Noninvasive tools to assess liver disease.

PURPOSE OF REVIEW: To improve understanding of recent developments in the noninvasive assessment of fibrosis and their potential clinical application to a range of chronic liver diseases. RECENT FINDINGS: Many noninvasive tests have evolved from their primary validation in chronic hepatitis C virus (HCV) infection to integration into clinical management algorithms for chronic liver disease of varying cause. Recent studies consider the use of these techniques in sequence or in combination, and compare the performance of serum and imaging-based noninvasive techniques to identify patients with s

In vivo biomechanical assessment of anterior rabbit urethra after repair of surgically created hypospadias.

PURPOSE: We studied the in vivo biomechanical properties of the anterior urethra in a rabbit hypospadias simulation model at long-term followup after urethroplasty. MATERIALS AND METHODS: A total of 38, 9-week-old white New Zealand male rabbits were randomized into active treatment and control groups. The active treatment group included hypospadias-like simulation and acute repair (sham, mobilization and advancement, tubularized incised posterior urethral plate). After 23 weeks all groups underwent biomechanical investigation using impedance planimetry. Cross-sectional area pressure measureme

Mechanical characterization of the softening behavior of human vaginal tissue.

The mechanical properties of vaginal tissue need to be characterized to perform accurate simulations of prolapse and other pelvic disorders that commonly affect women. This is also a fundamental step towards the improvement of therapeutic techniques such as surgery. In this paper, the softening behavior or Mullins effect of vaginal tissue is studied by proposing an appropriate constitutive model. This effect is an important factor after the birth, since vaginal tissue has been supporting a high load distribution and therefore does not recover its original behavior. Due to the anisotropy of the

Noninvasive diagnosis of nonalcoholic fatty liver disease.

Nonalcoholic fatty liver disease (NAFLD) is the most common cause of chronic liver disease, affecting approximately 30% of Western populations and a frequent indication for liver transplantation. The histologic spectrum of NAFLD includes simple steatosis, which has a benign prognosis, and nonalcoholic steatohepatitis, a more aggressive form of liver injury that may progress to cirrhosis and its complications. At present, the only widely accepted means of differentiating these lesions, including the severity of hepatic fibrosis, is liver biopsy. However, due to the invasiveness of this procedur

Determination of low-pass filter cutoff frequencies for high-rate biomechanical signals obtained using videographic analysis.

Diffuse brain injury (DBI) commonly results from blunt impact followed by sudden head rotation, wherein severity is a function of rotational kinematics. A noninvasive in vivo rat model was designed to further investigate this relationship. Due to brain mass differences between rats and humans, rotational acceleration magnitude indicative of rat DBI ( approximately 350 krad/s(2)) has been estimated as approximately 60 times greater than that of human DBI ( approximately 6 krad/s(2)). Prior experimental testing attempted to use standard transducers such as linear accelerometers to measure loadin

Hemoglobin dynamics in red blood cells: correlation to body temperature.

A transition in hemoglobin behavior at close to body temperature has been discovered recently by micropipette aspiration experiments on single red blood cells (RBCs) and circular dichroism spectroscopy on hemoglobin solutions. The transition temperature was directly correlated to the body temperatures of a variety of species. In an exploration of the molecular basis for the transition, we present neutron scattering measurements of the temperature dependence of hemoglobin dynamics in whole human RBCs in vivo. The data reveal a change in the geometry of internal protein motions at 36.9 degrees C

Renal versus extrarenal activation of vitamin D in relation to atherosclerosis, arterial stiffening, and hypertension.

Low dietary intake of calcium stimulates the activation of vitamin D3 precursors to calcitriol in the kidney. This circulating hormone raises blood and urinary calcium by increasing both gastrointestinal absorption of calcium and bone resorption. Renal activation of vitamin D3 is under tight feedback control. Macrophages also activate vitamin D3, but, unlike renal tubular cells, they lack feedback suppression of the activating 1alpha-hydroxylase. In large-scale epidemiologic studies, blood pressure correlated positively with serum and urinary calcium but inversely with the dietary intake of ca

The effect of dystocia and previous cesarean uterine scar on the tensile properties of the lower uterine segment.

OBJECTIVE: The remodeling of uterine connective tissue during labor can lead to the reorganization of the extracellular matrix that, in turn, may influence the biomechanical properties of the myometrial wall. We hypothesized that the stretching of the lower uterine segment in laboring women with dystocia changes the viscoelastic properties of the uterine wall. STUDY DESIGN: We tested the tensile strength of lower uterine segment myometrium in 68 pregnant women at term. The biomechanical, structural, and biochemical properties were compared among 3 groups: (1) 39 laboring women who underwent p

Effect of systolic blood pressure and carotid stiffness on baroreflex gain in elderly subjects.

BACKGROUND: Aging is associated with diminished baroreflex sensitivity (gain), which predisposes elderly people to orthostatic hypotension, syncope, and cardiovascular morbidity. Aging is also associated with systolic blood pressure (SBP) elevation and carotid artery stiffness, which may both affect baroreflex gain. METHODS: We examined the relation between SBP, carotid artery stiffness, and baroreflex gain in 34 healthy elderly (71 +/- 4 years) and 10 healthy young (31 +/- 3 years) subjects. SBP (Finapres) and carotid artery stiffness (ultrasound measures of relative carotid artery diameter

Ultrasonic parameters and relationship between compressive strength, microstructure of gall bladder stones.

Patients with symptomatic stones are at a great risk for complications and these complications are a major cause of morbidity. The gall bladder stones may have a complex structure and variable composition. In the present investigation stones have been grouped into three categories namely cholesterol, bilirubinate and mixed, and a correlation between the surface structure, ultrasonic parameters and compressive strength is estimated. A double-probe through-transmission technique was used for the ultrasonic parameters study, a universal testing instrument for hardness and a scanning electron micr

The effect of monomer-to-powder ratio on the material properties of cranioplastic.

Percutaneous vertebroplasty consists of injecting polymethylmethacrylate cement into the cancellous bone of vertebral bodies for the treatment of various lesions of the spine, including osteoporotic compression fractures. Clinicians practicing vertebroplasty commonly alter the mixture of monomer-to-powder recommended by the manufacturer in an effort to decrease viscosity and increase the working time. The purpose of the current study was to measure the effect that varying the monomer-to-powder ratio has on the compressive material properties of a cement (Cranioplastic) commonly used in vertebr

Atrophoderma elastolytica discreta.

Atrophoderma elastolytica discreta is the clinicopathologic name for a unique entity herein first described in a patient with cutaneous lesions simulating atrophoderma of Pasini and Pierini but coupled with histopathologic changes of anetoderma. The clinical and histological findings seen here have not been previously seen in the many variants of anetoderma, and, as such, they are sui generis evidence of a new entity in clinical dermatology.

[Beeswax pessary in conservative treatment of uterine prolapse].

The own method of uterine prolapse treatment with the use of beeswax pessary is presented. The described method is applied in cases where general health conditions do not allow to perform the surgery. The study revealed that the beeswax is a good material to make a vaginal pessary. It's great plasticity allows individual modelling in compliance with anatomical conditions of treated patient. That provides the proper stability of uterine position. The risk of possible urinary stress incontinence due to uterine reposition is also diminished.

[Cutaneous retraction. Data from liposuction and other clinical procedures].

The remarkable capacity of the skin to stretch is used in the skin expander technique. In parallel, the skin is also able to retract, which … a determinant factor on the potential of the skin to retract postoperatively. In practice, the skin elasticity appears to be very different in different treatment sites, and considerable variations can even be observed between two neighbouring regions. In the neck and cheeks the skin seems considerably more retractable than in other sites, and we can trace t …

Rheology of sickle cells and its role in microcirculatory dynamics.

The alterations in the viscoelastic properties of the hemoglobin S solution following deoxygenation can be correlated, on the one hand, with the gelation of the HbS molecules and, on the other hand, with changes in the rheological behavior of the deoxygenated SS erythrocytes. The abnormal rheology of SS erythrocytes forms the basis of their altered flow behavior through filter pores in vitro and microcirculation in vivo. The results support the notion that the rheological behavior of sickle cells is an important link between the molecular events in hemoglobin S following deoxygenation and the

Viscoelastic properties of proteoglycan solutions with varying proportions present as aggregates.

Monomer and aggregated proteoglycans were prepared from pig laryngeal cartilage. Vascoelastic flow properties, comprising linear complex dynamic shear modulus, nonlinear steady-state shear-rate dependent viscosity, and primary normal stress difference, were measured in proteoglycan solutions containing varying proportions of aggregate (0-80%) and at different concentrations (10-50 mg/ml). Results were analyzed using the simple Oldroyd four-parameter nonlinear rate-type rheological equation. All solution properties were strongly dependent on proteoglycan concentration and on the proportion of a
