Complications associated with repair of a distal biceps rupture using the modified two-incision technique.

Reinsertion of a ruptured distal biceps through a modified 2-incision approach is a well-accepted method of repair. Only 1 large series has focused specifically on the complications associated with this approach, and the authors found no instances of radioulnar synostosis. In a retrospective review of 45 consecutive cases, 12 of 45 patients (27%) experienced 14 postoperative complications, including nerve dysfunction in 7, functional radioulnar synostosis in 3, loss of motion unrelated to heterotopic ossification in 2, early rerupture in 1, and reflex sympathetic dystrophy in 1. The incidence

Chronic rupture of the extensor apparatus of the knee joint.

Chronic ruptures of the extensor mechanism of the knee are uncommon injuries, and previously reported literature assumes the presence of an intact patella for repair. We present a case of chronic rupture following patellectomy done twelve years previously. The defect in the extensor apparatus was extensive (18cm), and this was bridged using a large fascia lata graft from the opposite thigh, with reasonably successful results.

Dynamic sonographic evaluation of peroneal tendon subluxation.

OBJECTIVE: Peroneal tendon subluxation is a major cause of posttraumatic lateral ankle pain. Because peroneal subluxation often occurs only when the foot is dorsiflexed and everted, findings on static imaging studies may appear to be normal. We therefore evaluated the effectiveness of sonography in revealing peroneal tendon subluxation in patients performing stress maneuvers. MATERIALS AND METHODS: From June 2001 to June 2003, 13 consecutive patients (10 females and three males; mean age, 30.4 years; age range, 16-66 years) in whom peroneal tendon subluxation was clinically suspected were sca

Interventions for treating mallet finger injuries.

BACKGROUND: Mallet finger, also called drop or baseball finger, is where the end of a finger cannot be actively straightened out due to injury of the extensor tendon mechanism. Treatment commonly involves splintage of the finger for six or more weeks. Less frequently, surgical fixation is used to correct the deformity. OBJECTIVES: To examine the evidence for the relative effectiveness of different methods of treating mallet finger injuries. SEARCH STRATEGY: We searched the Cochrane Musculoskeletal Injuries Group trials register (November 2003), the Cochrane Central Register of Controlled Tri

[Rupture of the patellar ligament one month after treatment with fluoroquinolone].

Musculoskeletal disorders including arthralgia and myalgia are adverse effect common to all fluoroquinolones. We report a case of spontaneous rupture of the patellar ligament in a 37-year-old man participating in leisure sports which occurred one month after a three-week course of Ciprofloxacine(R). Several cases of tendon ruptures have been reported in the literature, including the patellar tendon. Unusual features in our case were the one-month delay before tendon rupture and the absence of inaugural signs. We reviewed the pathophysiological mechanisms leading to fluoroquinolone-related tend

Incidence and symptoms of clinically manifest rotator cuff lesions.

A clinical study of 200 unselected elderly subjects with an average age of 78 years living in retirement homes was performed to determine the prevalence of clinically manifest rotator cuff tears and the relevance of the tear for daily life. A comprehensive questionnaire was performed and the examination included the UCLA- and Constant-shoulder assessment scales and a multitude of special tests. The criteria for clinical diagnosis of a rotator cuff tear were a positive Jobe-test, diminished external rotation, a positive zero-degree-abduction test, the drop arm sign, pseudoparalysis and atrophy.

[Golden standard: patellar tendon-plasty--technique and management of postoperative complications].

ACL reconstruction with a patellar tendon autograft has reached a high grade of perfection. Surgery can be performed mini-open or arthroscopically, both techniques are presently equal in morbidity and results. The optimal insertion areas for the graft have been defined and can be controlled intra- and postoperatively by radiographs. The bone blocks of the graft allow for stable fixation and an aggressive functional rehabilitation program stressing active full extension of the joint. Disadvantages are a certain donor site morbidity and a rate of restrictions in range-of-motion. The management o

MR imaging of the adult elbow.

Magnetic resonance (MR) imaging can provide important diagnostic information in the evaluation of the adult elbow. Optimal imaging technique should include the use of proper positioning, surface coils, and appropriate sequences and imaging planes as indicated by the suspected abnormalities. A familiarity with the anatomy of the normal elbow is crucial to identifying pathologic entities correctly and avoiding diagnostic pitfalls. Disruption of the collateral ligaments can best be demonstrated with thin-section coronal gradient-echo MR images. Injuries to the flexor and extensor muscle groups, b

Review article: MRI of the knee.

The inherent vulnerability of the knee to injury and degenerative change means that there is a major requirement for an imaging technique capable of assessing such damage accurately, rapidly and non-invasively. Over a relatively short time magnetic resonance imaging (MRI) has emerged as the technique of choice in the assessment of internal derangement of the knee. The present review describes the wide application of MRI to the knee and considers the position of MRI in relation to other diagnostic methods.

Arthroscopic subacromial decompression in the treatment of full-thickness rotator cuff tears.

Arthroscopic subacromial decompression has become an accepted treatment for patients with impingement syndrome; however, its use for full-thickness rotator cuff tears is controversial. We observed 25 patients with full-thickness rotator cuff tears treated by arthroscopic subacromial decompression and cuff debridement alone with a minimum of 1 year follow-up observation. Based on the University of California at Los Angeles shoulder rating, 84% of the cases were rated as excellent or good. There was significant improvement in pain, function, motion, and strength. Eighty-eight percent of the pati

[Treatment of osseous ruptures of the flexor tendons with resorbable materials].

The avulsion fracture of the extensor tendon is first fixed with two PDS-splints to prevent rotation. Thereafter, the fragment is put on compression by a PDS-suture. The treatment is similar to the use of Kirschner wires and a wire tension-band.

A modified midlateral incision for volar approach to the digit.

This article describes an incision which incorporates principles of both the Bunnell midlateral incision and the Bruner zig-zag. It allows rapid access to the volar aspect of the digit without sacrificing exposure or risking flexion contractures while maintaining an unviolated volar skin flap. In this study of 125 patients, 189 digital problems were treated, using the modified midlateral incision. Ages ranged from eleven months to seventy-one years. Complications following surgery were unrelated to the incision.

Correction of abduction. Deformity of the small finger caused by avulsion of the insertion of the third volar interosseous muscle.

Wartenberg described the clinical sign of the abducted extended small finger that can result from ulnar nerve palsy. When this sign occurs in ulnar nerve palsy, it is a result of imbalance created by paralysis of the third volar interosseous muscle allowing the extensor digiti minimi to abduct the small finger as the finger is extended. If the third volar interosseous muscle is traumatically avulsed at its insertion and the ulnar nerve is intact, the abductor digiti minimi manus is an additional deforming force.

Favorable change in patient-reported outcomes following peroneus longus to brevis tendon transfer and lateral ankle ligament reconstruction.

BACKGROUND: A peroneus longus to brevis tendon transfer is recommended for a severely torn peroneus tendon, but there is little research on the outcome. We conducted a prospective cohort study to examine patient-reported outcomes after this procedure. METHODS: Thirty-two patients underwent a peroneus longus to brevis tendon transfer and lateral ankle ligament reconstruction, 11 had an additional calcaneal osteotomy. The Foot and Ankle Outcome Score (FAOS) and Short Form-36 (SF-36) were assessed preoperatively, six and 12 months after surgery. RESULTS: Preoperative mean FAOS was 51.7 (SD 17.8

Biomechanical analysis of latissimus dorsi tendon transfer with and without superior capsule reconstruction using dermal allograft.

BACKGROUND: Irreparable rotator cuff tears (ICTs) remain a challenging treatment dilemma. Superior capsule reconstruction (SCR) acts as a static stabilizer to decrease superior humeral head migration. Latissimus dorsi tendon transfers (LDTs) dynamically decrease superior humeral head migration and improve external rotation. We hypothesized that the dynamic stabilizing effect of the latissimus transfer plus the static stabilizing effect of SCR would improve shoulder kinematics in shoulders with ICTs. MATERIALS AND METHODS: Eight fresh-frozen cadaveric shoulders were tested in 5 conditions: (1)

An Alternative Bundle-to-Bundle Suturing Technique for Repairing Fresh Achilles Tendon Rupture.

The main concern about conventional Achilles tendon repair surgical techniques is how to maintain the initial strength of the ruptured Achilles tendon through complicated suturing methods. The primary surgical problem lies in the properties of the soft tissue; the deterioration of the Achilles tendon, especially in its elasticity; and the surface lubricity of the local tissues. In the present study, we describe an innovative bundle-to-bundle suturing method that addresses these potential problems.

Channel-assisted minimally invasive repair of acute Achilles tendon rupture.

BACKGROUND: Percutaneous (minimally invasive) suturing is a promising option for Achilles tendon (AT) repair with low rerupture and infection rates. Sural nerve lesions are the major problem to avoid with the technique. A new device was therefore designed for suturing the AT, resulting in channel-assisted minimally invasive repair (CAMIR). The purpose of this study was to compare the clinical and functional outcomes of CAMIR with traditional open techniques. METHOD: Eighty two patients with AT rupture were included: 41 for CAMIR, 41 for open repair. All patients followed a standardized rehabi

Does the latissimus dorsi tendon transfer for massive rotator cuff tears remain active postoperatively and restore active external rotation?

HYPOTHESIS: The purpose of this study is to evaluate the muscle activity with surface electromyography (EMG) and the clinical outcome of the latissimus dorsi transfer. It remains unclear whether the clinical results of the latissimus dorsi transfer for massive posterosuperior rotator cuff tears are achieved either by active muscle contractions or by a passive tenodesis effect of the transfer. METHODS: Eight patients were evaluated preoperatively and at 1 year (SD, 0.1) after the latissimus dorsi transfer. Clinical evaluation of outcomes included active range of motion, Constant score, and vis

Outcomes of modified 2-incision technique with use of indomethicin in treatment of distal biceps tendon rupture.

Multiple surgical techniques for distal biceps tendon ruptures exist. Heterotopic ossification is an associated omplication of the 2-incision technique. The purpose of this study was to review the results of distal biceps tendon repairs via the modified 2-incision technique using indomethacin chemoprophylaxis. A retrospective review of 34 2-incision repairs of the distal biceps tendon was performed. All patients received 6 weeks of indomethacin treatment postoperatively to prophylax against heterotopic ossification. Outcome measures included disabilities of the arm, shoulder, and hand (DASH) s
