[Arthroscopic stabilization of a primary traumatic dislocation of the glenohumeral joint].

PURPOSE OF THE STUDY: The authors present their experience with arthroscopic stabilization of a primary traumatic dislocation of the glenohumeral joint in young patients. MATERIAL: In up to 80% of patients younger than 25 years, traumatic dislocation of the glenohumeral joint is associated with its recurrence within one year of treatment even in well performed conservative therapy. Repeated dislocations gradually damage the joint and eventually result in the development of arthritis. The articular capsule becomes loose, glenoid surface is reduced and cartilage of the humeral head is affected.

[Necrotizing subcutaneous infection by Streptococcus agalactiae].

Necrotizing soft tissue infections constitute some of the most potentially threatening infections that may be acquired in the community or in the hospital milieu as they are associated with a high mortality rate. In most cases they are produced by Streptococcus pyogenes. We report a case of a necrotizing soft tissue infection caused by Streptococcus agalactiae (group B beta hemolytic streptococcus) that involved the leg of an elderly man with chronic lymphatic leukemia and diabetes mellitus. The lesions notably improved after initiating intravenous antibiotic treatment with amoxicillin-clavuna

Return to Sport and Long-term Performance Levels in Weight Lifters Treated for Advanced Glenohumeral Arthritis With Inlay Total Shoulder Arthroplasty.

BACKGROUND: Total shoulder arthroplasty (TSA) is challenging in weight lifters due to their relatively young age and desire for sport continuation. PURPOSE/HYPOTHESIS: This study aimed to assess return to weight lifting and long-term outcomes after inlay-TSA (iTSA). It was hypothesized that iTSA would lead to high return to sport and performance. STUDY DESIGN: Case series; Level of evidence, 4. METHODS: This study included weight lifters (<65 years of age) with advanced osteoarthritis (OA) treated with iTSA (2011-2023). Radiographic analysis established glenoid morphology (Walch classificat

Subpectoral biceps tenodesis with BicepsButton fixation in the young population: which technique works best?

BACKGROUND: Injuries of the long head of the biceps (LHB) tendon are a prevalent source of anterior shoulder pain and are commonly treated with tenodesis. Not only a stable fixation of the LHB but also anatomic restoration of the length-tension relationship plays a central role in providing satisfactory functional and cosmetic outcomes, especially in young patients. We report the clinical outcomes of 2 different subpectoral tenodesis techniques using unicortical button fixation. METHODS: Patients aged ≤ 50 years who were treated between April 2015 and January 2020 with 1 of the 2 following su

Proximal humeral fractures treated with an intramedullary cage and plate: clinical and radiographic outcomes at a minimum of 1 year postoperatively.

BACKGROUND: Proximal humeral fractures are common injuries. Although certain fracture types may benefit from surgery including open reduction-internal fixation (ORIF), the optimal method for fixation is unclear. Newer implant designs that improve healing by minimizing hardware failure and recurrent fracture displacement may optimize clinical outcomes. METHODS: Over a 27-month period, 37 consecutive patients with proximal humeral fractures were treated by a single surgeon with a lateral humeral plate through which an intramedullary nitinol cage was inserted. Additional screws were placed throu

Construction of efficacious gait and upper limb functional interventions based on brain plasticity evidence and model-based measures for stroke patients.

For neurorehabilitation to advance from art to science, it must become evidence-based. Historically, there has been a dearth of evidence from which to construct rehabilitation interventions that are properly framed, accurately targeted, and credibly measured. In many instances, evidence of treatment response has not been sufficiently robust to demonstrate a change in function that is clinically, statistically, and economically important. Research evidence of activity-dependent central nervous system (CNS) plasticity and the requisite motor learning principles can be used to construct an effica

Radiographic Healing and Observed Complications Following Light-Cured Polymer Immobilization: A Retrospective Cohort Study of 108 Patients.

Purpose: Traditional plaster and fiberglass casts remain widely used for fracture immobilization but are associated with recognized challenges, including skin irritation, hygiene limitations, and distress during cast removal, particularly in pediatric populations. Light-cured polymer immobilization (LCPI) systems have been introduced as an alternative method of fracture support. The primary objective of this study was to describe radiographic healing and alignment outcomes among patients treated with an LCPI system. Secondary objectives were to document skin- and device-related events and to i

Surgical Fixation Versus Cast Immobilization for Adults With Bicortical Scaphoid Fractures: A Target Trial Emulation of the SWIFFT Trial.

AIMS: The Scaphoid Waist Internal Fixation for Fractures Trial (SWIFFT) reported no notable difference in functional outcomes between surgical fixation and cast immobilization for minimally displaced scaphoid waist fractures in UK settings. We aimed to emulate SWIFFT using Chinese hospital electronic health record (EHR) data to evaluate whether similar treatment effects are observed in a population with different risk profiles and to identify subgroups that may benefit differentially from surgery. METHODS: This target trial emulation used a clone-censor-weight design with inverse probability

Total Hip Arthroplasty for Advanced Femoral Head Osteonecrosis in a 24-Year-Old Competitive Wrestler: A Case Report.

INTRODUCTION: In chronic pectoralis major (PM) tendon tears, muscle retraction and involution of the residual tendon usually preclude the use of direct repair, and augmentation methods are commonly required. Investigators have tried to find ways to maximize biomechanical fixation strength, with studies focusing mainly on methods of bony fixation and showing few differences among fixation techniques. Achilles allograft reconstruction of chronic pectoralis tendon ruptures is a viable treatment option. CASE REPORT: A 38-year-old male weightlifter presented with a 2-year-old left-sided PM tendon

Effects of Mobility-Fit, a tailored multicomponent physical activity program with upper-limb emphasis, on strength, mobility and fall risk among older adults in long-term care: a cluster randomised controlled trial.

BACKGROUND: Older adults in long-term care (LTC) face accelerated functional decline due to frailty, affecting mobility and quality of life (QoL). Upper-body strength is crucial for daily activities and injury prevention, yet conventional physical activity (PA) programs often overlook it. This cluster randomised controlled trial evaluated Mobility-Fit, a 12-week multicomponent PA program emphasising upper-limb and core strength among LTC residents. METHODS: Twenty LTC facilities were randomised into the Mobility-Fit (10 facilities, n = 73) or control (10 facilities, n = 73) group. Mobility-Fi

Transcutaneous vagus nerve stimulation as a pain modulator in knee osteoarthritis: a randomized controlled clinical trial.

BACKGROUND: Our understanding of osteoarthritis (OA) has evolved from a degenerative disease to one in which low-grade, chronic inflammation plays a central role. In addition, evidence suggests that OA is accompanied by both peripheral and central nervous system sensitization that can cause pain. It has been demonstrated that transcutaneous vagus nerve stimulation (tVNS) can relieve pain, inflammation, and central sensitization in other conditions including fibromyalgia, pelvic pain, and headaches. We aimed to assess the efficacy and safety of tVNS on nociceptive pain, central sensitization, a

Proximal median nerve neuropathy: electrodiagnostic and ultrasound findings in 62 patients.

OBJECTIVES: Proximal median nerve (PMN) neuropathies are caused by lesions proximal to the carpal tunnel, which include the forearm, elbow, upper arm, and brachial plexus. Differentiating between carpal tunnel syndrome and PMN neuropathies is important to guide management and is based on clinical, electrodiagnostic (EDX), and ultrasound (US) findings. This study describes the clinical, EDX, and US features in 62 patients with PMNs. METHODS: All patients underwent EDX studies, and 52 (83.9%) had a US study. The patients were assigned to one of the following four localization zones of PMN neuro

Locking plate fixation in comminuted coronoid fractures with partial loss of the articular cartilage - Of basal-1 type according to the O'Driscoll classification.

BACKGROUND: We have attempted to restore the arc of motion by considering trochlear-coronoid articulation as a complete circle during fixation of the coronoid, even for comminuted coronoid fractures with partial loss of articular cartilage (CCFPLAC), using various kinds of locking plates. Herein, we report the radiological and clinical outcomes after fixation of the basal-1 type of CCFPLAC (O'Driscoll classification) using our method. METHODS: Thirty-one patients diagnosed with CCFPLAC were admitted between January 2012 and December 2020. Sixteen of these patients met the inclusion/exclusion

Minimally invasive open reduction of greater tuberosity fractures by a modified suture bridge procedure.

BACKGROUND: Most greater tuberosity fractures can be treated without surgery but some have a poor prognosis. The surgical procedures for avulsion fractures of the humeral greater tuberosity include screw fixation, suture anchor fixation, and plate fixation, all of which have treatment-associated complications. To decrease surgical complications, we used a modified suture bridge procedure under direct vision and a minimally invasive small incision to fix fractures of the greater tuberosity of the humerus. AIM: To investigate the clinical efficacy and outcomes of minimally invasive modified sut
