Sleep problems and daytime behavior in childhood idiopathic epilepsy.

PURPOSE: To evaluate the presence of sleep problems and their association with behavioral and adjustment problems in children with idiopathic epilepsy. METHODS: A parental questionnaire was used to assess sleep problems in 89 children with idiopathic epilepsy for comparisons with 49 siblings and 321 healthy control children, equally distributed for age and sex. Sleep problems were clustered into five factors: parasomnias, parent/child interaction during the night, sleep fragmentation, daytime drowsiness, and bedtime difficulties. Daytime behavior and psychological adjustment were assessed by …

Does sleep fragmentation impact recuperation? A review and reanalysis.

Studies have shown that next-day performance and alertness are impaired by sleep fragmentation procedures even when total sleep time (TST) is unaffected. Based on these studies it has been hypothesized that both the duration and continuity of sleep determine its recuperative value. This review of the literature suggests that when sleep fragmentation procedures increase the relative amount of stage 1 sleep, next-day performance and alertness are impaired. Other studies suggest that stage 1 sleep has little or no recuperative value. Total sleep time, however, is typically defined as the sum of t…

Obstructive sleep apnoea syndrome in hereditary gelsolin-related amyloidosis.

Gelsolin-related amyloidosis (AGel amyloidosis) is a rare autosomal dominant disorder, reported worldwide in kindreds carrying a G654A or G654T gelsolin gene mutation. The main clinical signs are cutis laxa, cranial and peripheral neuropathy, and corneal lattice dystrophy but heavy intermittent snoring also occurs. To evaluate whether sleep apnoea is present we performed nocturnal sleep recordings, cephalometric and spirometric analyses and multiple sleep latency tests (MSLT) in five snoring patients with a G654A gelsolin gene mutation. Four patients had obstructive sleep apnoea syndrome (OSAS…

Eyelid, conjunctival, and corneal findings in sleep apnea syndrome.

OBJECTIVE: To determine the prevalence of eyelid, conjunctival, and corneal findings in patients with sleep apnea syndrome (SAS). DESIGN: Case series. PARTICIPANTS: Seventy-two white patients referred for evaluation of suspected SAS. INTERVENTION: Complete examination of eyelids, conjunctiva, and cornea, including videokeratography. MAIN OUTCOME MEASURES: Spearman rank correlations were determined between the respiratory disturbance index (RDI) during night sleep, a value used to diagnose and grade SAS, and tear film break-up time, eyelid distraction distance, presence or absence of ocular…

Clinical and polysomnographic features of sleep-related eating disorder.

BACKGROUND: Sleep-related eating disorder is a recently described clinical syndrome that combines characteristics of both eating and sleep disorders. Nocturnal partial arousals are followed by rapid ingestion of food and subsequent poor memory for the episode. Only two case series examining this disorder have been published, and both are from the same sleep disorders center in a general hospital. METHOD: The author describes 23 consecutive cases of sleep-related eating disorder that presented to the Sleep Disorders Center at McLean Hospital. All patients were administered at a standardized cl…

Accuracy of clinical evaluation in pediatric obstructive sleep apnea.

Eighty-two children underwent polysomnography (PSG) for symptoms suggestive of obstructive sleep apnea (OSA). Symptoms reported included snoring, witnessed apneic episodes, daytime somnolence, mouth breathing, and enuresis. Tonsillar size, nasal airway patency, and percentile weight were recorded. OSA was diagnosed on PSG when obstructive events were noted and apnea + hypopnea index was five or more per hour. The overall predictive accuracy of clinical suspicion of OSA was 25 (30%) of 82. Predictive accuracies (as a percentage of those with symptoms/signs who have OSA) and prevalences (as a pe…

Upper airway imaging in sleep apnoea syndrome: clinical applications.

Symptoms of habitual snoring and excessive daytime sleepiness are extremely common in the general population, and have poor predictive value in identifying patients with "true" sleep-related disordered breathing. The upper airways are the main anatomical site responsible for snoring and sleep apnoea; therefore, their examination via different means has been quite extensively assessed. Clinical examination may point to severe micrognathia or retrognathia, grossly hypertrophied tonsils, obvious macroglossia, and oedema and inflammation of the uvula and soft palate. A recently proposed model is p…

[Adeno-tonsillar hyperplasia and obstructive sleep related respiratory disorders].

The most common cause of obstructive apnoeas in children is adenotonsillar hyperplasia. Possibilities of therapy are surgical intervention (adeniodectomy, tonsillectomy) or nasal CPAP. The decision on the most appropriate therapy must be individual. 26 children aged 2-5 years were investigated polysomnographically before therapy, after operation or with nCPAP-therapy. Polygraphy is a helpful aid in making up one's mind in favour of one of the possibilities of therapy, but also for controlling the therapeutic effect.

[Laser assisted uvulopalptoplasty treatment for obstructive sleep apnea syndrome].

71 patients of obstructive sleep apnea symdrome (OSAS) were treated by Nd:YAG laser assisted uvulopalptoplasty (LAUP). According to the atonamic features of every patient, and type of operation which should be selected, the success rate of surgery would increase, and complications would be reduced. The effect rate is 97.2% by clinic, 90.0% by polysimnograph (PSG).

Occupational exposure to organic solvents and sleep-disordered breathing.

STUDY OBJECTIVE: To investigate whether people with occupational exposure to organic solvents have a higher prevalence of obstructive sleep apnea syndrome (OSAS) than the general population and to examine the relationship between snoring and exposure to organic solvents. DESIGN AND PARTICIPANTS: Consecutive patients, aged 30-64 years, referred during a 3-year period to the sleep laboratory at Avesta Hospital, Sweden, because of suspected OSAS made up the patient groups. Following admission, patients underwent a simplified sleep apnea investigation and were divided into two groups, OSAS (n = 3…

Cardiac autonomic function during sleep in several neuropsychiatric disorders.

Cardiac autonomic activity during sleep is only very slightly influenced by the emotional state of the patient and, unlike some of the traditional tests of autonomic function, may be studied in all patients. In an attempt to evaluate autonomic function in patients with different neuropsychiatric disorders, two different methods of quantifying the changes in sympathetic and parasympathetic cardiac control during sleep were used: (1) the ratios of consecutive R-wave (R-R) intervals before and after spontaneous body movements; (2) spectral analysis of R-R intervals. It was found that more than on…

Experiences of a sleep disorders center: 1700 patients later.

Sleep studies reveal many patients to have specific sleep abnormalities different from what might be suspected from the clinical history. For example, in our experience, patients who were later found to have central sleep apnea presented with chief complaints of excessive sleepiness or insomnia. In patients requesting evaluation for sleep apnea, screening studies that detect only sleep-disturbed breathing (ie, oximetry) may miss other diagnoses in one fourth of cases.

Cheyne-Stokes respiration during sleep in congestive heart failure.

Cheyne-Stokes respiration (CSR) is a form of sleep-disordered breathing seen in approximately 40% of congestive heart failure patients with a left ventricular ejection fraction of < 40%. It is characterized by a crescendo-decrescendo alteration in tidal volume separated by periods of apnea or hypopnea. Sleep is generally disrupted, often with frequent nocturnal arousals. Clinical features include excessive daytime sleepiness, paroxysmal nocturnal dyspnea, insomnia, and snoring. Proposed mechanisms include the following: (1) an increased CNS sensitivity to changes in arterial PCO2 and PO2 (incr…

Midline glossectomy and epiglottidectomy for obstructive sleep apnea syndrome.

Obstructive sleep apnea syndrome is caused by narrowing of the pharyngeal airway and loss of dilator tone during sleep. In patients with severe apnea surgical correction often requires attention to both the oropharynx and hypopharynx. Tongue reduction surgery has been described for persistent apnea after failure of palatal surgery. We describe our experience with midline glossectomy with epiglottidectomy in 12 patients with a mean age of 48.8 +/- 14.2 years and body mass index of 36.0 +/- 8.8 kg/m2. Response to treatment was defined as a postoperative respiratory disturbance index (RDI) below …

[Neurovegetative symptoms in infants with sleep-related respiratory disorders].

A relationship between neurovegetative symptoms and an increased risk of sudden infant death (SID) has been frequently described. Such symptoms are vomiting because of gastroesophageal reflux, breathing disorders while nutritive sucking, excessive sweating during sleep, prolonged apneas and apneas with associated symptoms, further unexplained episodes of cyanosis, pallor and loss of muscle tone. An acute decompensation of the cardiorespiratory autonomic system during sleep is currently discussed to be one of the causes of SID. Polysomnographic investigations are used to diagnose sleep related …

[Medical treatment of snoring and obstructive sleep apnea syndrome].

Although snoring may have deleterious effects by itself, its only clearly identified hazard is that of a social nuisance; therefore, treatment of snoring should before all make no harm. Conversely, the potential hazards of obstructive sleep apnea are well established, and it is clearly important to eliminiate sleep apneas. The medical treatments which may improve snoring and obstructive sleep apneas are basically the same. They include avoidance of risk factors such as obesity, alcohol and hypnotics as well as active treatments such as positional treatment, dental appliances and by nasal conti…

Chronic lymphatic leukaemia presenting as severe obstructive sleep apnoea.

A case of adenotonsillar enlargement from chronic lymphatic leukaemia leading to severe obstructive sleep apnoea (OSA) is described. Nasal continuous positive airway pressure (nCPAP) therapy was poorly tolerated because even at high pressures (+15 cm H2O) obstructive hypopnoeas persisted. Tonsillectomy resulted in significant improvement, though OSA persisted. nCPAP was reintroduced and at low pressures (+6 cm H2O) was efficacious in alleviating the patient's symptoms. This report highlights the need to consider OSA as a cause of constitutional symptoms in adults with lymphoreticular disease, …

Knowledge gained in sleep medicine over the past 10 years has not yet been incorporated into laws for the severely handicapped and pension law. The impact of increased accident incidence of patients with hypersomnia has not yet been recognized in the guidelines for medical diseases in automobile traffic in governmental driving regulations. In preparation for implementation of sleep-wake disorders into medico-legal jurisdiction they must be introduced to the guidelines as a separate medical entity according to the International Classification of Sleep Disorders.
