Advances in videoassisted anterior surgical approach to the craniovertebral junction.

At the present time, an update to the classical microsurgical transoral decompression is supported by the most recent literature dealing with the introduction of the endoscopy in spine surgery. In this paper, we present all the reported experience on the surgical approaches to anterior cranioveretebral junction (CVJ) compressive pathology managed by endoscopy. Surgical strategies dealing with decompressive procedures by using an open access, microsurgical technique, neuronavigation and endoscopy are summarized.Endoscopy represents a useful complement to the standard microsurgical approach to t

Characteristics of lower extremity clonus after human cervical spinal cord injury.

Clonus can interfere with self-care and rehabilitation of people with spinal cord injury. Our aim was to characterize clonus and to evaluate factors that influence clonus duration in muscles paralyzed chronically by spinal cord injury. Electromyographic activity was recorded from soleus and 7 other limb muscles (5 ipsilateral, 2 contralateral) during clonus. In 14 subjects, clonus frequency in soleus averaged 5.4±0.9 Hz and was slower when the reflex path was longer. Contraction frequency slowed at the beginning and end of clonus (sometimes by 2 Hz). The magnitude of one cycle changed the timi

Imaging of the cervical spine in injured patients.

Cervical spine injury can be excluded by clinical examination, without the need for radiographic study, in many patients. For those who require study, computed tomography of the cervical spine with sagittal and coronal reconstruction is the best modality for both screening and diagnosing cervical spine injury. Optimal evaluation of the obtunded patient remains controversial.

Development and characterization of a novel rat model of cervical spondylotic myelopathy: the impact of chronic cord compression on clinical, neuroanatomical, and neurophysiological outcomes.

Cervical spondylotic myelopathy (CSM) is the most common cause of spinal cord impairment worldwide and is a risk factor for traumatic central cord syndrome. Despite advances in surgery, there are no effective neuroprotective treatments for CSM, which reflects a limited understanding of its pathophysiology. In order to develop therapeutic strategies, we have developed a novel rat model of chronic progressive cervical spinal cord compression that mimics CSM. A titanium-screw-based chronic compression device (CCD) was designed to achieve progressive cord compression at the C6 level. The CCD was f

Quadruple spontaneous cervical artery dissection following aneurysm embolization: a rare posttreatment complication.

We present a unique case of multiple sCADs occurring after a ruptured intracranial aneurysm embolization. We discuss the impact of head extension during embolization as the prevailing factor in multiple artery dissections in this case and point out another cause of new-onset neurologic deficit in patients with aneurysmal SAH.

Caudal cervical arthrodesis using a distractable fusion cage in a dog.

This case report describes a cervical fusion cage, surgical technique and the long-term outcome of caudal cervical arthrodesis used to stabilise dynamic spinal cord compression at the sixth and seventh cervical intervertebral disc space (C6, C7) in a dog. A seven-year-old, 41 kg, entire male Dobermann Pinscher was admitted for progressive ataxia of two weeks duration. Neurological examination revealed ambulatory tetraparesis. Computed tomographic myelogram scans in neutral and traction positions of the neck were performed and were used to determine presence of a dynamic component. A C6-C7 surg

[Pott's disease of the upper cervical spine (three cases and literature review)].

BACKGROUND: Sub-occipital Pott's disease is rare, but dangerous. AIM: To recall diagnostic aspects and spatially the neuroradiological ones of sub-occipital Pott's disease. METHODS: We describe three patients with sub-occipital Pott's disease followed in our department from 2002 to 2007. RESULTS: There were two women and one man (mean age at onset: 48 years). The early clinical picture was non specific. Exploration, in addition to standard radiographies included a computerized Tomography (CT) (n=2) and MRI (n=1 and n=3). The diagnosis was confirmed histologically on a biopsy of the abscess

The long-term results of anterior surgical reconstruction in patients with postlaminectomy cervical kyphosis.

BACKGROUND CONTEXT: Postlaminectomy kyphosis of the cervical spine is a challenging condition to treat because it has a combination of an exposed cord, progressive kyphosis, segmental instability, and anterior neural compression. The ideal mode of surgical correction remains controversial. In terms of surgical strategy, there are few large series that have reported the long-term results of anterior surgical treatment of this condition. PURPOSE: This study was designed to determine the long-term results and outcomes of anterior surgical treatment alone for the patients of postlaminectomy cervi

Central cord syndrome.

Central cord syndrome is the most common type of incomplete spinal cord injury. This syndrome most often occurs in older persons with underlying cervical spondylosis caused by a hyperextension mechanism. It also occurs in younger persons who sustain trauma to the cervical spine and, less commonly, as a result of nontraumatic causes. The upper extremities are more affected than the lower extremities, with motor function more severely impaired than sensory function. Central cord syndrome presents a spectrum, from weakness limited to the hands and forearms with sensory preservation, to compete qu

Achondroplasia and cervical laminoplasty.

Achondroplasia is associated with short pedicles that predispose individuals with this trait to develop symptomatic spinal canal stenosis. Laminoplasty is an excellent means of treating cervical myelopathy due to stenosis in selected individuals. Laminoplasty preserves segmental motion and stability, both of which are of benefit to all individuals. The authors report the successful surgical treatment of an achondroplastic adult woman with laminoplasty. This procedure alleviated her symptoms, and she was doing well at 2-year follow-up.

Problems related to cervical fusion: malalignment and nonunion.

Two common complications associated with cervical surgery are malalignment, particularly after multilevel laminectomies, and nonunion. In both situations, prevention and early recognition are critical in clinical management. In postlaminectomy kyphosis, the prevailing cause is an alteration in the normal spinal biomechanics. The exact incidence is unknown; however, this malalignment can cause significant morbidity. Surgical treatment is used once a patient becomes symptomatic. Anterior cervical procedures are frequently used to correct the deformity. Like postlaminectomy kyphosis, most nonunio

Vasovagal reactions and other complications of cervical vs. lumbar translaminar epidural steroid injections.

OBJECTIVE: To document the per patient rate of vasovagal reactions and other complications of cervical vs. lumbar translaminar epidural steroid injections. BACKGROUND: Anecdotal observations at our institution suggested that vasovagal reactions are much more common during cervical epidural steroid injections than lumbar injections, and more frequent than previously reported. METHODS: Records of 249 patients undergoing their first cervical epidural steroid injection were reviewed for vasovagal reactions and other adverse events. For comparison, a first lumbar epidural steroid injection perfor

Isolated spinal neurocysticercosis with clinical pleomorphism.

Neurocysticercosis, involvement of the central nervous system by taenia solium, is one of the most common parasitic diseases worldwide. However, isolated spinal involvement by neurocysticercosis, either intramedullary or extramedullary, is uncommon. The authors report a case of C1-C2 intradural extramedullary neurocysticercosis in a young male that presented initially with signs of raised intracranial pressure followed by high cervical myelopathy. Transventricular migration of the neurocysticercal cyst along the cerebrospinal pathways leading to clinical pleomorphism as described by the author

Subaxial positional vertebral artery occlusion corrected by decompression and fusion.

STUDY DESIGN: A rare case of subaxial vertebral artery (VA) positional occlusion is reviewed and treatment methods discussed. OBJECTIVE: The decision process involved in treating subaxial VA positional occlusion is reviewed. SUMMARY OF BACKGROUND DATA: Bow Hunter stroke is a symptomatic vertebrobasilar insufficiency caused by stenosis or occlusion of the VA with physiologic head rotation. It most commonly occurs at the junction of C1 and C2 and less commonly as the VA enters the C6 transverse foramen. Rotational stenosis of the VA is quite rare during its passage through the foramen transver
