A Giant Choledochal Cyst Mimicking Pancreatic Pseudocyst: A Case Report.

A choledochal cyst (CC) is a rare congenital anomaly manifested as cystic dilatation of the biliary tree. A giant choledochal cyst is defined as a cyst with a maximum diameter of ≥ 10 cm. It is an unusual entity and rarely revealed in adulthood. We describe the case of a giant, infected CC presenting as acute pancreatitis with results of abdominal ultrasound and computed tomography consistent with a pancreatic pseudocyst. The diagnosis of CC was made on magnetic resonance cholangiopancreatography (MRCP) findings. We proceeded with cholecystectomy and the complete resection of the diverticulum …

Lemmel Syndrome as a Rare Cause of Prolonged Right Hypochondrial Pain: A Case Report.

Lemmel syndrome is a rare cholestatic disease caused by a periampullary duodenal diverticulum (PAD) compressing the common bile duct (CBD) or pancreatic duct, which results in acute abdominal pain and/or obstructive jaundice in the absence of other pathology explaining the symptoms. It can be easily misdiagnosed unless carefully detected by abdominal ultrasound (US), barium studies, computed tomography (CT) scan, esophagogastroduodenoscopy (EGD), magnetic resonance cholangiopancreatography (MRCP), and endoscopic retrograde cholangiography (ERCP), which is also the treatment modality of choice.…

[Meckel's diverticulitis by ascaris in children: about a case].

We here report the case of a 4-year-old male child presenting with congenital umbilical bud that, recently, had begun to bleed. Physical examination showed pinkish umbilical bud with blood stains but without any fistula, measuring about 1,5 cm in diameter. Abdominal ultrasound was performed, which suggested urachal sinus. Surgery revealed umbilical bud communicating, in the abdominal portion, with hyperemic, inflammatory Meckel´s diverticulum placed 90 cm away from the ileocaecal angle in which many ascaris were detected. Anatomo-pathological examination of the surgical specimen showed diverti…

Anterior urethral trauma in childhood: presentation of two cases.

INTRODUCTION: The incidence of urethral trauma in children is low given that they have a short, mobile urethra, largely protected by the pubis. CLINICAL CASE: We describe two clinical cases illustrating the variety of presentations anterior urethral lesions can have. The first child had acute urinary retention two months after falling astride, with subsequent diagnosis of urethral stricture following serial voiding cystourethrogram (SVCU). The second child had urinary incontinence after falling off a horse. SVCU showed a urethral diverticulum, characterized through ultrasound imaging. Both we…

A rare cause of abdominal pain with fever.

An 89-year-old man presented to the emergency department with a 1-month history of upper right quadrant pain, worsened in the last 3 days and accompanied by fever and chills. On physical examination, he had scleral icterus and right upper quadrant tenderness. Laboratory findings showed hyperbilirubinemia, elevated liver enzymes and C reactive protein. The patient was admitted, suspecting of an acute cholangitis and started on antibiotics. An upper gastrointestinal endoscopy revealed the presence of a juxtapapillary diverticulum in the second portion of the duodenum, which was confirmed by a ma…

A case of a urethral diverticular adenocarcinoma after the fenestration of the anterior vaginal wall for pelvic floor abscess.

A 61-year-old woman presented to the gynecology department with complaints of atypical genital bleeding. Magnetic resonance imaging revealed a localized urethral tumor extended to vagina. Histological test of the biopsy tissue of the mass suggested the adenocarcinoma. The patient was performed the fenestration of the anterior vaginal wall 15 years ago. Under the diagnosis of urethral diverticular adenocarcinoma, we performed standard open total cystectomy with lymph node excision and ileal conduit. We could not establish a diagnosis of urethral diverticulum from the histological test; however,…

Suppurative meckel diiverticulum in a 3-year-old girl presenting with periumbilical cellulitis.

Meckel diverticulum (MD) is one of the most common congenital gastrointestinal anomalies and occurs in 1.2-2% of the general population. MD usually presents with massive painless rectal bleeding, intestinal obstruction or inflammation in children and adults. Suppurative Meckel diverticulitis is uncommon in children. An experience is described of a 3-year-old girl with suppurative inflammation in a tip of MD. She complained of acute colicky abdominal pain, vomiting and periumbilical erythema. Laparoscopic surgery found a relatively long MD with necrotic and fluid-filled cystic end, which was at…

Thoracoscopic esophagectomy in the prone position for esophageal cancer with right aortic arch: case report.

A congenital anomaly of the right aortic arch (RAA) is rare, and esophageal cancer associated with the vascular ring is even more rare. In such cases, it is very important to understand the anatomical situation in the upper mediastinum in order to perform a safe and curative operation. A 52-year-old man who presented with odynophagia was admitted to our department after a diagnosis of advanced esophageal cancer. Chest computed tomography revealed an RAA with an aberrant subclavian artery and showed that the esophagus was completely encircled by the RAA, aortic diverticulum, and pulmonary arter…

The recurrent laryngeal nerve was recurrent around the left subclavian artery. A Kommerell diverticulum was found at the origin of the left subclavian artery. The patient experienced no complications. We conclude that video-assisted thoracoscopic lobectomy with mediastinal dissection is feasible for treating lung cancer with a right aortic arch.

Missed diagnosis of complete urethral transection after sling: the case for translabial ultrasound.

Patients with complications of urethral sling placement for stress urinary incontinence are often treated for recurrent symptoms for years after initial reassuring evaluation. Translabial ultrasound is a noninvasive modality with minimal risks that can clearly diagnose urethral mesh complications. We present a 47-year-old premenopausal woman referred for treatment of urethral stricture and diverticulum 8 years after mesh sling placement. The diagnosis was made at an outside institution by voiding cystourethrogram and cystoscopy. However, translabial ultrasound confirmed the diagnosis of comple…

Imaging a boa constrictor--the incomplete double aortic arch syndrome.

Incomplete double aortic arch is a rare anomaly resulting from atresia rather than complete involution in the distal left arch resulting in … und to have an incomplete double aortic arch with hypoplasia of the left arch segment and an aortic diverticulum. Recognition of this abnormality by imaging is important to inform both corrective surgery in symptomatic patients, as well as assist in the planning of percutaneous coronary and vascular interventions.

Structural lesions such as thyromegaly, cervical hyperostosis, congenital web, Zenker's diverticulum, neoplasm, radiation fibrosis, and retropharyngeal abscess must be considered as other causes of dysphagia as well. Retropharyngeal abscess seldom occur in adults and if it does so, a search for a prior dental procedure, trauma, head and neck infection is needed. The symptoms may include neck pain, dysphagia, sore throat, and in rare cases, dyspnea accompanied…

[Sigmoidovesical fistula caused by diverticulitis detected with sigmoidoscopy].

Enterovesical fistular is an abnormal communication between the intestine and the bladder. It represents a rare complication of intestinal diverticulitis, colorectal malignancy, bladder cancer, inflammatory bowel disease, radiotherapy, and trauma. The most common etiology is diverticular disease. A 70-year-old man came to our hospital due to frequent urinary tract infection, dysuria, pneumaturia and fecaluria. Sigmoidoscopy revealed a large diverticulum with impacted stool at the sigmoid colon. When the scope was inserted into the site, the patient complained of severe urgency and pneumaturia.…

Left pneumothorax secondary to colonoscopic perforation of the sigmoid colon: a case report.

We present here the case of a 75-year-old woman who complained of acute abdominal pain after a diagnostic colonoscopy. Abdominal x-rays demonstrated pneumoperitoneum, whereas chest x-rays showed pneumomediastinum and left pneumothorax. A chest drain was placed and subsequently an exploratory laparoscopy was performed, during which air was found in the subserosa of the sigmoid colon and in the mesosigmoid secondary to perforation of a sigmoid diverticulum. The perforation was repaired and a protective loop colostomy was fashioned. The patient was discharged 8 days postoperatively in a good gene…

Urolithiasis associated with bilateral pelvic diverticula: a case report.

We present a case of renal stone associated with bilateral pelvic diverticula. The initial diagnosis by ultrasonography and plain abdomen radiography (KUB) was urolithiasis with a 15-mm calculus in the right renal pelvis. The patient was referred for extracorporeal shock wave lithotripsy, but no stone fragments were yielded. So, further evaluations were performed by using repeated ultrasonography, intravenous urography, and computerized tomography, which revealed the presence of diverticula in both right and left renal pelvises with stone fragments within the right sided diverticulum. We concl…
