INTRODUCTION. We present a series of children who underwent a video-polysomnographic recording at our Sleep and Epilepsy Unit, who received a diagnosis of nocturnal frontal lobe epilepsy (NFLE). AIMS. To describe electroclinical and video polygraphic features of paediatric NFLE that differentiate this condition from other sleep disorders that overlap and mimic the sleep motor and autonomic events of NFLE. PATIENTS AND METHODS. The inclusion criterion was that the patients have their first video-EEG-PSG recording in our laboratory. RESULTS. Twenty-four out of 190 children were diagnosed with NF

Sleep disturbances are common in adolescents. The etiologies include biological, environmental, and sociocultural factors. The structure of sleep changes during adolescence, and it is easy for adolescents to stay awake. Sleep deprivation causes psychiatric symptoms. On the other hand, many psychiatric disorders cause secondary insomnia. The primary care is sleep hygiene counselling and behavioural therapy. If the sleep disorder is secondary, treatment of the primary disorder is the most important help. The scientific data of pharmacological management of insomnia of adolescent is thin. Melaton

BACKGROUND: Difficulty sleeping is common among patients with fibromyalgia (FM); however, its impact on health-related quality of life (HRQoL) is not well understood. The aim of the current study was to assess the burden of sleep difficulty symptoms on HRQoL among patients with FM.
METHODS: The current study included data from the 2009 National Health and Wellness Survey (N=75,000), which is a cross-sectional, Internet-based survey representative of the adult US population. The prevalence of sleep difficulty symptoms among patients with FM (n=2,196) were compared with matched controls (n=2,19

Fatigue is a feature of several chronic diseases of the central and peripheral nervous system. The pathophysiology of central fatigue is complex and often not well-defined. In contrast, peripheral fatigue is more objectively defined and measured. Fatigue can be part of the primary disease process, but there are often contributions from comorbid factors such as depression, sleep disturbance, medication, or deconditioning. Multiple sclerosis (MS) offers an example of central fatigue. More than 40% of MS patients complain of fatigue. Validated questionnaires are used to assess fatigue severity an

OBJECTIVES: To review the literature on sleep disturbances in nursing home settings. Although 50% of older adults complain of sleep difficulties, increased reports of sleep disturbances are generally associated with poor health. After controlling for comorbidities that often contribute to poor sleep, studies have shown that primary sleep disturbances in healthy older adults are, in fact, rare. Although common in older adults, sleep complaints are even more prevalent in elders living in nursing homes and the sleep disturbances experienced by institutionalized older adults are more severe. Somnolence is one of the most common adverse effects of a dopaminergic agonist, rotigotine. We report putative adverse effects experienced by a spouse of a man treated with this compound because of advanced Parkinson disease. We propose the exposure to rotigotine through the seminal fluid because protected sexual intercourse eliminated her postcoital symptoms. This previously unrecognized mechanism may be more common and associated with other psychoactive compounds penetrating the blood-testis barrier, and it may account for otherwise unexplained postcoital somnolence or fatigue.

BACKGROUND: Sleep problems among assisted living facility (ALF) residents are not well understood, and sleep-related differences between ALF residents and home-dwelling older adults have not been examined.
METHODS: We compared sleep patterns in 19 ALF residents to sleep patterns in 19 matched home-dwelling older people (age > or =65 years). All were participating in the follow-up portion of a longitudinal study of sleep and functional outcomes following post-acute rehabilitation. Sleep was assessed with the Pittsburgh Sleep Quality Index and 1 week of wrist actigraphy.
RESULTS: By actigraphy

1. Multiple system atrophy (MSA) is a sporadic neurodegenerative disorder that manifests with parkinsonism, cerebellar ataxia, and autonomic failure in various combinations. 2. Orthostatic hypotension, neurogenic bladder, laryngeal stridor and sleep apnea, and rapid eye movement (REM) sleep behavior disorder are prominent manifestations of MSA. 3. In MSA, there is severe depletion of catecholaminergic neurons of the C1 and A1 areas in the ventrolateral medulla, and this may contribute to orthostatic hypotension and endocrine disturbances in this disorder, respectively. 4. Loss of corticotrophi

This multi-institutional sleep study involved two phases aimed at investigating sleep alterations in patients with any stage of breast and lung cancer. The first phase of this study used an 82-item, 20-minute telephone survey to elicit information regarding the impact of sleep disturbances on a convenience sample of 150 patients. Of these patients, 44% reported a sleep problem during the month before the interview. Significant relations included these: report of sleep problems prediagnosis over the past month (x = 5.82; p = 0.02), duration of sleep medication use and frequency of sleep problem

Fasting motility of the stomach and small intestine and that after carbohydrate, protein and fat meals were studied with tubes furnished with tensometers over 24-50 hours in 7 healthy and 31 duodenal ulcer males. Over the first night 7 healthy and 17 ulcer subjects were subjected to polygraphic sleep registration including electroencephalogram. Sleep quality was assessed by means of biorhythmologic questionnaire. Time co-incidence was registered for the most active phases of 90-min rhythms of the sleep and periodic gastrointestinal activity. In patients with poor sleep the pattern of periodic

A longitudinal evaluation of the degree and type of psychological distress was carried out in 50 geriatric medical in-patients, using the Symptom Distress Checklist. They were tested within three days of admission, after 12-15 days of hospital stay, and 20-30 days after discharge. The first two evaluations showed significantly higher distress than the third for almost all the subscales. Scores indicating a significant depressive distress decreased from 70 per cent (first evaluation) and 76 per cent (second evaluation) to 34 per cent at the third one. The number of patients with significant dis

We present three patients with Treacher Collins or Pierre Robin syndromes who had historical and physical evidence of airway obstruction, difficulty feeding, and sleep disturbances. These preoperative findings correlated with difficult airway management intraoperatively. Based on this experience, we recommend that children with obstructive symptoms have laryngoscopy prior to anaesthetic induction. If the glottic opening is visualized, inhalational induction can proceed. If the glottic structures cannot be visualized, then the anaesthetist must choose between awake oral or nasal intubation, ele

Five patients (3 male, 2 female, mean age 20.4 years, range 14-28) with juvenile neuronal ceroid lipofuscinosis were studied using the Static Charge-Sensitive Bed method. Three distinct patterns of respiration (calm, periodic and restless) were differentiated in an automatic analysis. The amount of restless respiration was found to increase with duration of the disease. The result is discussed in terms of methodology and the clinical picture of JNCL.

Disorders of arousal (somnambulism and night terrors) were significantly more common among children with Gilles de la Tourette's syndrome (n = 57) than among children with seizure disorders (n = 58) or learning disabilities (n = 53). Neurohumoral disturbances may account for the differences.

Intermittent oral or rectal administration of diazepam for the prophylactic treatment of febrile convulsions has given results comparable to the continuous use of phenobarbital while limiting side effects and risks of toxicity. Since we believe that nitrazepam is a better anticonvulsant than diazepam, we performed a study to evaluate the effectiveness of this medication in the prophylactic treatment of febrile convulsions. Nitrazepam was given only when the children had fever and almost exclusively in children with a high risk of recurrence (less than 12 months of age at first convulsion; atyp

The physician-patient relationship plays a major part in the management. The consultations should be extended and fruitful for the patient, but the physician should also provide pharmacodynamic help and to achieve this tiapride is of value. Tiapride is effective against disorders of sleep, of affect and of behaviour. Twenty patients were given 300 mg tiapride daily (1 tablet three times a day) for one month. Results were as follows: in disturbances of sleep: 83,3% good results; in disorders of affect: 79% good results; in behavioural disorders: 77% good results; in alcoholic tremor: 100% good
