and optimise a reproducible injury protocol for application to 3D bioprinted cell-laden hydrogel constructs and to evaluate the resulting cellular responses. A custom mechanical loading system was used to apply compressive injury to 3D bioprinted gelatin methacryl

A human iPSC-derived 3D spinal cord organoid model to study radiation-induced neural injury.

BACKGROUND: Radiation therapy remains a cornerstone in the treatment of primary and metastatic tumors; however, its efficacy is limited by the spinal cord's high sensitivity to radiation-induced injury. To better understand the mechanisms underlying spinal cord radiosensitivity, we developed a three-dimensional human spinal cord organoid model derived from human-induced pluripotent stem cells using a neuroectodermal differentiation protocol that closely mimics embryonic spinal cord development. METHODS: Mature spinal cord organoids were exposed to a clinically relevant 2 Gy dose of ionizing r

Sleep Deprivation Induces Glial Dysfunction, Synaptic Loss, Metabolic Imbalance, and Cognitive Impairment in 12-Month-Old Mice: Protective Effects of D30.

BACKGROUND: Sleep deprivation (SD) has been increasingly implicated in age-related cognitive decline. However, the mechanisms linking SD duration to progressive disruption of glial homeostasis, synaptic vulnerability, and metabolic dysregulation remain poorly defined. METHODS: We employed 12-month-old Thy1-EGFP and Cx3CR1-EGFP mice to investigate the effects of short-term sleep deprivation (SSD) and long-term sleep deprivation (LSD) on neuronal architecture, microglial morphology, and astrocytic homeostasis. Quantitative analyses included dendritic spine density, synaptic protein expression,

Molecular insights into glial neuroimmune cross reactivity with CNS antigens and its role in neuroinflammation.

Neuroinflammation has been increasingly considered a key player of neurodegenerative as well as psychiatric disorders. This review integrates existing knowledge on glial-neuroimmune interactions, emphasizing the roles of cytokine signaling, glial activation, and BBB modulation in neuro-pathogenesis. A systematic review was performed studying peer-reviewed literature on molecular pathways of microglia, astrocytes, endothelial cells, and peripheral immune mediators. A possible explanation of this finding could be that the model is based on the underlying pathophysiology, and this is shared acros…

Modulation of Mitochondrial Dynamics in Primary Hippocampal Cultures of 5xFAD Mice by Mdivi-1, MFP, and Exogenous Zinc.

BACKGROUND: Mitochondrial dynamics-the balance between fission, fusion, and mitophagy-are essential for maintaining cellular homeostasis and are increasingly implicated in the pathogenesis of Alzheimer's disease (AD). METHODS: Here, we investigated the effects of targeted modulation of mitochondrial fission and fusion on mitochondrial morphology and metabolic status in primary hippocampal cultures derived from 5xFAD transgenic mice. Mitochondrial dynamics were modulated using the fission inhibitor Mitochondrial Division Inhibitor 1 (Mdivi-1), the fusion promoter mitochondrial fusion promoter
