Severe acute alcoholic hepatitis: an audit of medical treatment.

Despite advances in treatment, severe alcoholic hepatitis is still associated with a high mortality rate of 30% to 40%. Nutritional support and steroids in selected patients are believed to improve prognosis. In controlled trials steroids have been beneficial in patients with a discriminant function (DF) value >32 or spontaneous hepatic encephalopathy. The aim of this study was to investigate current practice and outcomes in the treatment of acute alcoholic hepatitis. We retrospectively studied patients admitted to our unit with acute alcoholic hepatitis over a 4 year period. Forty-three patie

Lymphocyte-mediated liver injury in alcohol-related hepatitis.

The pathogenesis of alcohol-related liver disease (ALD) remains inadequately explained. Increasing alcohol intake is associated with an increased risk of ALD, but many heavy drinkers develop no liver damage. An explanation for ALD susceptibility requires theories that extend beyond a biochemical understanding of alcohol metabolism. Several hepatic cell populations are involved in the pathogenesis of liver injury. The liver-associated lymphocyte (LAL) response to alcohol intake plus immune stimulation may determine susceptibility to liver damage. We have isolated rat LALs and demonstrated the f

The effect of chronic alcohol abuse on gastric and duodenal mucosa.

Alcohol consumed in small quantities is not dangerous for health but if it is drunk in big amounts it has a negative effect on somatic and psychical health. A number of studies have been published on the harmful effect of ethanol on the nervous system, circulation, endocrine and immune systems. Ethyl alcohol can directly damage the mucosa of the alimentary tract. The studies conducted and published so far have estimated acute ethanol damage of gastric mucosa in experimental animals. The views found in literature on the effect of continuous consumption of ethanol on the stomach are differentiat

Alcohol abuse in social phobic patients: is there a bipolar connection?

BACKGROUND: Epidemiological and clinical studies have reported the frequent co-occurrence of social phobia (SP) and alcohol use disorders. Patients with SP often use alcohol to cope with the social situations they fear, and to lessen anticipatory anxiety, behavioral inhibition, and phobic avoidance. We investigated whether the presence of lifetime comorbidity with alcohol abuse was associated with significant differences as regards demographic and clinical features, family history and pattern of comorbidity in a large clinical sample of SP outpatients. METHOD: The sample comprised 153 outpati

Hormone responses to citalopram in abstinent alcohol dependent subjects.

BACKGROUND: The integrity of serotonin neurotransmission may be important in reducing risk for alcoholism and in preventing relapse to alcohol dependence. There are several lines of evidence suggesting that alcohol dependent persons have an altered and/or injured serotonin system. The purpose of this study was to examine ACTH, cortisol, and prolactin responses to the selective serotonin reuptake inhibitor (SSRI), citalopram, as a function of personal or family history of alcohol dependence in a group of abstinent alcohol dependent men. METHODS: Twelve healthy, abstinent male participants who

The use of divalproex in alcohol relapse prevention: a pilot study.

Anticonvulsant agents show promise in the treatment of the acute symptoms of alcohol withdrawal and may also treat some symptoms associated with the protracted abstinence syndrome. Impulsivity, hostility and irritability are common characteristics of alcohol-dependent individuals, and there is some evidence that anticonvulsant agents decrease these traits in individuals with a number of different psychiatric disorders. This pilot study is a 12-week, double-blind, placebo-controlled trial of an anticonvulsant agent, divalproex (DVPX), in alcohol-dependent individuals. Alcohol use (Timeline Foll

Moderate alcohol intake in pregnancy and the risk of spontaneous abortion.

Women attending routine antenatal care at Aarhus University Hospital, Denmark, from 1989 to 1996 were eligible for this cohort study on the association between alcohol intake in pregnancy and the risk of spontaneous abortion. We included 24 679 singleton pregnancies in the study. We used Cox regression analyses with delayed entry for the analyses. We found an increased risk of spontaneous abortion in the first trimester (7-11 completed weeks of gestation) for women consuming > or =5 drinks/week. We found no association between alcohol intake and spontaneous abortion during the second trimester

Alcohol-water as a novel medium for beta-hematin preparation.

It is shown that alcohol-water mixtures (e.g., 43 wt% ethanol) provide a suitable medium for the efficient production in high yield of beta-hematin from ferriprotoporphyrin IX (FP), as measured by infrared spectroscopy. Previous and present light-absorption data of FP, obtained under specific acid conditions in an ethanol-water medium, suggest the presence of FP monomers, which are considered to promote the reaction leading to beta-hematin. Other aspects of the mechanism of reaction are discussed.

Liver development in a rat model of fetal alcohol syndrome.

The inhibitory effects of alcohol on hepatic growth in adults raises the possibility that the liver may be involved in fetal alcohol syndrome (FAS) in infants. To test this hypothesis, pregnant Sprague-Dawley rats were fed liquid diets containing either ethanol as 36% of the total calories, or were allowed ad libitum feeding of a control liquid diet (controls) throughout pregnancy. Other dams were exposed to the ethanol diet only during the first or last half of pregnancy. Pups delivered of dams exposed to the various diets (N = 40-45/group) were killed at 1, 3, 7, and 14 days of age. In addit

The effects of alcohol and illicit drugs on the human embryo and fetus.

Women at childbearing age often use alcohol and various illicit drugs such as cocaine and heroin. These agents pass through the human placenta and may affect the developing embryo and fetus. Indeed, large amounts of alcohol ingested by the pregnant woman may produce a specific syndrome manifested by prenatal and postnatal growth retardation, a variety of facial dysmorphic features and mental retardation. Ingestion of smaller amounts of alcohol will produce the fetal alcohol effects with only few and minor dysmorphic features but with developmental delay and some degree of intellectual impairme

Alcohol and urinary 2-thiothiazolidine-4-carboxylic acid.

The aim of this work was to study the effect of alcohol on the excretion characteristics of urinary 2-thiothiazolidine-4-carboxylic acid (TTCA) in people exposed to carbon disulfide (CS(2)) and in non-exposed subjects. Ten male non-exposed volunteers consumed 150, or 250 ml, of liquor (38 degrees ) and the excretion dynamics of urinary TTCA were detected by HPLC. A questionnaire and urinary TTCA analyses were conducted in 152 male workers exposed to CS(2) and 60 male controls. CS(2) concentrations at the workplace were measured by GC. Results showed that urinary TTCA levels in non-exposed volu

Hepatocyte apoptosis is a pathologic feature of human alcoholic hepatitis.

BACKGROUND/AIMS: The pathogenesis of alcoholic hepatitis (AH) remains poorly understood. Although apoptosis is now recognized as a mechanism of liver injury, the extent and mechanisms of apoptosis in human AH remain unknown. Thus, our aims were to quantify hepatocyte apoptosis in patients with AH, correlate it with disease severity, and identify the mechanisms of apoptosis induction. METHODS: Hepatocyte apoptosis was assessed in 26 patients with AH and 27 controls without liver disease using the TUNEL assay and immunohistochemistry for activated caspase 3. Liver specimens were also graded for

Clinical and biological relevance of hepatocyte apoptosis in alcoholic hepatitis.

BACKGROUND/AIMS: Although human and experimental studies have shown that apoptosis plays a role in hepatocyte death in alcoholic liver disease, its clinical and biological significance has not been investigated in alcoholic hepatitis (AH). The aim of this study was to quantify hepatocyte apoptosis in AH and to attempt to relate it to the clinical and biological severity of the disease. METHODS: The hepatocyte apoptotic index was determined using a double in situ transferase-mediated dUTP nick end (TUNEL) and CD15 (neutrophils) labelling on 35 liver biopsies from patients with AH lesions of di

Fetal and infantile alcohol-mediated associative learning in the rat.

BACKGROUND: Infant rats express conditioned responses to an odor experienced prenatally as a chemosensory cue associated with moderate alcohol intoxication. This study examined postnatal intake of a chemosensory cue (cineole) that had been paired with alcohol's unconditioned effects. It also tested the interaction between prenatal association and postnatal conditioning with cineole and alcohol. METHODS: Pregnant female rats intubated with cineole were given ethanol (EtOH).25 or 4.0 hr later. Other groups received only water or water paired with ethanol. During postnatal day 15 (PD15), infant

Treatment of alcohol-dependent outpatients with acamprosate: a clinical review.

Acamprosate (calcium acetyl-homotaurine) is a synthetic compound whose chemical structure resembles that of homotaurine, a naturally occurring amino acid. Acamprosate acts centrally and appears to restore the normal activity of glutaminergic neurons, which become hyperexcited as a result of chronic alcohol exposure. Although not yet approved for use in the United States, acamprosate has been available by prescription in France since 1989 and is now available in many other countries throughout the world. This article reviews data from all published double-blind, placebo-controlled clinical tria
