Etiology and outcome of community-acquired lung abscess.

BACKGROUND: Anaerobes are the first and Streptococcus species the second most common cause of community-acquired lung abscess (CALA) in the West. The etiologic pathogens of this disease have changed in Taiwan, with Klebsiella pneumoniae being reported as the most common cause of CALA. OBJECTIVE: To determine the etiologies of community-acquired lung abscess. METHODS: We retrospectively reviewed the records of 205 Japanese adult patients with CALA to evaluate etiologies and outcomes. We used not only traditional microbiological investigations but also percutaneous ultrasonography-guided trans

Community-acquired pneumonia in an intensive care unit.

Community-acquired pneumonia (CAP) is the leading cause of sepsis in adult critical care. We present a retrospective study of patients admitted to a polyvalent intensive care unit with CAP from 1st June 2004 - 31st December 2006. We analysed 76 patients with a mean age of 62.88 (18.75) years. Mean APACHE II score was 24.88 (9.75). Mean SAPS II was 51.18 (18.05), with a predicted mortality of 47.27%. Aetiology was identified in 42.1% of the patients. Streptococcus pneumoniae was the most frequent aetiological agent, but the group of aetiological agents more frequently identified was Gram-negati

Diagnosis of malaria by community health workers in Nigeria.

OBJECTIVE: The introduction of primary health care made Nigeria, a developing country, train and retrain community health workers to work all over the country especially in the rural communities where there is dearth of doctors. Despite their training and experience many people are skeptical of their competence to diagnose accurately what more treating endemic disease like malaria. The need to find out the diagnostic competence of the health workers in malaria control programme now in Nigeria necessitated this study. METHOD: A rural primary health centre, the sentinel site for malarial contro

Urinary schistosomiasis in the Danjarima community in Kano, Nigeria.

BACKGROUND: Studies in northern Nigeria have suggested a linear relationship between urinary schistosomiasis and individual water-related activities. Knowledge of the perceptions of the local populace about schistosomoasis and of gender and cultural restrictions could be beneficial to control programmes. We studied the prevalence of urinary schistosomoasis and the socio-demographic factors associated with the disease in Danjarima community of Kano, northern Nigeria. METHODOLOGY: A cross-sectional survey involving 890 subjects was conducted in the community. Urine samples were collected and ex

Representations of autism: implications for community healthcare practice.

The work presented in this paper is part of a larger project in which online asynchronous discussion groups were employed to examine how a range of contributors--including people with autism, parents of people with autism, and professionals working within the field of autism--view and understand autism. In this paper, we focus on the voices of people with autism. The terminology used in the paper takes its lead from the writings of people with autism, who frequently use the term 'AS' to refer to autistic spectrum and Asperger syndrome and 'NT', which refers to neurologically typical people wit

Reasons for relocation to retirement communities: a qualitative study.

The purpose of this qualitative study is to understand the reasons why elders move to retirement communities and what living in retirement communities is like from the perspective of relocated elders. Several themes emerged following qualitative analysis. The themes reflect three categories, labeled as "pushing" factors, "pulling" factors, and "overlapping" factors. Pushing factors included own or spouse's failing health, getting rid of responsibilities, not helped, facility closed out, and loneliness. Pulling factors were location, familiarity and reputation of the facility, security, and joi

Identifying hidden sexual bridging communities in Chicago.

Bridge populations can play a central role in the spread of human immunodeficiency virus (HIV) by providing transmission links between higher and lower prevalence populations. While social network methods are well suited to the study of bridge populations, analyses tend to focus on dyads (i.e., risk between drug and/or sex partners) and ignore bridges between distinct subpopulations. This study takes initial steps toward moving the analysis of sexual network linkages beyond individual and risk group levels to a community level in which Chicago's 77 community areas are examined as subpopulation

White matter abnormalities and cognition in a community sample.

White matter hyperintensities (WMH) can compromise cognition in older adults, but differences in sampling, WMH measurements, and cognitive assessments contribute to discrepant findings across studies. We examined linear and nonlinear effects of WMH volumes on cognition in 253 reasonably healthy adults. After adjusting for demographic characteristics and total brain volumes, WMH burden was not associated with cognition in those aged 20-59. In participants aged 60 and older, models accounted for > or =58% of the variance in performance on tests of working memory, processing speed, fluency, and f

Community pharmacy involvement in vaccine distribution and administration.

This study identified the type and number of doses of vaccine purchased, distributed, and administered in community pharmacies. Telephone interviews were conducted with 1704 community pharmacies in 17 states (response rate=69.1%). The 17-state population-level projections reveal that about 10% of hepatitis A, hepatitis B, meningococcal, MMR, and tetanus-containing vaccines for adults were administered in pharmacies and 90% were distributed to other sources for administration during July 2004-June 2005. Further, 24.1% of diphtheria-tetanus-pertussis for children (DTaP), 30.4% of influenza, 36.2

Bacterial communities associated with flea vectors of plague.

The microbial flora associated with fleas may affect their ability to transmit specific pathogens, including Yersinia pestis, and also could be used to develop paratransgenesis-based approaches to interfere with transmission. To begin addressing this hypothesis, the microbial flora associated with the relatively efficient Y. pestis vectors Xenopsylla cheopis (Rothschild) (Siphonaptera: Pulicidae) and Oropsyllamontana (Baker) (Siphonaptera: Ceratophyllidae), and the inefficient vector Ctenocephalides felis felis (Bouché) (Siphonaptera: Pulicidae) were investigated using polymerase chain reactio

Biennial hyperepidemic shigellosis in an observant Jewish community.

We aimed to study patterns of shigellosis in a large observant Jewish community in Israel and to describe local interventions during outbreaks. Surveillance data from the Ministry of Health were used to calculate incidence rates in 1998-2006, both in the city of Bene Beraq and the Tel Aviv district. Information on isolates was collected from the bacteriological laboratory of a community hospital. Public notices and educational conferences conveyed a message to increase personal hygiene and keep ill children at home. During a 9-year period, a clear biennial pattern of outbreaks was noticed. Managing chronic oedema: a collaborative community approach.

Chronic oedema affects over 100 000 people in the UK and is regularly treated by different health care professionals, most commonly community nurses. The effect of chronic oedema on patients can be both physical and emotional and is a huge financial burden on the NHS. Collaborative working between lymphoedema services and community nurses is outlined in this article, highlighting potential benefits to patient care and substantial cost savings. Modified lymphoedema management strategies to treat chronic oedema effectively are identified with the emphasis on joint packages of care and patient go

Cycloidal vibration for the treatment of cellulitis in a community setting.

Cellulitis of the lower limb is a common infection seen in primary care resulting in a significant number of hospital admissions per year. The NHS is proposing and developing services to treat patients more effectively in the community to prevent hospital admission. Cycloidal vibration (CV) therapy is a medical device that when combined with standard antibiotic therapy for cellulitis results in an effective treatment. This retrospective analysis is a review of patients with cellulitis seen in a community medical centre before and after the introduction of CV therapy. This therapy in conjunctio

Increasing accessibility for community participants at academic conferences.

BACKGROUND: To decrease the gulf between academic and community perspectives, there has been an increased call among stakeholder groups to collaborate in solving complex urban health problems. Despite these recent shifts, however, community partners face barriers to participate in conferences, ultimately limiting exchange of ideas and uptake of research data. This paper reports on the evaluation of the strategy used to engage community participants at the 4th International Conference on Urban Health (ICUH) held in Toronto, Canada, in October, 2005. METHOD: We surveyed participants (n = 98) an

Eighty-five percent of reproductive-aged women experience physical or emotional changes with their menstrual cycle. Up to 40% of women are b … edicines and anxiolytics. Effective treatment of cyclic symptoms hinges on effective doctor-patient communication. Patients who report that their health care providers converse with them and engage them in decision making obtain more relief and are more satisfied with their health care.
