Factors underlying taking a child to HIV care: implications for reducing loss to follow-up among HIV-infected and -exposed children.

OBJECTIVE: With the aim of reducing pediatric loss to follow-up (LTFU) from HIV clinical care programs in sub-Saharan Africa, we sought to understand the personal and socio-cultural factors associated with the behavior of caregivers taking HIV-infected and -exposed children for care in western Kenya. METHODS: Between May and August, 2010, in-depth interviews were conducted with 26 purposively sampled caregivers caring for HIV-infected (7), HIV-exposed (17) and HIV-unknown status (2) children, documented as LTFU from an urban and rural HIV care clinic. All were women with a majority (77%) bein

Rehabilitation of dynamic visual acuity in patients with unilateral vestibular hypofunction: earlier is better.

PURPOSE: Patients with acute peripheral unilateral hypofunction (UVH) complain of vertigo and dizziness and show posture imbalance and gaze instability. Vestibular rehabilitation therapy (VR) enhances the functional recovery and it has been shown that gaze stabilization exercises improved the dynamic visual acuity (DVA). Whether the effects of VR depend or not on the moment when it is applied remains however unknown, and investigation on how the recovery mechanisms could depend or not on the timing of VR has not yet been tested. METHODS: Our study investigated the recovery of DVA in 28 UVH pa

Medical Attention Seeking After Transient Ischemic Attack and Minor Stroke Before and After the UK Face, Arm, Speech, Time (FAST) Public Education Campaign: Results From the Oxford Vascular Study.

IMPORTANCE: Risk of major stroke is high during the hours and days after transient ischemic attack (TIA) and minor stroke but is substantially reduced by urgent medical treatment. Public education campaigns have improved the response after major stroke, but their association with behavior after TIA and minor stroke is uncertain. The number of potentially preventable early recurrent strokes in patients who delay or fail to seek medical attention is unknown. OBJECTIVE: To investigate the association of public education with delays and failure in seeking medical attention after TIA and minor str

Increments in physical activity relate to reductions in tinnitus severity: a 2-year prospective observational study.

INTRODUCTION: Tinnitus, the perception of sound without external stimuli, affects approximately 14.4% of adults, significantly impacting quality of life. Previous cross-sectional research has shown that individuals who engage in greater levels of physical activity (PA) reported lower tinnitus severity. While these findings are promising, they are limited by the inherent constraints of cross-sectional designs. Hence, the current study investigated whether longitudinal changes in PA are associated with concurrent changes in tinnitus severity. METHODS: In an observational longitudinal study, 2,7

Factors associated with sports-related dental injuries among young athletes: a cross-sectional study in Miyagi prefecture.

BACKGROUND: Sports-related dental injuries, such as tooth fracture, loosening, and avulsion, are a major concern among young athletes because they directly impair oral function. Although the preventive efficacy of mouthguards has been well established, the prevalence of sports-related dental injuries remains high among young athletes. The aim of this study is to identify the variables contributing to the risk of sports-related dental injuries by conducting a survey on large population of young athletes in Miyagi prefecture. METHODS: A cross-sectional study was conducted with school-aged athle

The effect of reward on listening effort as reflected by the pupil dilation response.

Listening to speech in noise can be effortful but when motivated people seem to be more persevering. Previous research showed effects of monetary reward on autonomic responses like cardiovascular reactivity and pupil dilation while participants processed auditory information. The current study examined the effects of monetary reward on the processing of speech in noise and related listening effort as reflected by the pupil dilation response. Twenty-four participants (median age 21 yrs) performed two speech reception threshold (SRT) tasks, one tracking 50% correct (hard) and one tracking 85% co

Neural representation of the self-heard biosonar click in bottlenose dolphins (Tursiops truncatus).

The neural representation of the dolphin broadband biosonar click was investigated by measuring auditory brainstem responses (ABRs) to "self-heard" clicks masked with noise bursts having various high-pass cutoff frequencies. Narrowband ABRs were obtained by sequentially subtracting responses obtained with noise having lower high-pass cutoff frequencies from those obtained with noise having higher cutoff frequencies. For comparison to the biosonar data, ABRs were also measured in a passive listening experiment, where external clicks and masking noise were presented to the dolphins and narrowban

Neural bases of categorization of simple speech and nonspeech sounds.

Categorization is fundamental to our perception and understanding of the environment. However, little is known about the neural bases underlying the categorization of sounds. Using human functional magnetic resonance imaging (fMRI) we compared the brain responses to a category discrimination task with an auditory discrimination task using identical sets of sounds. Our stimuli differed along two dimensions: a speech-nonspeech dimension and a fast-slow temporal dynamics dimension. All stimuli activated regions in the primary and nonprimary auditory cortices in the temporal cortex and in the pari
