The purpose of this study was to investigate the value of ultrasonographic examination in the diagnosis of Tietze's syndrome and assessment of the changes in costal cartilage following local steroid injection. Intrathecal administration of bupivacaine and clonidine results in a significant prolongation of both motor and sensitive blocks but side effects-hypotension and bradycardia-are observed. We compared two groups of patients allocated randomly. One group received intrathecal bupivacaine-morphine and the other one received clonidine-bupivacaine-morphine. A sensitive analytical procedure for bupivacaine dosing in plasma samples by reversed-phase high-performance liquid chromatography is described. After a two-step extraction, the analysis was performed using a C18 column and a mobile phase of 0.01 M sodium dihydrogen-phosphate (pH 2.1)-acetonitrile (80:20, v/v). The extraction yield of bupivacaine from plasma was 73.5 +/- 5.1% (mean +/- S.D., n = 10). Fifty consecutive patients starting treatment for sciatica with epidural injections of steroid and local anaesthetic underwent concomitant epidurography. The prognostic potential of this combination was investigated. Epidurography was confirmed as a safe out-patient procedure. Incorrect needle siting accounted for five total and two partial failures. We studied the influence of various extracellular Ca++ and Mg++ concentrations on human natural killer (NK) cells and found that NK cell-mediated target cell lysis requires the presence of both divalent cations. The calcium channel blocker verapamil and the local anaesthetics procaine and lidocaine inhibited NK cell-mediated killing of K 562 target cells, whereas the selective K+ channel blocker tetraethylammonium and the selective Na+ channel blocker tetrodotoxin had no effect on NK activity. Local anaesthetics and antiarrhythmic agents produced changes in responses of rabbit platelets to ADP and thrombin that varied with the agent and its concentration.
