OBJECTIVE: The American Academy of Pediatrics (AAP) recommends behavioral therapy as first-line evidence-based treatment for young children with ADHD, and in combination with medications for children ages 6 years and older. However, a significant percentage of young children are receiving medication treatment for ADHD rather than behavioral treatment. There is limited research capturing parent experiences of barriers to implementing behavioral health services for young children with ADHD diagnoses. This paper seeks to fill this gap by exploring these perceived barriers among parents of childre

BACKGROUND: Optimising the dosage of pharmacological treatments for ADHD is key to maximising their benefits, yet most clinical guidelines provide only limited information on this issue. Dose-effect relationships have not been comprehensively assessed across all ADHD medications and age groups, despite growing concerns about subtherapeutic prescribing. We aimed to estimate dose-effect curves for efficacy and tolerability of ADHD medications (stimulants and non-stimulants) across age groups.
METHODS: We conducted a systematic review and dose-effect network meta-analysis of double-blind randomi

This study examined the long-term effects (4-8 years) of Cognitive Behavioural Therapy (CBT), added to pharmacotherapy and psychoeducation (PE), on quality of life (QoL) in adults with Attention-Deficit/Hyperactivity Disorder (ADHD). Although previous findings showed no added QoL benefit of CBT immediately post-treatment, CBT provides skill-building and coping strategies that may yield delayed benefits in terms of ADHD-related functional impairments. We hypothesize that CBT will provide delayed treatment gains and thus results in higher QoL at follow-up or a slower decline in QoL over time. A

Astrocyte-derived extracellular matrix (ECM) proteins are critical for synaptic development and glutamatergic signaling, yet their role in attention deficit hyperactivity disorder (ADHD) remains poorly understood. This study examined serum levels of Thrombospondin-1 (Tsp-1), Sparc, and Sparcl-1 in children with ADHD and explored their associations with behavioral symptoms, executive dysfunction, and social impairments. Eighty-six children aged 6-12 years participated: 46 medication-naive with ADHD and 40 age- and sex-matched neurotypical controls. Clinical measures included the Conners' Parent

BACKGROUND: Previous studies have shown that abnormalities in the oxytocin system in children with attention deficit hyperactivity disorder (ADHD) may affect bone mineral density, and that medications used to treat ADHD may cause growth suppression. This study aimed to evaluate bone structure and microarchitecture in children diagnosed with ADHD using mandibular indices and fractal dimension analysis as well as to compare these results with those of healthy children. METHODS: The study included 61 children aged 4–15 years who had been diagnosed with ADHD, along with 122 healthy children matche

BACKGROUND: Cognitive disengagement syndrome (CDS) is an emerging cognitive-attentional construct characterized by excessive daydreaming, mental slowness, drowsiness, and reduced engagement with external demands. Although CDS symptoms are substantially associated with attention-deficit/hyperactivity disorder (ADHD) symptoms, accumulating psychometric and external-validity evidence supports their conceptualization as related yet distinguishable symptom dimensions. However, large-scale evidence regarding the distribution and demographic correlates of elevated CDS symptoms, as well as their diffe

Attention deficit-/hyperactivity disorder (ADHD) medications are commonly prescribed to children and adolescents, but therapeutic reference ranges have not been systematically evaluated yet. This study aimed to establish preliminary therapeutic reference ranges for methylphenidate (MPH), d-amphetamine (d-AMP), atomoxetine (ATX), and guanfacine (GFC), based on a systematic review of the existing relevant literature.Therapeutic reference ranges were calculated based on blood concentrations measured in responder children and adolescents with ADHD. Therapeutic ranges were determined both as mean v

Altered cortisol regulation is implicated in Attention-Deficit/Hyperactivity Disorder (ADHD), but causality remains debated. While meta-analyses suggest that lower basal cortisol levels, especially in the morning, correlate with ADHD, study heterogeneity prompts further inquiry. Leveraging post-genome-wide association approaches, we examined morning cortisol levels (n = 25,314) and ADHD (n = 225,543). We employed seven Mendelian Randomization methods for causal inference, assessed global and local genetic correlations, and applied the conjFDR method. Additionally, we conducted polygenic score

Non-invasive brain stimulation is being explored as a potential method for enhancing cognitive function and reducing symptoms in individuals with Attention-Deficit/Hyperactivity Disorder (ADHD), the most common neurodevelopmental disorder in children. Time perception, the ability to estimate and process time intervals, is often impaired in individuals with ADHD and is crucial for daily tasks like planning, decision-making, task completion, etc., Here we examined the effect of Transcranial Random Noise Stimulation (tRNS) on time perception in ADHD individuals. A total of 40 participants, includ

Attention-deficit/hyperactivity disorder (ADHD) has been associated with suboptimal functioning of both the prefrontal cortex and the striatum. These abnormalities may impede the acquisition of perceptual categories, important for fundamental abilities such as object recognition and speech perception. While prior research has shown that children with ADHD perform comparably to neurotypical peers in visual category learning despite using suboptimal strategies, much remains unknown about how adults with ADHD acquire perceptual categories, where more mature functions may shape learning processes

Environmental enrichment (EE) has emerged as a promising intervention capable of modulating behavior. The isogenic rat strains Spontaneously Hypertensive Rats (SHR) and SHR.LEW-(D4Rat76-D4Mgh11) (SLA16) share an identical genome, differing only in a specific region on chromosome 4. Despite this genetic similarity, these two strains exhibit notable behavioral differences, suggesting a potential influence of environmental and genetic factors. This study aimed to evaluate the effects of continuous EE (40 days, 8 different layouts) during adolescence on the behavior of SHR and SLA16 rat strains. A

CX717, a low-impact ampakine, is a positive allosteric modulator of AMPA-glutamate receptors. This clinical study evaluated CX717 for the treatment of ADHD symptomology in a randomized, double-blind, placebo-controlled, multi-center, 2-period crossover with two doses of CX717 (200 or 800 mg BID). Each treatment period lasted 3 weeks with a 2-week washout. Subjects met DSM-IV criteria for adult ADHD and had moderate to severe symptoms. The primary efficacy measure was the change from baseline on the ADHD Rating Scale (ADHD-RS) with prompts. 68 male subjects, aged 18-50 years old were randomized

BACKGROUND: This article presents a bioethical analysis of the use of Methylphenidate and Atomoxetine, exploring their roles as cognitive enhancers and therapeutic agents for Attention Deficit Hyperactivity Disorder (ADHD).
METHODS: The analysis centers around the principle of non-maleficence, examining the ethical implications of causing harm in the pursuit of cognitive enhancement and therapeutic benefits. It delves into the blurred boundaries between therapy and enhancement and the challenges of defining "necessary harm" in these contexts.
RESULTS: When used for cognitive enhancement rath

BACKGROUND AND OBJECTIVE: Attention-Deficit/Hyperactivity Disorder (ADHD) is a common neurodevelopmental condition characterized by inattention, hyperactivity, and impulsivity. Emerging evidence suggests that ADHD is linked to hypofunction of the prefrontal-parietal attention network, accompanied by compensatory hyperactivation in the cerebellum and brainstem. However, the underlying neural mechanisms remain insufficiently understood. In recent years, computerized cognitive training has gained attention as a promising non-pharmacological intervention for alleviating ADHD symptoms, though its m

We performed a genome-wide association meta-analysis (GWAMA) of 290,134 attention-deficit/hyperactivity disorder (ADHD) symptom measures of 70,953 unique individuals from multiple raters, ages and instruments (ADHDSYMP). Next, we meta-analyzed the results with a study of ADHD diagnosis (ADHDOVERALL). ADHDSYMP returned no genome-wide significant variants. We show that the combined ADHDOVERALL GWAMA identified 39 independent loci, of which 17 were new. Using a recently developed gene-mapping method, Fine-mapped Locus Assessment Model of Effector genes, we identified 22 potential ADHD effector ge

BACKGROUND: Attention-deficit/hyperactivity disorder (ADHD) is a neurodevelopmental condition associated with excessive levels of inattention and/or hyperactivity/impulsivity. Symptoms usually persist into adulthood and negatively influence functioning and quality of life. Although sex differences in ADHD are commonly reported, ADHD remains under-recognised in females (including women and people assigned female at birth), and there is limited knowledge about experiences of females with ADHD across hormonal life phases. This study aimed to investigate self-reported ADHD and related traits in fe

Reward-based learning and decision-making are prime candidates to understand symptoms of attention deficit hyperactivity disorder (ADHD). However, only limited evidence is available regarding the neurocomputational underpinnings of the alterations seen in ADHD. This concerns flexible behavioral adaption in dynamically changing environments, which is challenging for individuals with ADHD. One previous study points to elevated choice switching in adolescent ADHD, which was accompanied by disrupted learning signals in medial prefrontal cortex. Here, we investigated young adults with ADHD (n = 17)

Individuals with ADHD struggle with time perception. The ventromedial prefrontal cortex (vmPFC) and dorsolateral prefrontal cortex (dlPFC) are two distinct cortical areas that are involved in the psychopathology of ADHD, including time perception. In the present study, we aimed to explore if modulation of the excitability of these areas with non-invasive brain stimulation alters time perception in ADHD. Twenty-six children with ADHD performed time discrimination and time estimation/reproduction tasks in five sessions with one week interval and specific tDCS conditions. The tDCS conditions were
