Personalized medicine: Prediction of disease vulnerability in mood disorders.

Personalized or precision medicine is a medical discipline that proposes tailoring health care to each individual by integrating data from their genetic makeup, epigenetic modifications, other biomarkers, clinical symptoms and environmental exposures. Currently, patients typically present for treatment of mood disorders relatively late in the disease course and this is of great concern both because delay in attaining remission reduces the success of subsequent treatment and depressive episodes have negative cumulative effects on the brain and body. In this article we will discuss progress in p

Bacillus cereus in personal care products: risk to consumers.

Bacillus cereus is ubiquitous in nature and thus occurs naturally in a wide range of raw materials and foodstuffs. B. cereus spores are resistant to desiccation and heat and able to survive dry storage and cooking. Vegetative cells produce several toxins which on ingestion in sufficient numbers can cause vomiting and/or diarrhoea depending on the toxins produced. Gastrointestinal disease is commonly associated with reheated or inadequately cooked foods. In addition to being a rare cause of several acute infections (e.g. pneumonia and septicaemia), B. cereus can also cause localized infection o

Comparing physicians personal prevention practices and their recommendations to patients.

BACKGROUND: Hypothetical choice studies suggest that physicians often take more risk for themselves than on their patient's behalf. OBJECTIVE: To examine if physicians recommend more screening tests than they personally undergo in the real-world context of breast cancer screening. DESIGN: Within-subjects survey. PARTICIPANTS: A national sample of female obstetricians and gynecologists (N = 135, response rate 54%) from the United States. In total, they provided breast care to approximately 2,800 patients per week. MEASURES: Personal usage history and patient recommendations regarding mammog

Personal, dyadic, and contextual resilience in parents experiencing homelessness.

Adopting a developmental psychopathology (DP) perspective, the present study systematically reviewed the quantitative literature on positive functioning and outcomes in parents experiencing homelessness. Studies were identified from PubMed, PsycInfo, and Web of Science using an exhaustive list of key terms. Of 3443 total studies screened, 219 were inspected, 176 were excluded, and 43 were included. Included studies fell into three outcome categories: the ability to function well personally (cope effectively, meet basic family needs, experience reduced psychopathology); dyadically (demonstrate

A single-person reduction and splinting technique for ankle injuries.

Ankle injuries are one of the most common orthopaedic conditions treated in the emergency department. Initial reduction and splinting techniques of these injuries are variable and can place undue stress on the physician and cause patient discomfort. Novice and experienced practitioners have had to repeat splint application because of poor preparation, variable assistant experience, loss of fracture reduction, and improper application. We present a modified Quigley technique for ankle reduction and splinting that simplifies these issues and reduces stress on the patient and physician. The techn

Ebola: Personal view from the field--Sierra Leone.

Experiences of healthcare workers responding to the Ebola epidemic in West Africa vary with the types of facility. Patients suspected of having Ebola virus disease (EVD) must be isolated from each other as well as from the wider community until testing is complete; in Sierra Leone such facilities were called Ebola holding units (EHUs). Once EVD was confirmed, patients were moved to Ebola treatment units (ETUs), where they could be cohorted together safely and treatment efforts focused on EVD itself. While a number of purpose-built units combined an EHU with an ETU, my personal experience was o

[Mantle cell lymphoma: Towards a personalized therapeutic strategy?].

Mantle cell lymphoma (MCL) is a clinically heterogeneous non-Hodgkin lymphoma with an aggressive clinical behaviour and short survival in some cases and an indolent course in others. Advances in the biology and pathogenesis of MCL have unveiled several genes involved in deregulation of cell cycle checkpoints and the finding of subclonal populations with specific recurrent mutations (p53, ATM, NOTCH2) with an impact on disease progression and refractoriness to treatment. Prognostic stratification helps to distinguish between indolent and aggressive forms of MCL. Currently, younger fit patients

Restoring adult stereopsis: a vision researcher's personal experience.

In February 2012, the author acquired improved stereoscopic vision after viewing Martin Scorsese's film Hugo in 3D. The author had been deficient in stereo vision all of his life because in the first two decades, one eye deviated outward from the gaze position of the other. At that time, he was an alternator (alternating exotrope) showing strabismus (>20 prism diopters) without amblyopia. After viewing the 2-hour film, the Wirt stereo threshold decreased from 200 to 80 arcsec, and stereoscopic vision became a vivid experience. Exophoria decreased to 7 prism diopters. Antagonist models for treating persons with substance use disorders.

This paper provides an overview on the status of antagonist models for treating patients with substance use disorders. It begins with an overview describing the ambivalence about stopping or not stopping substance use and how antagonist approaches, combined with psychosocial treatment, are aimed to address it. It then goes on to review data on disulfiram and acamprosate treatment of alcohol dependence and naltrexone treatment of opioid and alcohol dependence. The superior results achieved by extended release formulations are emphasized. The mixed findings on naltrexone treatment for amphetamin

Barriers to involvement in physical activities of persons with mental illness.

Participating in physical activities could be essential for reducing the multiple risk factors for health problems that persons with severe mental illness (SMI) may suffer. However, people with SMI are significantly less active than the general population. To develop knowledge about factors related to the perceived barriers hindering this population's participation in physical activities and the benefits this participation would have, a study was conducted in Israel with 86 people with mental illness living in community mental health facilities prior to their participation in a health promotio
