Therapy for atherosclerotic occlusive subclavian arterial disease is undergoing a paradigm shift from open to endoluminal therapy. The aim of this study was to review the changing patterns of presentation and clinical outcomes based on presenting symptoms of subclavian artery revascularization. We performed a retrospective analysis of consecutive patients treated for symptomatic atherosclerotic occlusive subclavian arterial disease from 1992 through 2006. Mean follow-up was 4 years. One hundred fourteen patients with a mean age of 63 years (range 33-89, 61% female) underwent 137 procedures. Of

Several minimally invasive approaches, avoiding median sternotomy, have been described within the last few years for cardiac surgery. Femoral arterial and venous cannulation for extracorporeal perfusion are required for many of these operations. The aim of this report is to assess the long-term outcomes of femoral cannulations in patients who underwent minimally invasive procedures. One hundred and sixty patients underwent operations by the port-access method between January 2002 and October 2006. Cardiopulmonary bypass was established by femoral artery-vein cannulation, and a transthoracic cl

BACKGROUND AND PURPOSE: In some people the blood supply to the posterior cerebral artery occurs partly or even exclusively via the carotid system. This anatomic configuration may influence the risk of occipital lobe infarction. We studied the presence and direction of flow in the posterior communicating artery (PCoA) in patients with an occipital lobe infarct and in healthy controls.
METHODS: Forty-seven patients with an occipital lobe infarct were studied by MR angiography, as well as 50 young healthy controls. Special attention was paid to the presence of a PCoA and, if present, to the dire

OBJECTIVE: To investigate the relationship between intracranial arterial dolichoectasia (IADE) and vascular risk factors, atherosclerosis of the carotid arteries as measured by ultrasound scan, and stroke subtypes.
METHODS: The sample consists of 510 consecutively recruited patients with brain infarction confirmed by MRI. The diagnosis of IADE was made by consensus between two neurologists based on MRI results. An independent reading of the 510 scans was made, with measurement of the diameter of the seven main intracranial arteries with a 16-diopter lens to validate the consensus and to bette

PURPOSE: To describe a sharp puncture technique for recanalization of chronic central venous occlusions that could not be traversed by a guide wire.
MATERIALS AND METHODS: Five patients presented with six longstanding central venous occlusions that could not be traversed with a guide wire after thrombolysis. The occlusions occurred following radiation for lung carcinoma (n = 2) and indwelling venous catheters (n = 4). The length of venous occlusion was determined by simultaneously advancing transbrachial and transfemoral catheters to the site of occlusion. Initially, a curved guiding catheter

Three cases of intracranial vertebrobasilar occlusion were successfully treated in the acute stage by thrombolysis and angioplasty. All three patients were admitted to the hospital because of consciousness disturbance and other brainstem signs. Initial angiography revealed intracranial vertebrobasilar occlusions. At first, a microcatheter was introduced into the distal site of the occlusion and thrombolysis was attempted by using urokinase. Recanalization was achieved in all cases but severe stenosis of the intracranial vertebral and basilar arteries was found. The recanalization was followed

BACKGROUND AND PURPOSE: Atherosclerotic disease of the proximal brachiocephalic circulation may produce disabling symptoms referable to cerebral or upper extremity hypoperfusion and embolization. Bypass of occlusive lesions can provide durable relief of symptoms with minimal complications. The ideal conduit for carotid-to-subclavian and subclavian-to-carotid bypass remains controversial, and it is not clear whether the outflow vessel influences patency and survival.
METHODS: We performed a retrospective analysis of 60 consecutive carotid-to-subclavian and subclavian-to-carotid bypass procedur

PURPOSE: The outcome of leg bypass in women is unknown. To date, most series of lower extremity bypass have included few women, and the results in women compared with those in men have not been reported. The experience with infrainguinal arterial reconstruction in women treated at the Oregon Health Sciences University has been greater than previously reported. We have reviewed our recent experience with lower extremity bypass to determine whether results in women differ significantly from those in men.
METHODS: In the past 11 years we have performed 823 infrainguinal arterial reconstructions

BACKGROUND: Atherosclerosis of the iliac artery may result in a stenosis or occlusion, which is defined as iliac artery occlusive disease. A range of surgical and endovascular treatment options are available. Open surgical procedures have excellent patency rates but at the cost of substantial morbidity and mortality. Endovascular treatment has good safety and short-term efficacy with decreased morbidity, complications and costs compared with open surgical procedures. Both percutaneous transluminal angioplasty (PTA) and stenting are commonly used endovascular treatment options for iliac artery

INTRODUCTION: The Subclavian artery aneurysms are not a commonly seen peripheral aneurysm [1-5]-. We present the experience of the Institute of Cardiovascular Diseases of the Serbian Clinical Centre, Belgrade.
PATIENTS AND METHODS: Eight cases of subclavian artery aneurysms are presented. There were 3 male and 5 female patients, average age 51 (32-65) years. Of them 3 aneurysms were of atherosclerotic origin, 4 developed due to thoracic outlet syndrome (TOS), and one developed after intra-arterial drug injection. More details about our cases are presented in Table 1. One of our patients had i

BACKGROUND: Complex abdominal aortic aneurysms (CAAAs) or thoracoabdominal aneurysms (TAAs) often require urgent repair, but custom-made devices are unavailable in such settings. Physician-modified endografts (PMEGs) allow for rapid, patient-specific fenestrated or branched endografts. This study assessed clinical outcomes of CAAA and TAA PMEGs performed at a single Swedish center and compared results using the Cook Zenith and Relay Plus platforms.
METHODS: A retrospective single-center review was conducted on all CAAA and TAA PMEG procedures at Linköping University Hospital between January 2

BACKGROUND: In situ laser fenestration (ISLF) of the left subclavian artery (LSA) is a simple and effective method for left subclavian artery revascularization. However, long-term outcome data for this technique are lacking. This study presents our expanded experience with long-term outcomes of ISLF of the LSA in zone 2 thoracic endovascular aortic repair (TEVAR).
METHODS: A single-center retrospective review of all consecutive patients who underwent zone 2 TEVAR with LSA revascularization by ISLF was performed (2009-2023). This technique was performed through retrograde percutaneous or open
