Diagnosis of urinary tract infection based on artificial intelligence methods.

BACKGROUND AND OBJECTIVE: Urinary tract infection (UTI) is a common disease affecting the vast majority of people. UTI involves a simple infection caused by urinary tract inflammation as well as a complicated infection that may be caused by an inflammation of other urinary tract organs. Since all of these infections have similar symptoms, it is difficult to identify the cause of primary infection. Therefore, it is not easy to diagnose a UTI with routine examination procedures. Invasive methods that require surgery could be necessary. This study aims to develop an artificial intelligence model

The effect of hospital care on early survival after penetrating trauma.

BACKGROUND: The effectiveness of emergency medical interventions can be best evaluated using time-to-event statistical methods with time-varying covariates (TVC), but this approach is complicated by uncertainty about the actual times of death. We therefore sought to evaluate the effect of hospital intervention on mortality after penetrating trauma using a method that allowed for interval censoring of the precise times of death. METHODS: Data on persons with penetrating trauma due to interpersonal assault were combined from the 2008 to 2010 National Trauma Data Bank (NTDB) and the 2004 to 2010

Determining the velocity required for skin perforation by fragment simulating projectiles: a systematic review.

INTRODUCTION: Explosively propelled fragments are the most common cause of injury to soldiers on current operations. Researchers desire models to predict their injurious effects so as to refine methods of potential protection. Well validated physical and numerical models based on the penetration of standardised fragment simulating projectiles (FSPs) through muscle exist but not for skin, thereby reducing the utility of such models. METHOD: A systematic review of the literature was undertaken using the Preferred Reporting Items for Systematic Reviews and Meta-Analyses methodology to identify a

Multivariate analysis of risk factors for operative delivery in nulliparous women. Canadian Early Amniotomy Study Group.

OBJECTIVE: Our purpose was to develop predictive models of operative delivery in nulliparous women on the basis of sociodemographic, anthropometric, and obstetric risk factors. STUDY DESIGN: Data were obtained prospectively on 925 nulliparous women in spontaneous term labor with a single fetus in cephalic presentation. Operative delivery was defined as either a midforceps or a cesarean delivery. Variables were grouped into two categories: those that could be assessed at admission and those appearing during labor. Multiple logistic regression was used to identify variables predictive of operat

A pooled analysis of the risk prediction models for mortality in acute exacerbation of chronic obstructive pulmonary disease.

OBJECTIVE: The prognosis for acute exacerbation of chronic obstructive pulmonary disease (AECOPD) is not optimistic, and severe AECOPD leads to an increased risk of mortality. Prediction models help distinguish between high- and low-risk groups. At present, many prediction models have been established and validated, which need to be systematically reviewed to screen out more suitable models that can be used in the clinic and provide evidence for future research. METHODS: We searched PubMed, EMBASE, Cochrane Library and Web of Science databases for studies on risk models for AECOPD mortality f

A phase III, multi-centre, double-masked randomised controlled trial of adjunctive intraocular and peri-ocular steroid (triamcinolone acetonide) versus standard treatment in eyes undergoing vitreoretinal surgery for open globe trauma (ASCOT): statistical analysis plan.

BACKGROUND: Open globe ocular trauma complicated by intraocular scarring (proliferative vitreoretinopathy) is a relatively rare, blinding, but potentially treatable condition for which, at present, surgery is often unsatisfactory and visual results frequently poor. To date, no pharmacological adjuncts to surgery have been proven to be effective. The aim of the Adjunctive Steroid Combination in Ocular Trauma (ASCOT) randomised controlled trial is to determine whether adjunctive steroid (triamcinolone acetonide), given at the time of surgery, can improve the outcome of vitreoretinal surgery in p

Development of a prediction model for infant hospitalisation and death using clinical features assessed by community health workers during routine postnatal home visits in Dhaka, Bangladesh.

BACKGROUND: To improve upon the WHO 8 danger signs used to identify young infants (<2 months) requiring referral during community health worker (CHW) home visits, aggregative features (eg, cumulative visits with fever) rather than visit-specific features (eg, fever at a single visit), and a machine learning random forest model, may enhance predictive performance. Applying these approaches, we aimed to develop a prediction model for infant hospitalisation and/or death using CHW-assessed clinical features during home visits in Dhaka, Bangladesh. METHODS: We analysed data from generally healthy

Prognostic models for COVID-19 needed updating to warrant transportability over time and space.

BACKGROUND: Supporting decisions for patients who present to the emergency department (ED) with COVID-19 requires accurate prognostication. We aimed to evaluate prognostic models for predicting outcomes in hospitalized patients with COVID-19, in different locations and across time. METHODS: We included patients who presented to the ED with suspected COVID-19 and were admitted to 12 hospitals in the New York City (NYC) area and 4 large Dutch hospitals. We used second-wave patients who presented between September and December 2020 (2137 and 3252 in NYC and the Netherlands, respectively) to eval

Interpreting risk reduction in clinical trials for pulmonary arterial hypertension.

Because of scepticism concerning study results when relying solely on relative effect estimates, the number needed to treat (NNT) has been used extensively to quantify the net clinical benefit of an intervention, and is reported increasingly in randomised trials and observational studies. This method is a simple measure representing the number of patients who would need to be treated to prevent one additional adverse event. However, like relative risk, the NNT is an inherently time-dependent measure. Thus, its calculation may lead to misleading interpretations, especially for studies involving

New model for surgical and nonsurgical therapy in adults with Class III malocclusion.

INTRODUCTION: Not all adult Class III malocclusion patients are candidates for surgical correction. In patient assessment and selection, major issues remain regarding diagnosis and treatment planning. The purpose of this investigation was to ascertain whether adding a transverse parameter to a discriminant analysis could improve the classification of adults with Class III malocclusion into 2 groups of patients: those who can effectively be treated by orthodontic therapy and those who require orthognathic surgery. METHODS: Cephalograms, plaster casts, and extraoral photos of 69 adults with Cla

Expression of inducible nitric oxide synthase in conduits used in patients with diabetes mellitus undergoing coronary revascularization.

BACKGROUND: Expression of inducible nitric oxide synthase (iNOS) is a marker of vascular inflammation which can result in thrombosis and atherosclerosis. This study was undertaken to examine the difference in iNOS expression in the internal mammary artery (IMA) and saphenous veins (SVs) of patients with diabetes mellitus undergoing coronary artery bypass graft (CABG) surgery using both qualitative and quantitative methodology. METHODS: Segments of IMA and SV harvested in 100 diabetic patients with diabetes mellitus undergoing CABG surgery were fixed in formalin and immunostained to detect the

Plasma concentrations of tumour necrosis factor-alpha, tumour necrosis factor-related apoptosis-inducing ligand, and FasLigand/CD95L in patients with Chagas cardiomyopathy correlate with left ventricular dysfunction.

AIMS: Cardiomyocyte apoptosis is reported to be involved in the pathogenesis of human chronic Chagas cardiomyopathy (CCC). Members of the tumour necrosis factor (TNF) superfamily (TNF-alpha, FasLigand/CD95L, and TNF-related apoptosis-inducing ligand) are known to activate the death receptor pathway. We therefore investigated whether levels of TNF-alpha, FasLigand/CD95L, and TRAIL correlated with changes in heart function of patients with Chagas disease (n = 31). METHODS AND RESULTS: Concentrations of TNF-alpha and TRAIL were clearly augmented in individuals with severe form CCC (n = 16). Leve

Integrating microarray and proteomics data to predict the response on cetuximab in patients with rectal cancer.

To investigate the combination of cetuximab, capecitabine and radiotherapy in the preoperative treatment of patients with rectal cancer, fourty tumour samples were gathered before treatment (T0), after one dose of cetuximab but before radiotherapy with capecitabine (T1) and at moment of surgery (T2). The tumour and plasma samples were subjected at all timepoints to Affymetrix microarray and Luminex proteomics analysis, respectively. At surgery, the Rectal Cancer Regression Grade (RCRG) was registered. We used a kernel-based method with Least Squares Support Vector Machines to predict RCRG base

Agitated "unipolar" depression re-conceptualized as a depressive mixed state: implications for the antidepressant-suicide controversy.

BACKGROUND: The nosologic status of agitated depression is unresolved. Are they unipolar (UP) or bipolar (BP)? Are they mixed states? Even more controversial is the notion that antidepressants might play some role in the suicidality of such patients (Akiskal and Mallya, 1987) [Akiskal, H.S., Mallya, G., 1987. Criteria for the "soft" bipolar spectrum: treatment implications. Psychopharmacol Bull. 23, 68-73]. METHODS: After excluding all patients with history of hypomanic episodes occurring outside the frame of a major depressive episode (MDE), even those with a shorter duration of hypomanic sy

Psychosocial, behavioral, and medical outcomes in children with epilepsy: a developmental risk factor model using longitudinal data.

OBJECTIVE: We studied factors predicting the risk of adverse long-term psychosocial, behavioral, and medical outcomes in children with epilepsy. METHODS: Children (N = 157, 4.5 to 13 years) were enrolled in a prospective longitudinal study when first seen. Potential subjects were excluded if they were moderately or severely mentally retarded, had motor or sensory handicaps interfering with testing, or did not speak either English or Spanish. MEASURES: To develop risk predictors, we collected information regarding the child's medical and seizure history, cognitive functioning, and behavior pr

Combining independent studies of a diagnostic test into a summary ROC curve: data-analytic approaches and some additional considerations.

We consider how to combine several independent studies of the same diagnostic test, where each study reports an estimated false positive rate (FPR) and an estimated true positive rate (TPR). We propose constructing a summary receiver operating characteristic (ROC) curve by the following steps. (i) Convert each FPR to its logistic transform U and each TPR to its logistic transform V after increasing each observed frequency by adding 1/2. (ii) For each study calculate D = V - U, which is the log odds ratio of TPR and FPR, and S = V + U, an implied function of test threshold; then plot each study
