Increasing Utilization of Palliative Care is Associated With Reduced Health Care Costs in Operative Trauma: A National Analysis.

IntroductionPalliative care (PC) has been shown to improve comfort for surgical patients nearing the end of life. Although single-institution studies suggest PC to be a cost-effective strategy, the contemporary national trends in costs and utilization of this modality remain unknown.MethodsAdult patients (≥18 years) who did not survive following hospitalization for surgical management of traumatic injury were tabulated in the 2016 to 2020 Nationwide Readmissions Database. Patients were stratified by receipt of PC. Entropy balancing on key covariates was used to ensure an equivalent comparison

Mortality prediction after major surgery in a mixed population through machine learning: a multi-objective symbolic regression approach.

INTRODUCTION: Understanding 1-year mortality following major surgery offers valuable insights into patient outcomes and the quality of peri-operative care. Few models exist that predict 1-year mortality accurately. This study aimed to develop a predictive model for 1-year mortality in patients undergoing complex non-cardiac surgery using a novel machine-learning technique called multi-objective symbolic regression. METHODS: A single-institution database of patients undergoing major elective surgery with previous cardiopulmonary exercise testing was divided into three datasets: pre-operative c

The effect of a post-bronchodilator FEV1/FVC < 0.7 on COPD diagnosis and treatment: a regression discontinuity design.

BACKGROUND: Global Initiative for Chronic Obstructive Lung Disease (GOLD) guidelines recommend the diagnosis of chronic obstructive pulmonary disease (COPD) only in patients with a post-bronchodilator forced expiratory volume in 1 s to forced vital capacity ratio (FEV1/FVC) less than 0.7. However the impact of this recommendation on clinical practice is unknown. OBJECTIVE: To estimate the effect of a documented post-bronchodilator FEV1/FVC < 0.7 on the diagnosis and treatment of COPD. DESIGN: We used a regression discontinuity design to measure the effect of a post-bronchodilator FEV1/FVC <

Nationwide Analysis of Failure to Rescue After Liver Transplantation.

INTRODUCTION: Failure to rescue (FTR) is mortality after a major complication. FTR may be an effective quality metric in liver transplantation (LT). However, there is a paucity of nationwide data on the rates and effects of FTR on outcomes. Our study aims to determine the nationwide rate of FTR and its impact on outcomes after LT. METHODS: We analyzed the 2015-2017 Nationwide Readmissions Database, including all patients with LT. Patients were stratified into terciles of average center mortality of < 1% for low (L), 1%-5.76% for intermediate (I), and > 5.76% for high (H). Postoperative compli

Time-Series Bert for Sepsis Detection: Uncovering Patient Trajectories Through Vital Sign Embeddings.

This study adapts BERT for vital sign time-series analysis in sepsis detection. Using MIMIC-III data, our model's embeddings reveal patient clusters that partition septic from non-septic cases while capturing physiological complexity through diagnosis count distributions. The BERT-based classifier achieves robust performance in both Precision-Recall Area Under Curve (PR AUC), measuring precision maintenance across recall thresholds, and Receiver Operating Characteristic Area Under Curve (ROC AUC), quantifying septic/non-septic case discrimination. Unsupervised learning reveals patient subgroup

Normative Database of Spatiotemporal Gait Metrics Across Age Groups: An Observational Case-Control Study.

INTRODUCTION: Gait analysis is a vital tool in the assessment of human movement and has been widely used in clinical settings to identify potential abnormalities in individuals. However, there is a lack of consensus on the normative values for gait metrics in large populations. The primary objective of this study is to establish a normative database of spatiotemporal gait metrics across various age groups, contributing to a broader understanding of human gait dynamics. By doing so, we aim to enhance the clinical utility of gait analysis in diagnosing and managing health conditions. METHODS: W

Simplifying Universal Pediatric Pre-operative Anesthesia Screening: Analysis of NSQIP Data to Evaluate the NARCO Score as a Tool for Pediatric Anesthesia Risk Stratification.

BACKGROUND: Universal pre-operative anesthesia screening may be resource-prohibitive and burdensome to hospitals and patients. However, patients at greatest anesthetic risk cannot be identified without some basis for risk-stratification. AIMS: This study investigated if a modified version of the Neurological, Airway, Respiratory, Cardiovascular, and Other (NARCO) pre-surgical pediatric risk score could stratify patients into high- and low-anesthetic-risk groups that would be predictive of perioperative adverse events. METHODS: A retrospective cohort study was conducted using 132,881 records
