Mediastinal emphysema results from multiple etiologies and is associated with a variety of underlying disease entities. Twenty five patients with pneumomediastinum treated on a surgical service over a 6 year period were reviewed. Mycotic aneurysms of the extracranial carotid artery are rare. We describe a case of a patient with an aneurysm of extracranial internal carotid artery, presented as a swelling at the right submandibular triangle. Carotid angiography revealed a saccular aneurysm. During the operation, an abscess, due to Salmonella, was found around the aneurysm. Parathyroid carcinoma is a rare tumor that is prone to recurrence and poor local-regional control. Despite advances in technologies that have shown promise for accurate diagnosis, the mainstay of initial diagnosis remains pathologic analysis and clinical assessment. A surgeon's intraoperative analysis is important in managing patients with parathyroid carcinoma. If parathyroid carcinoma is suspected intraoperatively, a more aggressive surgical strategy should be implemented. This article presents a case series and summary of the existing parathyroid carcinoma literature.
