Hypothermia for hypoxic-ischemic brain injury.

Hypoxic-ischemic brain injury and hypoxic-ischemic encephalopathy (HIE) remain a serious problem for both preterm and term neonates with the spectrum of injury ranging from neuronal injury to encephalopathy and death. Neonatal encephalopathy due to such injury occurs in 3-9 of every 1000 term infants. Of these, it is estimated that nearly a third to a half will either have severe adverse outcomes or die. Treatment of infants with HIE remains generally supportive with attention to resuscitation, fluid and electrolyte homeostasis, maintenance of acid-base balance, nutrition and feeding issues an

[Pathogenesis of the lethal postoperative cerebral inflammatory-purulent complications in neurooncological patients].

The main and immediate causes of death, pathogenetic mechanisms of postoperative cerebral inflammatory-purulent complications (CIPC) in patients, suffering the brain tumour, were analyzed. The main pathogenetic patterns of the postoperative period course in the CIPC occurrence were studied, basing on the data of analysis of 30 patients, who died after the operation. There was proved, that the sepsis occurrence on the background of postoperative meningoencephalitis means the lethal ending of the disease. There was established, that in occurrence of septic complications, connected with postopera

Acute injury to the immature brain with hypoxia with or without hypoperfusion.

This article reviews the imaging features and evolution of immature brain injury caused by hypoxia with or without hypoperfusion in the neonate and young child. Clinical presentations and available literature on mechanisms and clinical outcomes are discussed. In many of these cases, diffusion-weighted imaging does not show the full extent of the injury but detects a pattern of injury that is important in guiding clinical care. Awareness of the delayed cell death mechanisms is essential to understand diffusion-weighted imaging sensitivity and evolution and to provide the most accurate clinical

Late sequelae of major trauma in children.

Traumatic injuries represent the most important threat to the health of children in the United States and are the leading cause of death after the first year of life. More than 20,000 children and young adults will die this year as a result of injury. For every child that dies, another 40 will require hospitalization and 1000 more will be evaluated in emergency departments. Moreover, 50,000 children and adolescents will sustain some degree of permanent disability, the majority of victims of brain injury. This article focuses on several important long-term implications for children, their famil

The mitochondrial DNA A8344G mutation in Leigh syndrome revealed by analysis in paraffin-embedded sections: revisiting the past.

In 1975, we presented the results of a study on a family with a constellation of features that included a chronic spinocerebellar syndrome, neuropathologically proven Leigh syndrome, and sudden death in infancy or childhood affecting several members over three generations. Inheritance was thought to be autosomal dominant. Twenty years later, we reinterpreted the inheritance pattern as maternal. Mitochondrial DNA (mtDNA) extracted from paraffin-embedded brain samples from the proband revealed the A8344G myoclonic epilepsy and ragged-red fiber (MERRF) mutation as the molecular basis for this mul

[Influence of kidney procurement techniques on urologic and vascular complications of the transplantation].

Surgical complications of renal transplantation, rejection and infectious diseases are factors contributing to poor renal graft survival. Factors directly concerning the donor can be involved in graft failure: age, medical history, causes of donor brain death. Urologic or arterial anatomic variations are often the source of difficult surgical conditions during renal transplantation. Technical errors during graft procurement must be avoided such as excess of traction or coagulation. Failure in perfusion preservation. As few renal grafts are available, it is thus essential to obtain optimal cond…

Neuroprotective effects of alpha-dihydroergocryptine against damages in the substantia nigra caused by severe treatment with 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine.

The effects on the substantia nigra of alpha-dihydroergocryptine (DEK), a drug with strong dopaminomimetic activity, were tested with a severe 1-methyl-4-phenyl-1,2,3,6-tetrahydropyridine (MPTP) treatment in monkeys. Compared with monkeys treated with MPTP alone, the animals which received DEK plus MPTP showed reduced neuronal death in the substantia nigra. The reactive astrocytes were increased in number. Moreover, several axons which were immunopositive to phosphorylated neurofilament proteins and with features similar to those of control animals were seen in the pars compacta. The findings

Protective and curative effects of rifampicin in Acanthamoeba meningitis of the mouse.

BALB/c mice inoculated nasally with Acanthamoeba culbertsoni, resulting in amebic encephalitis and death 3-7 days, were treated with rifampicin prophylactically (daily for 2 days with 75 and 100 mg/kg) and after infection (daily for 5 days with doses of 10-100 mg/kg). Prophylactic treatment resulted in full protection against infection, as assessed by absence of symptoms of central nervous system malfunction and negative brain culture 10 days after inoculation. Curative treatment was effective at the same doses; however, at doses of 10, 25, and 50 mg/kg, only two of six animals were free of sy

Replication of Langat virus in immunocompetent cells of mice subjected to immobilization stress.

Immobilization stress (hypokinesis) in Balb/c mice may aggravate asymptomatic infection with Langat virus (strain TP-21) as evidenced by 4-fold increased lethality in comparison with control animals. The virus levels in the spleen and brain of stressed and infected mice and the in vitro yield of the virus in immunocompetent cells derived from stressed mice were significantly higher than in controls. Enhanced virus replication in latter cells may contribute to increased accumulation of the infectious agent in lymphatic tissues, which would facilitate virus invasion into CNS followed with acute

Fatal nonmeningitic Listeria rhombencephalitis. Report of two cases.

Two cases of rapidly fatal Listeria rhombencephalitis with normal cerebrospinal fluid (CSF) findings occurred in previously healthy adults. The infection presented with nausea and headache followed by fever and signs of lower cranial nerve dysfunction, without associated meningismus, and progressed to death within four and six days of hospitalization. Because of normal CSF findings (including ventricular fluid in one patient) and negative culture results of both blood and CSF, the diagnosis was not suspected. Listeriosis should be considered early in any febrile patient presenting with signs o

Hydatid cysts in children and youths.

In the 15 years from 1960 through 1974, 58 patients (38 male and 20 female) from 3 to 18 years of age were treated. In 24 only the lungs were involved, in 23 only the liver, and in 2 both organs were involved. Various other organs including the orbit, omentum, kidney, uterus, brain, and spinal cord contained cysts in one or more cases. Diagnosis was by X-ray, angiography, and myelography. The Casoni skin test was positive in 36 of 43 (83.7%) trials. All patients were treated surgically and, except for one death from pneumonia, all did well.

[The death of Ravel].

The composer Maurice Ravel (1875-1937) was chronically ill during the last five years of his life. He was suffering from Pick's astrophy, the first signs of which appeared in 1923 and 1927 and which were probably already reflected in the stereotypy of the "Bolero" of 1928. Transient apractic symptoms were prominent, later, difficulty in finding words, agraphia and alexia. Ravel left no completed composition after an accident to the head in 1932. Complete apathy and involutional phenomena characterize the period before the death of the master, who passed away a few days after a surgical operati

Characteristics of traumatic brain injury-related healthcare visits across social determinants of health: A population-based birth cohort study.

BACKGROUND: Traumatic brain injury is a major cause of death and disability worldwide, with almost half of new cases occurring in children, adolescents, and young adults. However, data on injury characteristics stratified by social determinants of health are scarce. This study explores severity, intent, and mechanism of traumatic brain injury sustained during childhood, adolescence, and young adulthood by social determinants of health. METHODS: This study utilizes a population-based birth cohort of births in publicly funded hospitals in Ontario, Canada, between April 1, 1992 and March 31, 202

Tissue-specific and repeat length-dependent somatic instability of the X-linked dystonia parkinsonism-associated CCCTCT repeat.

X-linked dystonia-parkinsonism (XDP) is a progressive adult-onset neurodegenerative disorder caused by insertion of a SINE-VNTR-Alu (SVA) retrotransposon in the TAF1 gene. The SVA retrotransposon contains a CCCTCT hexameric repeat tract of variable length, whose length is inversely correlated with age at onset. This places XDP in a broader class of repeat expansion diseases, characterized by the instability of their causative repeat mutations. Here, we observe similar inverse correlations between CCCTCT repeat length with age at onset and age at death and no obvious correlation with disease du

A Clinical Report of Two Cases of Cryptogenic Brain Abscess and a Relevant Literature Review.

Brain abscess, a severe intracranial infectious disease, refers to the parenchyma abscess caused by local infection or remote spread. Recently, advancements in modern medicine, especially the wide application of antimicrobial drugs, have contributed to the gradual decrease in the prevalence of this disease. However, cases of cryptogenic brain abscess that feature an unknown origin and atypical symptoms are rising. In this retrospective study, we report and analyze two cases of cryptogenic brain abscess. The first patient was a 30-year-old healthy man who was admitted to our hospital due to 1 w

[Experimental slow influenza infection in mice].

In baby mice born to mothers--virus carriers (surviving for various periods after intranasal administration of influenza virus), infectious persisting influenza virus in titres of 10(0.5) to 10(2.5) EID50/0.1 ml was found in the blood, lungs, livers, kidneys, spleens, and brains. Not infrequently, such animals developed a severe pathological process accompanied by growth retardation and characterized by progressive involvement of the hypothalamus, immunocompetent organs, endocrine system and always terminating by the death of the animals. Possible mechanisms of the described slow form of influ

Extracellular vesicles derived from plasmodium-infected red blood cells alleviate cerebral malaria in plasmodium berghei ANKA-infected C57BL/6J mice.

RTS,S is the first malaria vaccine recommended for implementation among young children at risk. However, vaccine efficacy is modest and short-lived. To mitigate the risk of cerebral malaria (CM) among children under the age of 5, it is imperative to develop new vaccines. EVs are potential vaccine candidates as they obtain the ability of brain-targeted delivery and transfer plasmodium antigens and immunomodulators during infections. This study extracted EVs from BALB/c mice infected with Plasmodium yoelii 17XNL (P.y17XNL). C57BL/6J mice were intravenously immunized with EVs (EV-I.V. + CM group)

Endogenous Protection from Ischemic Brain Injury by Preconditioned Monocytes.

Exposure to low-dose lipopolysaccharide (LPS) before cerebral ischemia is neuroprotective in stroke models, a phenomenon termed preconditioning (PC). Although it is well established that LPS-PC induces central and peripheral immune responses, the cellular mechanisms modulating ischemic injury remain unclear. Here, we investigated the role of immune cells in the brain protection afforded by PC and tested whether monocytes may be reprogrammed by ex vivo LPS exposure, thus modulating inflammatory injury after cerebral ischemia in male mice. We found that systemic injection of low-dose LPS induces

Effects of matrix metalloproteinase 9 inhibition on the blood brain barrier and inflammation in rats following cardiopulmonary resuscitation.

BACKGROUND: Neuroprotective strategies following cardiopulmonary resuscitation (CPR) are an important focus in emergency and critical care medicine. Matrix metalloproteinases (MMPs), especially MMP9 attracted much attention because of its function in focal brain ischemia/reperfusion injury. In the focal cerebral ischemia model in rats, SB-3CT can suppress the expression of MMP9, relieving brain edema, and there was no studies on global cerebral ischemia-reperfusion injury after CPR. METHODS: One hundred and twenty rats were randomly assigned to sham-operated (n = 40), resuscitation treatment
