We present a patient from Germany with Hantavirus infection, admitted in the Emergency room of our hospital, with fever, thrombocytopenia, acute renal failure, oliguria, mild proteinuria and hematuria. Percutaneous renal biopsy revealed an acute interstitial nephritis without medulla haemorrhages. The virus infection confirmation was made by detection of IgM against Hantavirus Puumala. This infection should be considered in patients with thrombocytopenia, fever and acute renal failure, over all if they are from North and Central Europe.

When confronted by the combination of initial high fever associated with intense malaise, splenomegaly, elevated levels of transaminases, and acute renal failure, consideration must be given to the differential diagnosis of leptospirosis even in Germany. As a rule, the diagnosis is confirmed by serological testing based on the titer curve. Renal involvement is frequent, but usually has a good prognosis, especially if jaundice has not developed. Treatment with doxycycline or penicillin can shorten the disease course and exudation, possibly also the nephritis, or hinder it.

We describe a 35-year old patient with nodular fasciitis, erythema migrans, and gonarthritis four months after a bite of a Borrelia afzelii infected tick. The Borrelia afzelii infection was identified by a polymerase chain reaction and direct sequencing of the amplification product. Borrelia-specific DNA was also detectable in nodular fasciitis tissue. We therefore conclude that Borrelia afzelii can be a causative agent of nodular fasciitis and Lyme arthritis in a highly endemic region of Northern Germany.

Falsely high serum triglyceride concentrations (410-850 mg/dl) were measured in four members of a family of five from Franconia in Germany. The cause was hyperglyceridaemia on the basis of glycerol kinase deficiency. None had any symptoms and no other metabolic anomaly was demonstrated. The possibility of glycerol kinase deficiency should be considered in any case of elevated serum triglyceride concentration but with clear serum, normal lipid electrophoresis and lack of response to lipid-lowering measures.

Only those pneumoconioses induced by organic dusts are discussed which are met with in Germany. They comprise: byssinosis which is caused by toxic agents in cotton and flax, farmer's lung, sprayer's fever, exogenous allergic alveolitis due to thermophil actinomyces in the dust of mouldy agricultural meterials or the air of air-conditioning systems, bird fancier's lung which is caused by antigens in animal waste products. Byssinosis and farmer's lung have now been acknowledged as occupational diseases.

The questionnaire sent to all practising dermatologists in Germany investigated the frequency of certain possible serious side effects. The new once daily 500 mg griseofulvin tablet will have to be in clinical use for some time yet before its efficacy can be assessed.

The shortage of donor kidneys is the main cause of the even insufficient supply of patients suffering from end stage renal disease. In 1987 a transplant coordinator to the districts Berlin, Frankfurt and Potsdam with 3.1 Mio inhabitants was introduced. The functions of the coordinator and the approach to organ procurement are described in detail. In comparison with 1986 the number of explanted kidneys could doubled in 1988 (68 vs 143). The number of 46.3 explanted kidneys per million inhabitants is comparable with other areas or countries with excellent organ procurement system.
