Anatomy of the Radial Recurrent Artery at the Entrance of the Supinator Canal and Its Relevance to Decompression of the Deep Branch of the Radial Nerve.

PURPOSE: Radial recurrent artery (RRA) has been reported as a potential entrapment site for the deep branch of the radial nerve (DBRN). This anatomical study aimed to describe the branching patterns and topography of the RRA at the entrance of the supinator canal to better understand this cause of posterior interosseous nerve (PIN) palsy and enhance surgical precision in nerve decompression procedures. METHODS: A total of 160 formaldehyde-preserved donated upper limbs were utilized in this study. The RRA and its branches forming the leash of Henry (LoH) were exposed and analyzed. The topograp

Liposomal bupivacaine versus ropivacaine for surgical site infiltration in lumbar fusion: a prospective randomized controlled trial.

INTRODUCTION: Effective postoperative pain control after lumbar spine surgery remains challenging, and excessive opioid use is associated with adverse outcomes. Evidence comparing liposomal bupivacaine (LB) with conventional long-acting local anesthetics in spine surgery is limited. PATIENTS AND METHODS: In this single-center, prospective, randomized, patient- and outcome assessor-blinded trial, adult undergoing one- or two-level posterior lumbar decompression and fusion were assigned (1:1) to surgical site infiltration with either LB (266 mg) plus 25 mg plain bupivacaine (LB group) or ropiva

Interventional Radiology in Acute Cholecystitis: A Review of Contemporary Percutaneous Strategies and Emerging Techniques.

Background/Objectives: Acute cholecystitis is a common surgical emergency associated with significant morbidity in elderly, frail, and critically ill poor surgical candidates. Early laparoscopic cholecystectomy remains the standard of care for low-risk patients, but interventional radiology (IR) modalities have been increasingly used in high-risk patients for gallbladder decompression, source control, and/or definitive non-operative treatment. Methods: A narrative review of the literature was performed to investigate current percutaneous IR options in acute cholecystitis. Evidence from interna

Recovery patterns from C5 palsy after anterior cervical decompression and fusion, posterior cervical decompression and fusion, and laminoplasty for degenerative cervical myelopathy: systematic review and meta-analysis of 748 C5 palsy cases.

Despite the favorable postoperative prognosis of C5 palsy (C5P), a certain proportion of these patients have less satisfactory outcomes. The current systematic review and meta-analysis thus aimed to comprehensively evaluate existing literature and identify the onset, recovery patterns, and outcomes of C5P following diverse surgical approaches. Five different databases (Google Scholar, Embase, PubMed, Web of Science, and Cochrane Library) were thoroughly searched for relevant literature on October 15, 2024. Studies reporting on incidences of C5P following surgery for degenerative cervical condi

Minimally Invasive Management of Neonatal Piriform Fossa Sinus Tract: A Scoping Review.

OBJECTIVE: In rare situations, neonates may present with respiratory distress and neck swelling from a piriform fossa sinus tract (PFST) lesion. Open surgical excision of PFST may be associated with higher complication rates, especially in neonates. Endoscopic treatment has been shown to have high success rates in older children, but neonatal outcomes appear to be less promising. The objective of this study was to review the existing literature on endoscopic treatment of PFST in neonates. REVIEW METHODS: PRISMA-ScR guidelines for scoping reviews were employed. Medline and Embase databases wer

Recovery of Supraspinal Microstructural Integrity and Connectivity in Patients Undergoing Surgery for Degenerative Cervical Myelopathy.

BACKGROUND: It remains unknown if the progressive loss of axonal conduction along sensorimotor tracts can be recovered after surgery in patients with degenerative cervical myelopathy (DCM) and if subsequent adaptive microstructural changes are associated with the neurological improvement. OBJECTIVE: To investigate the upstream recovery of microstructural integrity and reorganization of microstructural connectivity that occurs in patients with DCM after surgical decompression. METHODS: Preoperative and postoperative cerebral diffusion tensor imaging and diffusion spectrum imaging data were co

Identifying Common Peroneal Neuropathy before Foot Drop.

BACKGROUND: Common peroneal neuropathy shares the same pathophysiology as carpal tunnel syndrome. However, management is often delayed because of the traditional misconception of recognizing foot drop as the defining symptom for diagnosis. The authors believe recognizing common peroneal neuropathy before foot drop can relieve pain and help improve quality of life. METHODS: One hundred eighty-five patients who underwent surgical common peroneal neuropathy decompression between 2011 and 2017 were included. The mean follow-up time was 249 ± 28 days. Patients were classified into two stages of se

Three-Dimensional Analysis of the Radiological Risk Factors for Progression of the Thoracic Ossification of the Posterior Longitudinal Ligament After Posterior Decompression and Stabilization.

BACKGROUND: Although research shows that the ossified masses of ossification of the posterior longitudinal ligament (OPLL) continue to grow postoperatively, information regarding thoracic OPLL (T-OPLL) is lacking. To date, no study has investigated the progression of T-OPLL within each motion segment. The purpose of this study was to analyze the progression of T-OPLL in each motion segment using a 3-dimensional measurement and evaluate whether the type of T-OPLL and surgical methods affect its progression postoperatively. METHODS: Clinical data from 20 patients (101 segments) with thoracic my

A Novel Diagnostic and Prognostic Tool for Simple Decompression of Ulnar Nerve in Cubital Tunnel Syndrome.

BACKGROUND: Surgical decompression of the ulnar nerve (UN) is effective for treating cubital tunnel syndrome (CubTS). Nevertheless, the outcome is not always satisfying. Different surgical, clinical, and imaging findings have been claimed as outcome predictors, but there is no consensus in the literature. We analyzed the outcome-predicting role of ultrasonography (US) of the UN in patients with CubTS and its possible role for diagnosis and follow-up. METHODS: Patients with CubTS treated by simple UN decompression underwent US and electrodiagnotic (ED) studies of the UN at the elbow before and

[Orbital compartment syndrome. The most frequent cause of blindness following facial trauma].

INTRODUCTION: Although orbital compartment syndrome is a rare condition, it is still the most common cause of blindness following simple or complicated facial fractures. Its pathomechanism is similar to the compartment syndrome in the limb. Little extra fluid (blood, oedema, brain, foreign body) in a non-space yielding space results with increasingly higher pressures within a short period of time. Unless urgent surgical intervention is performed the blocked circulation of the central retinal artery will result irreversible ophthalmic nerve damage and blindness. Aim, material and method: A retr

Multiple myeloma presenting with acute bony spinal cord compression and mechanical instability successfully managed nonoperatively.

BACKGROUND CONTEXT: Multiple myeloma (MM) with spinal involvement may present with spinal cord or cauda equina compression, with or without neurological impairment. This occurs when a soft-tissue myelomatous mass extends into the epidural space (Barron et al., 1959 [1]). The mainstay of management for such lesions in patients with normal neurology is chemotherapy and radiotherapy or radiotherapy alone, but those with neurological compromise require surgical decompression with adjuvant therapy (Patchell et al., 2005 [2]). Infrequently, patients with MM present with spinal cord compression and n

Should anyone perform percutaneous endoscopic laser diskectomy and percutaneous lumbar disc decompressions?

BACKGROUND: Increasingly, pain management specialists (P-S) (e.g., anesthesiologists, radiologists, or physiatrists), who are not spinal surgeons, are performing percutaneous endoscopic laser diskectomy (PELD), percutaneous lumbar disc decompression (PLDD), and target percutaneous laser disc decompression (T-PLDD) in patients with minimal/mild disc herniations. Here, theoretically, the laser vaporizes/shrinks a small portion of disc tissue that lowers intradiscal pressure/volume, and thereby provides "symptomatic relief" (e.g., low back pain/radiculopathy). Nevertheless, the vast majority of t

Minimally invasive surgery through endoscopic laminotomy and foraminotomy for the treatment of lumbar spinal stenosis.

BACKGROUND: Lumbar spinal stenosis is a common cause of radicular and generalized back pain among older adults. Endoscopic minimally invasive surgery, in contrast to open decompression, may provide the opportunity for a less invasive surgical intervention. Thus, the purpose of this study is to evaluate the safety (operative complications, estimated blood loss, operative room time) and effectiveness (pre- versus postoperative level of disability and pain severity) of minimally invasive surgery using endoscopic laminotomy and foraminotomy among a large sample of patients with lumbar spinal steno
