We describe a case of reversible dementia caused by Tropheryma whippelii (TW). Diagnosis was confirmed by a positive polymerase chain reaction for this pathogen both on serum and cerebrospinal fluid. Specific antibiotic therapy resulted in distinct clinical and neuroradiological improvement. Control polymerase chain reaction for T. whippelii on serum and cerebrospinal fluid was negative.

Cutaneous and soft tissue lesions are uncommon manifestations of brucellosis. Though breast involvement in animal brucellosis is not uncommon, involvement of the breast in human brucellosis is extremely rare. We report a case of breast abscess in a 39-year-old female caused by Brucella melitensis. Treatment with combination of trimethoprim/sulphamethoxazole (TMP/ SMX; cotrimoxazole) and doxycycline for 3 months resulted in clinical cure.

In a series of experiments in rabbits the dermal reaction, provoked by a single dose or intermittent doses of irradiation, was prevented or modified by topical or parenteral administration of local anaesthetics, compared to irradiated control animals. The topical application of a eutectic lidocaine/prilocaine cream, EMLA 5%, was found to be more effective than intravenously injected lidocaine (Xylocain 1%).

B. catarrhalis is a potential pathogen in the upper and lower respiratory tract which has been implicated as a clinically important cause of chronic bronchitis and otitis media in children. Since the late 1970s the proportion of B. catarrhalis strains elaborating beta-lactamase seems to have significantly increased; some centres are now reporting prevalence rates as high as 76%. Such a dramatic increase in the number of beta-lactamase positive strains is of clinical importance when assessing the indirect pathogenic potential of B. catarrhalis and when selecting suitable antimicrobial therapy.

We report 5 confirmed cases of boutonneuse fever. All patients presented fever with a generalised maculopapular rash, and the tache noire at the site of the tick bite. Trimethoprim sulfamethoxazole failed to cure one patient. Treatment with oral oxytetracycline was effective in all cases.

Acinetobacter baumannii represents a cunning pathogen with multiple resistance genes. The authors report their experience with the treatment of two multiple drug-resistant A. baumannii clones. At least one positive culture was noted in 359 patients and, 323 had sufficient data for analysis. Of these, 42 patients were colonized leaving 281 antibiotic-treated infected patients.
