Time-Motion Analysis of Four Automated Systems for the Detection of Chlamydia trachomatis and Neisseria gonorrhoeae by Nucleic Acid Amplification Testing.

Next-generation diagnostics for Chlamydia trachomatis and Neisseria gonorrhoeae are available on semi- or fully-automated platforms. These systems require less hands-on time than older platforms and are user friendly. Four automated systems, the ABBOTT m2000 system, Becton Dickinson Viper System with XTR Technology, Gen-Probe Tigris DTS system, and Roche cobas 4800 system, were evaluated for total run time, hands-on time, and walk-away time. All of the systems evaluated in this time-motion study were able to complete a diagnostic test run within an 8-h work shift, instrument setup and operatio…

Photoinduced electron transfer and fluorophore motion as a probe of the conformational dynamics of membrane proteins: application to the influenza a M2 proton channel.

The structure and function of the influenza A M2 proton channel have been the subject of intensive investigations in recent years because of their critical role in the life cycle of the influenza virus. Using a truncated version of the M2 proton channel (i.e., M2TM) as a model, here we show that fluctuations in the fluorescence intensity of a dye reporter that arise from both fluorescence quenching via the mechanism of photoinduced electron transfer (PET) by an adjacent tryptophan (Trp) residue and local motions of the dye molecule can be used to probe the conformational dynamics of membrane p…

Complication rates, dislocation, pain, and postoperative range of motion after reverse shoulder arthroplasty in patients with and without repair of the subscapularis.

BACKGROUND: Despite improved results with reverse shoulder arthroplasty (RSA), questions still remain regarding certain technical aspects of the operation. One particular area of question is the effect of subscapularis repair on complication rates, dislocation, pain, and overall range of motion. Some authors suggest that when a deltopectoral approach is used, not repairing the subscapularis leads to a higher complication rate, especially for dislocation. MATERIALS AND METHODS: From a reverse total shoulder arthroplasty database of 3 surgeons at 1 institution, we identified 55 patients who und…

Using a nurse invented T-Bar device in a rehabilitation program improved the range of motion for rotator cuff repair patients.

AIMS AND OBJECTIVES: This study investigates the effects on patient outcomes of using a T-bar in rehabilitation programs in shoulder arthroscopic surgical procedure patients. BACKGROUND: Orthopaedic nurses play an important role in facilitating restoration of patients' range of motion shoulder function. Conventionally, nurses instruct patients to use the unaffected arm to hold the surgical arm when performing range of motion. However, patients often have difficulty performing the entire range of motion efficiently in this manner. Therefore, nurses have invented a T-bar device to help patients…

Changes in the range of motion of the cervical spine and adjacent segments at ≥24 months after uninstrumented corpectomy for cervical spondylotic myelopathy.

BACKGROUND: Few clinical studies have described the changes in the range of motion (ROM) of the cervical spine and adjacent segments following central corpectomy. We aimed to quantify the changes in range of motion (ROM) of the cervical spine and the adjacent segments at ≥24 months following uninstrumented central corpectomy (CC) for cervical spondylotic myelopathy (CSM) and to determine the contribution of the adjacent segments to the compensation for loss of motion of the cervical spine following CC. METHODS: Preoperative and follow-up lateral cervical spine radiographs of 36 patients who u…

Ocular pursuit and the estimation of time-to-contact with accelerating objects in prediction motion are controlled independently based on first-order estimates.

The present study examined for the first time both the ocular and manual responses in a prediction motion (PM) task requiring participants to estimate time-to-contact (TTC) of an accelerating object. Results showed that while the ocular response initially matched well the object motion, smooth pursuit decayed towards zero following object occlusion, during which participants exhibited a saccadic response that placed the eyes in the region of the point of contact. The primary saccade was completed in advance of the object reaching the point of contact, and was best predicted by a first-order es…

Immunohistochemical analysis of the extracellular matrix in the posterior capsule of the zygapophysial joints in patients with degenerative L4-5 motion segment instability.

OBJECT: Although the hypertrophied shape of the zygapophysial joints in degenerative instability of the lumbar spine is well known, its unde … of intervertebral disc height that occurred in all patients with severe degeneration of the lumbar motion segment promotes an increased range of axial rotation that places the posterior capsule under greater mechanical load. Compared with normal joints studied previously, the posterior capsules involved in these degenerative joint complexes were hypertr …

Role of collateral flow in a pharmacological stress test (a combination of low-dose dobutamine and a vasodilator) as a predictor of wall motion reversibility.

The role of collateral flow was evaluated in a pharmacological stress test [a combination of low-dose dobutamine (DOB) and a vasodilator] as a predictor of wall motion reversibility at rest after percutaneous transluminal coronary angioplasty (PTCA) using ultrafast computed tomography (UFCT). Segments with wall motion abnormalities before PTCA were divided into two groups; ie, either with or without collateral flow. Patients were scanned at rest for baseline and again after 5 min of intravenous administration of 4 micrograms/kg per min of DOB after nitroglycerin (0.3 mg sublingually) or isosor…

Evaluation of mitral leaflet motion by echocardiography and jet direction by Doppler color flow mapping to determine the mechanisms of mitral regurgitation.

OBJECTIVES: This study was designed to assess the accuracy of two-dimensional and Doppler echocardiography in determining the mechanism of mitral regurgitation, as compared with direct inspection of the valve at operation. BACKGROUND: Valve repair for mitral regurgitation offers substantial advantages over valve replacement, but it is technically more demanding and requires understanding of the mechanism of dysfunction. METHODS: We studied 286 patients undergoing mitral valve repair. Intraoperative two-dimensional echocardiography was used to classify mitral leaflet motion as excessive, norm…

Studies on motion sickness caused by high curve speed railway vehicles. Evaluation of the swing and its effects on passengers and conductors.

The high curve speed railway vehicles (HCSRVs) of the Japanese National Railway have been operating since 1973 with the aim of increasing speed on ordinary routes with many curve track sections. Although the aim of increased speed has been attained, it has been pointed out that the swing of HCSRVs is stronger than that of ordinary-type trains and it may increase motion sickness among the passengers and conductors. In this study, the authors examined motion sickness symptoms among 119 passengers and 100 conductors of both trains with a self-administered questionnaire, and evaluated the vibratio…

A reduction of vestibulo-visual integration during transdermally administered scopolamine and dimenhydrinate. A presentation of gain control theory in motion sickness.

The effect of transdermally administered scopolamine (TTS scopolamine) (release rate 5 micrograms/h) and dimenhydrinate (100 mg) was examined on optovestibular nystagmus in 16 volunteers in a randomized double-blind trial. A statistically significant decrease in the optokinetic part of nystagmus was observed during all treatments. Most profound reduction was found during treatment with two TTS scopolamine. The vestibular part was reduced by treatment with two TTS scopolamine, only. The results indicate that scopolamine and dimenhydrinate exert their effect in motion sickness by reducing the ve…

Clinical application of basic research on continuous passive motion for disorders and injuries of synovial joints: a preliminary report of a feasibility study.

Since 1970, when the concept of continuous passive motion (CPM) was originated by one of the authors (R.B.S.), he and a succession of his research fellows have investigated its biological effects on the healing and regeneration of articular tissues in a wide variety of experimental models in rabbits. From this basic research he concluded that CPM is well tolerated, seems to be painless, stimulates the healing and regeneration of articular tissues, prevents joint stiffness, and permits the normal healing of arthrotomy incisions. Beginning in 1975, one of the authors (H.W.H.), and in 1978, the r…

Cervical spine motion during tracheal intubation with videolaryngoscopy versus macintosh laryngoscopy in adults undergoing cervical spine surgery: a randomized controlled trial.

BACKGROUND: Excessive cervical spine motion during tracheal intubation may increase neurological risk in individuals with cervical spine pathology. Videolaryngoscopy may reduce cervical motion compared with Macintosh direct laryngoscopy, but any biomechanical reduction should be distinguished from neurological protection. METHODS: Adults scheduled for elective cervical spine surgery were randomized 1:1 to intubation with a ClearVue videolaryngoscope (group V) or a Macintosh laryngoscope (group M) under manual in-line stabilization. Lateral fluoroscopy was acquired immediately before laryngosc…

Assessing Nonverbal Interaction Dynamics in Therapeutic Relationships: a Proof-of-Concept Study using Motion Energy Analysis and Nonlinear Time-Series Analysis.

This study examined nonverbal synchronization as a means of assessing relational processes within forensic youth psychiatry. The primary aim was to explore whether Movement Energy Analysis (MEA) combined with Cross-Recurrence Quantification Analysis (CRQA) can provide clinically meaningful insight into therapist-client interaction dynamics and potentially inform clinical supervision. We investigated synchronized bodily movement in two therapeutic relationships involving the same client: one with a key worker and one with a process supporter, whose roles imply distinct therapeutic behaviors. We
