Management of tuberculosis in children in low-income countries.

Children become infected when they are exposed to infectious adults with smear-positive tuberculosis (TB). Most children become infected, but few progress to disease (TB). Children at greatest risk of developing disease are those younger than 5 years of age, HIV-infected and severely malnourished. TB is diagnosed in a child when the child has been exposed to an infectious case, has symptoms and a radiological picture suggestive of TB. Children are treated by the DOTS strategy, and can be treated with 6- or 8-month regimens. HIV-infected children are treated with the same regimens. Children und

Use of vaccine trials to estimate burden of disease.

Vaccine trials, the most informative way of determining the efficacy of a vaccine, can also provide valuable information about the burden of disease. The burden of Haemophilus influenzae type b (Hib) remains a major barrier to the use of Hib vaccines, especially in Asia. Recent studies in Indonesia and Bangladesh have used vaccine-trial designs, with known effective vaccines, to estimate the vaccine-preventable burden of Hib disease in those communities. New vaccines against pneumonia and diarrhoeal diseases are usually directed at only one of various causes of the syndrome. In the case of pne

Comparison of haemoglobinometry by WHO Haemoglobin Colour Scale and copper sulphate against haemiglobincyanide reference method.

Although estimation of haemoglobin is essential for diagnosing anaemia and assessing its severity, many health centres in developing countries do not have the facilities for haemoglobinometry. The WHO Haemoglobin Colour Scale (HCS) method is a simple and inexpensive clinical device that was recently developed in order to diagnose anaemia in such centres. In Indonesia, the copper sulphate specific gravity method is used for blood donor screening and also in primary health clinics in the rural and remote areas. In this study, the HCS method is compared with the copper sulphate method and with an…

Historic and early life origins of hypertension in Africans.

Cardiovascular disease (CVD) causes 12.4 million deaths annually, most (9.6 million) occurring in developing countries. Hypertension, the most common CVD, arises within the context of obesity, but the underlying mechanisms remain obscure. Obesity and salt intake are two important risk factors for hypertension and are the focus of this paper. Traditional African populations show a low prevalence of hypertension, but hypertension is more common in migrant African populations in the West than in other ethnic groups. One explanation is genetic, but no causative gene has been confidently identified…

Obstetric fistula: the challenge to human rights.

Obstetric fistula can be explained to result from different causes. These holes in the tissue wall between the vagina and bladder and/or rectum are most prevalent in resource-poor countries, attributable to prolonged obstructed labour and absent or inaccessible remedial prenatal services. Obstructed labour is often due to small pelvic size, resulting from women's youth and premature childbearing and/or malnutrition. Poverty at national health-service and family levels often predisposes pregnant populations to suffer high rates of fistula. Global estimates showing up to 100,000 new cases each y

Feasibility of eliminating ocular Chlamydia trachomatis with repeat mass antibiotic treatments.

CONTEXT: Mass antibiotic administrations for ocular chlamydial infection play a key role in the World Health Organization's trachoma control program. Mathematical models suggest that it is possible to eliminate trachoma locally with repeat mass treatment, depending on the coverage level of the population, frequency of mass treatments, and rate that infection returns into a community after each mass treatment. Precise estimates of this latter parameter have never been reported. OBJECTIVE: To determine the rate at which chlamydial infection returns to a population after mass treatment and to es

Measles outbreak in a vaccinated population in Dhankutta.

A measles outbreak occurred in Dhankutta town in year 2056 Falgun to 2057 Baishak (3 months). A total of 130 cases were registered at the Dhankutta District Hospital. A large proportion of the cases had severe disease with complications and were admitted at the hospital. None of the patients registered at the hospital died. Interestingly, 50.0% of cases were in higher age group (10 years and above) which is an unusual trend in developing countries. All the cases gave history of measles vaccination, which points towards the need for investigation of vaccine efficacy and cold chain efficiency.

Partnership and promise: evolution of the African river-blindness campaigns.

This article describes the evolution of the partnership, between various health and developmental agencies, that has sustained the campaign against river blindness in Africa. The international community was oblivious to the devastating public-health and socio-economic consequences of onchocerciasis until towards the end of the 1960s and the beginning of the 1970s. Then a 'Mission to West Africa', supported by the United Nations Development Programme, and a visit to the sub-region by the president of the World Bank culminated, in 1974, in the inauguration of the Onchocerciasis Control Programme

Emerging issues in vaccine economics: perspectives from the USA.

Economic studies of vaccines, including vaccine development and delivery issues, are increasingly needed to inform policy recommendations an … funds and assessing the cost-effectiveness of HIV, rotavirus, meningococcal and malaria vaccines in developing countries. To optimize the usefulness of vaccine economics research, conceptual issues, such as how to set values for the prevention of illness and how to maximize social equity through investments in vaccines, must be addressed.

Immunizations for international travel.

Many infections encountered by international travelers can be prevented by adherence to personal protective measures and appropriate vaccinations. This review outlined the incidence and importance of the major vaccine-preventable infectious diseases encountered by U.S. travelers, as well as the indications, contraindications, and side effects of available vaccines. Official proof of yellow fever vaccine often is required for entry into some countries. Based on endemic or epidemic infections at destinations, planned activities, and age and medical history of international travelers, other vacci

Eradicating chancroid.

Genital ulcers are important cofactors of HIV transmission in the countries most severely affected by HIV/AIDS. Chancroid is a common cause of genital ulcer in all 18 countries where adult HIV prevalence surpasses 8% and is rare in countries with low-level HIV epidemics. Haemophilus ducreyi, the causative organism of chancroid, is biologically vulnerable and occupies a precarious epidemiological niche. Both simple, topical hygiene and male circumcision greatly reduce risk of infection and several classes of antibiotics--some of which can be administered in single-dose treatment regimens--provi

Active population growth and immigration hypotheses in Western Europe.

The paper examines, in respect of 12 Western European countries over a period of 20 years, the widely held view that any decline in their working population should be offset by greater reliance on immigrant labor. This research, based on demographic projections and forecasts regarding labor market participation rates by age and sex for each of the countries concerned, focuses on the two most likely scenarios. It appears that only Italy will be faced with a fall in its working population. All other western countries will either maintain the same level or, more generally, see their workforce gro

Review of current research in the prevention of nerve damage in leprosy.

Nerve damage and the consequences of nerve damage set leprosy apart from other diseases. The irreversible motor and sensory impairments caused by leprosy lead to increasing secondary impairments long after the disease process has been arrested. Interventions that prevent, reverse or limit the impairments resulting from leprosy are, therefore, of the highest priority. Self-care has been demonstrated to be an effective means of preventing secondary tissue damage, and its implementation must now be encouraged within the framework of basic health care. Currently, a comprehensive effort has been ma

Ocular cysticercosis--a review of 25 cases.

OBJECTIVE: To study the clinical manifestations, management and outcome of cases of ocular cysticercosis in India. METHODS: Retrospective analysis of records of patients presenting to the ophthalmology clinics, with cysticercosis, during years 1990-98. RESULTS: A total of 25 patients had ocular cysticercosis, during the period studied. Majority could be surgically removed without residual effects. However four patients (16%) had no useful vision left in the involved eye. CONCLUSION: Cysticercosis is still endemic in India and contributes to preventable blindness. Improving sanitation and he
