Effects of rapid atrial pacing on the arrhythmogenic activity of single cardiomyocytes from pulmonary veins: implication in initiation of atrial fibrillation.

BACKGROUND: Pulmonary veins (PVs) are important sources of paroxysmal atrial fibrillation. Long-term rapid atrial pacing (RAP) changes atrial electrophysiology and facilitates the maintenance of atrial fibrillation. It is not clear whether RAP alters the arrhythmogenic activity of PVs. The purpose of this study was to isolate single PV cardiomyocytes from control and RAP dogs and evaluate their electrophysiological characteristics. METHODS AND RESULTS: The action potential and ionic currents were investigated in PV cardiomyocytes from control and long-term (6 to 8 weeks) RAP (780 bpm) dogs by…

Intravenous MgSO4 alone and in combination with glucose, insulin and potassium (GIK) prolong the atrial cycle length in chronic atrial fibrillation.

AIMS: To investigate the effects of parenteral administration of MgSO4, and glucose, insulin, and potassium (GIK), on the dominant atrial cycle length during chronic atrial fibrillation (CAF). METHODS AND RESULTS: The length of the dominant atrial cycle (DACL) in the power-frequency spectrum of the QRST-suppressed lead V1 ECG was identified before and after intravenous administration of MgSO4 alone and after 5 and 10 h of MgSO4 and GIK infusion, in 13 patients with CAF. The changes in DACL were compared with changes in heart rate (HR), blood pressure and blood parameters. MgSO4 alone increase…

Hemodynamic abnormalities in the left atrial appendage in patients with paroxysmal atrial fibrillation, with special reference to albumin-contrast echocardiographic aspects.

AIM: We assessed the prolonged dysfunction of the left atrial appendage caused by paroxysmal atrial fibrillation. METHODS AND RESULTS: Transesophageal echocardiography with intravenous albumin-microspheres (Albunex, 0.2 ml/kg) was performed in 100 consecutive patients (44 patients in sinus rhythm without previous paroxysmal atrial fibrillation: 13 patients in sinus rhythm who had had previous episodes of paroxysmal atrial fibrillation; and 43 patients with sustained atrial fibrillation). We compared the left atrial appendage ejection fraction and degree of opacification in the left atrial app…

Left atrial appendage thrombus is not uncommon in patients with acute atrial fibrillation and a recent embolic event: a transesophageal echocardiographic study.

OBJECTIVES: The objective of this study was to determine the frequency of left atrial thrombus in patients with acute atrial fibrillation. BACKGROUND: It is commonly assumed but unproved that left atrial thrombus in patients with atrial fibrillation begins to form after the onset of atrial fibrillation and that it requires > or = 3 days to form. Thus, patients with acute atrial fibrillation (i.e., < 3 days) frequently undergo cardioversion without anticoagulation prophylaxis. METHODS: Three hundred seventeen patients (250 men, 67 women; mean [+/- SD] age 64 +/- 12 years) with acute (n = 143)…

Efficacy and safety of flecainide acetate in the maintenance of sinus rhythm after electrical cardioversion of chronic atrial fibrillation or atrial flutter.

The efficacy and safety of flecainide were studied in the maintenance of sinus rhythm after electrical cardioversion for chronic atrial fibrillation or atrial flutter. Eighty-one patients were randomized to flecainide treatment or no treatment. Baseline characteristics of both groups were comparable. Compared to previous studies, patients could be classified as difficult-to-treat patients. Multiple regression analysis showed New York Heart Association class I for exercise tolerance (p = 0.0004) and flecainide treatment (p = 0.01) to be the main factors increasing the arrhythmia-free episode. H…

Concomitant cryoballoon pulmonary vein and posterior wall isolation in persistent atrial fibrillation: Results from the Left Atrial Posterior Wall Isolation in Conjunction With Pulmonary Vein Isolation Using the Cryoballoon for Treatment of Persistent Atrial Fibrillation (PIVoTAL) IDE trial.

BACKGROUND: Pulmonary vein isolation (PVI) remains the cornerstone of atrial fibrillation (AF) ablation, but its efficacy in patients with persistent AF (PersAF) remains suboptimal compared with those with paroxysmal AF. OBJECTIVE: This study investigated the outcomes of cryoballoon PVI with concomitant "direct" posterior wall isolation (PWI) vs PVI alone in patients with symptomatic PersAF. METHODS: The PIVoTAL IDE was a prospective, multicenter trial (NCT04505163) that randomized patients with symptomatic, drug-refractory PersAF, refractory/intolerant to ≥1 class I-IV antiarrhythmic drug, …

Anxiety sensitivity moderates prognostic importance of rhythm-control versus rate-control strategies in patients with atrial fibrillation and congestive heart failure: insights from the Atrial Fibrillation and Congestive Heart Failure Trial.

BACKGROUND: Patients with high anxiety sensitivity (AS) become extremely anxious with heart rate increases, palpitations, and symptoms of psychological arousal. AS predicts panic attacks. In atrial fibrillation (AF), AS correlates with symptom preoccupation and reduced quality of life. We assessed whether AS is associated with outcomes of rhythm-control versus rate-control in congestive heart failure (CHF) patients with AF. METHODS AND RESULTS: Before random assignment, 933 participants (172 women) in the Atrial Fibrillation and Congestive Heart Failure Trial completed the Anxiety Sensitivity…

Investigating Factors Associated with the Occurrence of Atrial Fibrillation after Non-cardiothoracic Surgery.

BACKGROUND: Although the occurrence of atrial fibrillation (AF) after cardiac and thoracic surgery is common and leads to serious complications such as patients' vital organ dysfunction, pulmonary edema, myocardial infarction, and death, evidence about the occurrence and risk factors associated with post-operative atrial fibrillation (POAF) as new AF immediately after non-cardiac thoracic surgery is limited. Therefore, the present study aimed at investigating the factors related to the occurrence of POAF after non-cardiothoracic surgery. MATERIALS AND METHODS: The present retrospective study …

Lactobacillus shapes LPS-reservoir modules within the gut microbiota to mitigate atrial fibrillation.

Atrial fibrillation (AF) is the most prevalent clinical arrhythmia, associated with numerous complications, elevated morbidity and mortality rates, and limited therapeutic interventions, which have been confirmed to be related to the intestinal microbiota. However, there is currently limited research on the interaction between gut microbiota and AF. Herein, we integrated the largest Chinese gut microbiota cohort (n = 355) related to AF to date and identified a marked microbial imbalance in AF patients, characterized primarily by a significant reduction in Lactobacillus abundance. Using a rat m…

Pulsed Field Ablation for Atrial Fibrillation in Obesity: Reassessing Risk, Safety, and Success.

Background: Obesity is a major independent risk factor for atrial fibrillation (AF), contributing to adverse outcomes and complicating rhythm control. While lifestyle modification is recommended, catheter ablation remains central to management. Conventional thermal ablation techniques often yield suboptimal results in obese patients due to anatomical and biophysical challenges. Pulsed field ablation (PFA), a novel non-thermal modality, may overcome these limitations. Methods: A systematic search of PubMed, Scopus, and Embase identified observational cohort studies (2017-2026) evaluating PFA ou…
