Six patients with positional compression (PC) of the neck and lower third of the face are described. This condition, virtually never described in maxillofacial surgery, was caused by forced posture of a patient sleeping after alcoholic intoxication. Characteristic clinical signs of PC syndrome are described in patients with different duration and severity of the process. Differential diagnosis and pathogenetic therapy are discussed.

Desaturation after induction of anaesthesia and repositioning due to mucus plug causing atelectasis (lung collapse) is a rare event. We present a case of intraoperative right lung collapse due to mucus plug in a patient undergoing left laparoscopic nephrectomy. Hypoxaemia occurred after the induction of anaesthesia and repositioning. X-ray chest revealed right lung collapse and surgery was subsequently postponed. The effect of right- and left-brain lesions on pentobarbitone-chlorpromazine induced tremor-bursts was studied in guinea pigs. The left-brain transections did not affect these tremor bursts except post-thalamic sections, which caused a complete disappearance of extensor jerks with coinciding tremors at four legs. These tremor bursts simultaneously occurring at four legs completely disappeared after a right-forebrain transection at the level of the rostral striatum. A 39 year old man with acute panautonomic and mild somatic neuropathy had severe postural hypotension 1 week after onset. Porphyric neuropathy was excluded. The final diagnosis was Guillain-Barré syndrome (GBS). After 2 months he began to recover progressively and after 9 months he presented asymptomatic postural hypotension. We consider the hypothesis of a spectrum of clinico-pathological entities at one end of which lies GBS with autonomic signs and at the other acute pure dysautonomia. The site of the autonomic lesion, might have been in post-ganglionic sympathetic fibers and vagus nerve.
