Spatial mapping of humeral head bone density.

BACKGROUND: Short-stem humeral replacements achieve fixation by anchoring to the metaphyseal trabecular bone. Fixing the implant in high-density bone can provide strong fixation and reduce the risk of loosening. However, there is a lack of data mapping the bone density distribution in the proximal humerus. The aim of the study was to investigate the bone density in proximal humerus. METHODS: Eight computed tomography scans of healthy cadaveric humeri were used to map bone density distribution in the humeral head. The proximal humeral head was divided into 12 slices parallel to the humeral ana

Relationship between stair ascent gait speed, bone density and gait characteristics of postmenopausal women.

Stair ascent is a biomechanically challenging task for older women. Bone health may affect gait stability during stair walking. This study investigated the gait biomechanics associated with stair ascent in a group of postmenopausal women in relation to walking speed and bone health, quantified by T-score. Forty-five healthy women (mean (SD) age: 67 (14) years), with bone density ranging from healthy to osteoporotic (T-score range +1 to -3), ascended a custom-made five-step staircase with two embedded force plates, surrounded by 10 motion capture cameras, at their self-selected speed. Multivari

Force transmission capacity of the lower limb during walking of amputees with bone-anchored prostheses compared with socket prostheses and persons with hip replacements.

BACKGROUND: Restoring the ability to walk with a prosthesis is considered a fundamental rehabilitation goal after transfemoral amputation. An essential prerequisite for achieving this goal is adequate force transmission between the prosthesis and the body. Does bone anchorage of an artificial limb permit a more normal force transmission? METHODS: Data of 15 healthy subjects, nine amputees fitted with bone-anchored prostheses, nine amputees using socket prostheses, and 18 patients with a total hip replacement were included in this multicenter, observational study. Ground reaction force was mea

The spectrum of gnathic osteosarcoma: caveats for the clinician and the pathologist.

Seven expansile jaw lesions in patients ranging from 7 to 63 years are presented to illustrate diagnostic and management issues pertaining to cases ultimately proven to be gnathic osteosarcoma (GO). Six of the cases in our series were low-grade osteoblastic and one high-grade chondroblastic. None of our cases exhibited the characteristic "sunburst" radiopaque appearance described for GO. All of our cases displayed cortical expansion and one showed development of diastema. Two occurred in the maxilla and five in the mandible. Two of the patients with mandibular lesions presented initially with

MR imaging of marrow changes adjacent to end plates in degenerative lumbar disk disease.

MR studies of the lumbar spine in 41 patients were analyzed at 203 disk interspaces to assess the appearance and frequency of bone marrow signal changes in the vertebral bodies adjacent to normal and degenerated disks. Degenerative changes were found at 58 interspaces; an abnormal bone marrow signal was identified in 29 (50%) of these. On spin-echo pulse sequences with short and long repetition times (TRs) and echo times (TEs), an area of relative increased signal intensity was seen in the vertebral body adjacent to the disk in 24 cases (17 were bandlike on both sides of the disk, four were fo

Time to Bone Union after Hybrid Closed-Wedge High Tibial Osteotomy.

Medial open- and lateral closed-wedge high tibial osteotomy (hybrid CWHTO) can overcome the limitations of conventional CWHTO and open-wedge HTO (OWHTO) for medial compartmental osteoarthritis (OA) of the knee. Hybrid CWHTO increases stability by using a rigid locking plate and allows early full weight-bearing. However, the literature contains no information about time to bone union after this new procedure. The aim of this study is to evaluate the time to bone union after hybrid CWHTO. We reviewed 44 knees treated with hybrid CWHTO. Patients were able to stand on both legs on the day after su

Relationship between articular cartilage damage and bone density in the first metatarsal.

It is well known that bone changes occur in relationship to the articular cartilage damage seen in osteoarthritis. Numerous studies have shown a relationship between bone density and articular cartilage damage in knee and hip joints, but it is controversial as to whether or not an increased subchondral bone density results in greater cartilage damage. We studied 49 pairs of cadaveric first metatarsals in an attempt to correlate the level of cartilage damage seen on the distal articular surface with areal and volumetric density and other parameters such as bone geometry. A positive correlation

Investigating Contact Models in Clavicle Fixation Plates: A Finite Element Study of Their Impact on Biomechanical Performance.

Clavicle fractures are among the most common orthopedic injuries and frequently require plate fixation to restore anatomical alignment and mechanical stability. The biomechanical performance of fixation constructs is strongly influenced by the contact interactions between the plate, bone, and screws. This study investigated the effect of four contact formulations (bonded, no-separation, frictionless, and frictional) on the structural stiffness, stress distribution, and interfragmentary strain of clavicle fixation plates using finite element analysis. A three-dimensional (3D) clavicle model rec

How the Human Cochlea Moves: Biomechanical Modeling of a Wide, Layered Osseous Spiral Lamina.

PURPOSE: The human cochlear partition (CP) at the high-frequency region features a radially wide, layered osseous spiral lamina (OSL) and a soft-tissue bridge connecting it to the basilar membrane (BM). The OSL consists of two thin bony plates separated by a cavernous space that serves as a conduit for auditory nerve fibers. We used a finite element model with two fluid chambers, incorporating novel implementations of the CP features, to study the human cochlea. Model results were compared with experimental measurements of CP motion. METHODS: Model geometrical and material properties either c

Disrupted type II collagenolysis impairs angiogenesis, delays endochondral ossification and initiates aberrant ossification in mouse limbs.

Cartilage remodelling and chondrocyte differentiation are tightly linked to angiogenesis during bone development and endochondral ossification. To investigate whether collagenase-mediated cleavage of the major cartilage collagen (collagen II) plays a role in this process, we generated a knockin mouse in which the mandatory collagenase cleavage site at PQG775↓776LAG, was mutated to PPG775↓776MPG (Col2a1Bailey). This approach blocked collagen II cleavage, and the production of putative collagen II matrikines derived from this site, without modifying matrix metalloproteinase expression or activit
