Treatment of vertebral hemangiomas with absolute alcohol (ethanol) embolization, cord decompression, and single level instrumentation: a pilot study.

BACKGROUND: Vertebral hemangiomas (VH) are the most common lesions of the vertebral column. OBJECT: To evaluate the role of intraoperative ethanol embolization, surgical decompression, and instrumented fusion in VH presenting with myelopathy. METHODS: This is was a prospective study of single-level symptomatic VH with cord compression. Exclusions were as follows: pathological fractures, deformity, or multilevel pathologies. Surgery consisted of intraoperative bilateral pedicular absolute alcohol injection and laminectomy at the level of pathology followed by a short-segment instrumented fusi

Vertebral fractures in dialysis: Endocrinological disruption of the bone-kidney axis.

BACKGROUND: The treatment of atlantoaxial instability (AAI) involves stable fixation and fusion with adequate decompression of spinal cord. After the advent of the Goel posterior joint manipulation technique, most of the once irreducible atlantoaxial dislocations (AAD) could be reduced and the need for transoral odontoidectomy became almost nil. Here we tried to iterate the indications of anterior transoral odontoid surgery for AAI in the current scenario. METHODS: A retrospective study compiling the clinical, radiological, and surgical characteristics of 6 cases (5 scenarios). These patients

Primary axillary-subclavian venous thrombosis: is aggressive surgical intervention justified?

Multimodal (thrombolysis, surgical decompression, venous reconstruction, oral anticoagulation) treatment of primary axillary-subclavian venous thrombosis was reviewed to assess the impact of venous patency on functional outcome. Since 1996, 7 patients (6 men, 1 woman) of ages 16-53 years (mean 33 years) presented with symptomatic acute axillosubclavian venous thrombosis as a result of a recent athletic or strenuous arm activity. Five patients had undergone previous (>2 weeks) catheter-directed thrombolysis and venous angioplasty. Diagnostic contrast venography followed by repeat catheter-direc

Decompression stent, stoma, transanal tube or immediate surgery: Systematic review and Bayesian network meta-analysis for left-sided malignant colonic obstruction.

BACKGROUND: The optimal initial management of left-sided malignant colonic obstruction (LMCO) remains debated, as previous studies have reported inconsistent results. To address this uncertainty, we conducted a systematic review and Bayesian network meta-analysis comparing various initial management strategies for potentially curable LMCO. The objective was to evaluate and compare short-term and long-term outcomes among bridge-to-surgery (BTS) approaches and immediate surgery. METHODS: PubMed, Embase and the Cochrane Library were systematically searched until the end of September 2025. Eligib

Safety of early discharge after elective lumbar spine surgery with subfascial drains and association with significant reduction in length of stay.

OBJECTIVE: After lumbar spine surgery, postoperative drain removal often delays discharge. Whether inpatient drain removal reduces the risk of surgical site infection (SSI) or hematoma remains controversial. Therefore, in patients undergoing elective lumbar spine surgery, the authors sought to determine the impact of inpatient versus outpatient drain removal on the following variables: 1) length of hospital stay (LOS), and 2) postoperative complications. METHODS: A single-center retrospective cohort study in which the authors used prospectively collected data of patients undergoing primary, e

Radiation stewardship in uniportal lumbar endoscopic decompression.

OBJECTIVE: Uniportal lumbar endoscopic decompression is fluoroscopy-dependent, and the two dominant approaches, transforaminal and interlaminar, might impose distinct intraoperative targeting demands. Therefore, the aim of this study was to examine differences in radiation exposure based on technique and identify modifiable predictors beyond choice of approach. METHODS: The authors identified 51 consecutive patients who underwent single-level uniportal lumbar endoscopic decompression with and without unilateral erector spinae blocks (ESBs). After excluding 4 with bilateral procedures, 2 with

Comparison of survival, neurological, and radiographic outcomes between corpectomy and instrumented separation surgery for spinal metastatic disease: A single institutional experience.

OBJECTIVES: Spinal metastatic disease incidence has increased due to advances in radiation and chemotherapy that improve survival. Surgical treatment includes separation surgery, for decompression of neural elements from the tumor to allow for post-operative stereotactic radiation versus maximal tumor resection via corpectomy. Data is mixed on whether more invasive corpectomy for tumor resection provides significant benefit over less invasive separation surgery. METHODS: A retrospective review of 100 consecutive patients was performed, 60 underwent corpectomy and 40 were treated with instrume

Efficacy of autologous stem cell at different doses combined with core decompression in the treatment of osteonecrosis of the femoral head: a systematic review and network meta-analysis.

OBJECTIVE: To compare autologous stem cell (SC) at different doses combined with core decompression (CD) versus CD alone for osteonecrosis of the femoral head (ONFH), using conversion to total hip arthroplasty (THA) as the primary outcome. METHODS: PubMed, Embase, Cochrane Library, Web of Science, and CNKI were systematically searched (from inception to July 2025) to identify studies comparing different SC dosages (low dose: <1×10^7; medium dose: 1×10^7-10×10^8; high dose: >1×10^8). Conventional meta-analyses were performed using Review Manager 5.3, while network meta-analyses (NMA) were cond

Objectively measured physical activity following lumbar decompression surgery: systematic review and meta-analysis.

Lumbar decompression surgery reliably improves pain and disability in lumbar disc herniation and spinal stenosis, but whether these symptomatic gains translate into greater free-living physical activity (PA) remains unclear. This systematic review and meta-analysis evaluates postoperative changes in objectively measured PA, and its association with patient-reported outcome measures (PROMs). PubMed, MEDLINE, EMBASE and Scopus databases were searched to identify studies reporting objectively measured PA after lumbar decompression, including laminectomy, laminotomy, foraminotomy and discectomy. Motor Strength Recovery Following Unilateral MILD® Procedure in a Patient With Severe Spinal Stenosis and Associated Foot Drop.

BACKGROUND: Lumbar spinal stenosis (LSS) is a prevalent degenerative condition, particularly in individuals over 65, and is a leading cause of disability and spinal surgery in this population. The hallmark symptom is neurogenic claudication, but severe cases may present with focal motor deficits, such as foot drop. While open decompressive surgery remains the standard for patients with progressive neurological deficits, minimally invasive techniques, like the minimally invasive lumbar decompression (MILD®, Stryker Medical, Kalamazoo, MI) procedure, have emerged as alternatives, especially for

Progressive Spastic Paraparesis Caused by Thoracic Spinal Epidural Lipomatosis: A Case Report and Systematic Review.

Background: Spinal epidural lipomatosis (SEL) is a rare disorder characterized by excessive epidural fat accumulation. Diagnosis may be challenging because imaging findings are frequently overlooked, and evidence-based management guidelines, particularly thoracic disease, remain limited. Case report: We present a case report of a 54-year-old male patient, who suffered from lower limb spastic paresis and associated muscle spasms (Nurick grade V). After a difficult diagnostic process, MRI of the thoracic spine confirmed dorsal/posterior spinal epidural lipomatosis (SEL; Manjila grade IIb-α) exte

Complications and Cost in Open Versus Endoscopic Lumbar Decompression: A Database Study.

STUDY DESIGN: A retrospective database study. OBJECTIVE: To compare complications and costs associated with endoscopic and open lumbar decompression on a large scale. BACKGROUND: Though open lumbar decompression is considered the gold standard, endoscopic procedures are on the rise. The majority of studies comparing endoscopic to open decompression have been limited to small retrospective studies or studies in other countries. METHODS: Patients who underwent single-level endoscopic or open lumbar decompression from 2017 to 2021 with a 2-year follow-up were identified in the PearlDiver datab

Comparison of open and percutaneous instrumented fixation for the treatment of spinal metastases.

OBJECTIVE: The primary goals in surgical treatment of spinal metastases are neural decompression and spinal stabilization. Understanding a patient's functional and oncological status plays a key role in patient selection. Careful patient selection and determining the ideal surgical strategy are paramount to achieving successful outcomes in spinal oncology given the high risk for perioperative morbidity. The aim of this study was to examine instrumented fixation for treatment of spinal metastatic disease and compare outcomes of open versus percutaneous fixation to understand factors that might

Andersson-like lesion in diffuse idiopathic skeletal hyperostosis; a rare diagnostic and therapeutic challenge.

BACKGROUND: Andersson-like lesions (ALL) in diffuse idiopathic skeletal hyperostosis (DISH) are rare and often underrecognized entities that can mimic infective spondylodiscitis or inflammatory lesions such as Andersson lesion (AL) in ankylosing spondylitis (AS). Timely differentiation is crucial to avoid misdiagnosis and inappropriate treatment. CASE PRESENTATION: We report a 57-year-old diabetic male presenting with progressive cervical myelopathy and radiological findings suggestive of a discovertebral lesion at C6-C7 in the setting of DISH. Despite initial suspicion of infection, negative

Superficial ocular vascular changes after orbital decompression in patients with thyroid ophthalmopathy measured by anterior segment OCT angiography; an observational study.

Thyroid eye disease (TED) is a common ophthalmologic manifestation of thyroid dysfunction. Despite various imaging techniques available, there hasn't been a widely adopted method for assessing the anterior segment vasculature in TED patients. Our study aimed to evaluate alterations in ocular surface circulation following orbital decompression surgery in TED patients and investigate factors influencing these changes. Using anterior segment optical coherence tomography-angiography (AS-OCTA), we measured ocular surface vascularity features, including vessel density (VD), vessel diameter index (VD
