[Preventive health care in travel, especially vaccinations].

Nearly 40 million journeys abroad were recorded from the Federal Republic of Germany last year. 60-70% of travellers going to southern countries seek medical advice for preventive measures, particularly in Public Health centres. Inquiries for vaccinations are prevalent. Current aspects of immunization against yellow fever, cholera, tetanus, polio, typhoid fever, hepatitis A, hepatitis B, rabies meningococcal meningitis, European tick-borne encephalitis, measles and tuberculosis are discussed. Finally, some remarks on malaria prevention, hygiene, health insurance and information services are gi…

Accuracy of radiographs in classification of impacted third molar teeth.

The accuracy of using panoramic radiographs as the basis for classification of impacted third molars and subsequent fee reimbursement by third party carriers was evaluated. Radiographs showing 20 impacted third molar teeth were classified independently by 40 oral and maxillofacial surgeons, 10 general dentists, and 11 health insurance consultants. No group had an accuracy rate better than approximately 50%. These findings demonstrate the unreliability of radiographs for classification of impacted teeth, irrespective of the training or experience of the evaluator.

[Posterior fossa infarct misdiagnosed as acute peripheral vestibulopathy].

Patients with acute central vestibular syndrome (AVS) can mimic acute peripheral vestibulopathy, which can mislead to a diagnosis of posterior fossa infarcts. Delayed diagnosis will prevent relevant treatment and may lead to severe disability and in worst case death. Understanding of AVS is extremely relevant for physicians in hospital and prehospital care to insure the right treatment. I this case report of a 35-year-old male patient with AVS the correct diagnosis was made relatively late.

Visualization of the probe tip during lacrimal outflow system probing and intubation.

Probing of the lacrimal outflow system is commonly performed by ophthalmologists for treating nasolacrimal duct obstruction in infants or for intubating the lacrimal system. Visualization of the probe tip in the inferior meatus during probing insures the surgeon of reaching the nose with the probe. Similarly, visualization of the probe is the key to success in intubation irrespective of the retrieval system used. Rotation of a curved metal probe plus infracturing of the inferior turbinate after shrinking the nasal mucosa with cocaine allows such visualization in most instances.

Integrating Kidney Health into Universal Health Coverage Beyond Dialysis: Lessons from Thailand for Southeast Asia.

PurposeChronic kidney disease (CKD) represents a growing health and equity challenge in Southeast Asia; however, kidney care remains insufficiently integrated into universal health coverage (UHC). This policy brief examines how countries in Southeast Asia can translate growing policy recognition of kidney health into financed and measurable healthcare system action, drawing primarily on Thailand's experience.Policy ContextWHO Resolution WHA78.6 has recognized kidney disease as an NCD priority requiring prevention, management, and surveillance at the national level. In Southeast Asia, its imple

Poverty and access to health care: the political economy of redesigning user charges in the context of fiscal pressure.

Global and regional commitments to universal health coverage emphasize reducing financial hardship due to out-of-pocket payments for health care. Despite this, many countries continue to rely on user charges-either to raise revenue or reduce demand-especially under fiscal pressure. We conducted a narrative review of academic literature on the theoretical basis for and empirical effects of user charges in health systems. This was complemented by recent case studies from Slovenia, Estonia, and Cyprus, selected to illustrate diverse approaches to user charge policy under fiscal constraints. Commo

Healthcare-associated infections and medical malpractice claims in the Veneto Region (2010-2024): a retrospective analysis of regional claims data.

BACKGROUND: Healthcare-associated infections (HAIs) are a major clinical and medico-legal concern, as they are associated with increased morbidity, mortality, and healthcare costs. Severe HAIs may lead to malpractice claims related to failures in prevention or management. Although compensation claims capture only a fraction of HAI episodes, they mainly reflect the most serious cases and provide relevant clinical and medico-legal information. This study analyses trends in malpractice compensation claims related to HAIs in the Veneto Region (Italy). METHODS: A retrospective analysis of medical

Medication Safety in Anesthesiology: A Closed-Claims Analysis.

BACKGROUND: Despite longstanding guidelines for safe medication administration during anesthesia care, medication errors continue to be an area of opportunity in perioperative patient safety. Analysis of closed claims can help identify contributing factors involving patients, health care providers, and medication, and suggest opportunities for reducing harm. METHODS: A claims database from a national malpractice insurer was queried for closed claims-with or without paid indemnity-from 2012 to 2022 involving medication-related liability in anesthesia. We performed a descriptive analysis of the

Medicare Reimbursement for Primary Hip and Knee Arthroplasty Is Disproportionately Decreasing Relative to Other High-Volume Inpatient Procedures: Leader of the Pack.

BACKGROUND: Decreasing Medicare surgeon reimbursement for total hip arthroplasty (THA) and total knee arthroplasty (TKA) may limit access to care. The purpose of this study was to examine trends in Medicare reimbursement fees for primary THA and TKA and compare reimbursement to other high-volume nonarthroplasty procedures. METHODS: A nationally representative database was retrospectively queried to identify the most frequently performed inpatient procedures in the United States from 2016 to 2020. Making sense of the economics of abortion in the United States.

In 2023 the editors of Perspectives on Sexual and Reproductive Health issued a special call for papers related to the economics of abortion. Ten of those submissions are included in this volume and address critical issues including: (1) the role Medicaid continues to play in abortion access and how changes in state Medicaid coverage of abortion have expanded and restricted abortion care use; (2) how low-income individuals without insurance coverage for abortion utilize resources from abortion funds and through crowdsourcing platforms; (3) how the price of medication abortion has decreased with…

Is the neglected tropical disease mass drug administration campaign approach an effective strategy to deliver universal health coverage? A case study of the Liberia neglected tropical disease programme.

BACKGROUND: Access to affordable, quality healthcare is the key element of universal health coverage (UHC). This study examines the effectiveness of the neglected tropical disease (NTD) mass drug administration (MDA) campaign approach as a means to deliver UHC, using the example of the Liberia national programme. METHODS: We first mapped the location of 3195 communities from the 2019 national MDA treatment data reporting record of Liberia. The association between coverage for onchocerciasis and lymphatic filariasis treatment achieved in these communities was then explored using a binomial geo

Retrospective State Medicaid Claims Analysis of Children and Adults With Obsessive-Compulsive Disorder.

OBJECTIVE: With a lifetime U.S. prevalence of 2.3%, obsessive-compulsive disorder (OCD) is a chronic condition often producing reduced quality of life and disability when left untreated. Little is known about the prevalence or treatment of diagnosed OCD in public behavioral health systems. METHODS: Using a claims analysis of 2019 New York State Medicaid data (N=2,245,084 children; N=4,274,100 adults), the authors investigated the prevalence and characteristics of children and adults with OCD. The authors also examined whether these individuals received treatment with medication or psychothera
