How sharp is the short QuickDASH? A refined content and validity analysis of the short form of the disabilities of the shoulder, arm and hand questionnaire in the strata of symptoms and function and specific joint conditions.

PURPOSE: To assess and compare content, validity, and specificity of the QuickDASH (Disability of the arm, shoulder and hand questionnaire) as compared to the full-length DASH and other instruments to give a recommendation for its use depending on a specific clinical situation. METHODS: Data of three large cohorts of patients with shoulder (n = 138), elbow (n = 79), and carpo-metacarpal I (n = 103) arthroplasties were analyzed. The item content of both instruments was compared within the subdomains function and symptoms. Scores and correlations to other instruments were compared in all strata…

Medial pectoral nerve to musculocutaneous nerve neurotization for the treatment of persistent birth-related brachial plexus palsy: an 11-year institutional experience.

OBJECT: Medial pectoral nerve (MPN) to musculocutaneous nerve (MCN) neurotization for recovery of elbow flexion by biceps reinnervation is a valid option following traumatic injury to the upper brachial plexus. A major criticism of the application of this technique in infants is the smaller size of the MPN and mismatch of viable axons. We describe our institutional experience utilizing this procedure and critically examine functional outcomes. METHODS: Office charts and hospital records of children from over an 11-year period beginning January 1997 were reviewed. Of the 53 children of various…

Long-term follow-up of combined fractures of the proximal radius and ulna during childhood.

Forty-five children with combined fractures of the proximal end of the radius and ulna were treated in our orthopedic department over a period of 16 years (1984-1999). The age of the patients ranged from 5 to 12 years (mean 8 years) with a peak of 7-8 years. The mechanism of injury was a fall on the outstretched hand with the elbow extended and with a valgus force being applied to the elbow. In 35 patients, closed reduction or immobilization in plaster, or both, were performed. In the other 10 patients, a surgical treatment was followed either for fractures of the radius (4) or for both the ra…

Analysis of joint mobility patterns among preschool children.

CONTEXT AND OBJECTIVE: Standardization of normal joint mobility criteria is complex, especially for preschool children, for whom differentiation from hypermobility (JH) is even more difficult. This study aimed to investigate joint mobility of the fifth finger, wrist, elbow, knee and trunk among preschool children, estimate the incidence of JH and evaluate the effect of gender and age and the applicability of standard criteria for identifying JH among preschool children. DESIGN AND SETTING: Cross-sectional descriptive and quantitative study, at São José do Rio Preto medical school. METHODS: 1…

Management of comminuted fractures of the olecranon by tension band wiring.

BACKGROUND: Open reduction and rigid internal fixation has become the generally accepted method of treatment for displaced fractures of the olecranon in order to allow early mobilisation and to prevent contracture of the elbow. Comminuted fractures of olecranon are unstable, therefore, bone graft and tension band wiring are supposed to give good stability. Here we give an account of our experience with this procedure. METHODS: We treated ten patients with comminuted fractures of the olecranon by multiple tension-band wiring and a graft from the iliac crest between 1999 and 2002 at Ayub Teachi…

Task differences with the same load torque alter the endurance time of submaximal fatiguing contractions in humans.

Endurance time, muscle activation, and mean arterial pressure were measured during two types of submaximal fatiguing contractions that required each subject to exert the same net muscle torque in the two tasks. Sixteen men and women performed isometric contractions at 15% of the maximum voluntary contraction (MVC) force with the elbow flexor muscles, either by maintaining a constant force while pushing against a force transducer (force task) or by supporting an equivalent inertial load while maintaining a constant elbow angle (position task). The endurance time for the force task (1402 +/- 728…

Personal protective equipment use by in-line skaters in Victoria.

OBJECTIVE: To describe use of personal protective equipment (PPE) by in-line skaters at four different skate settings (rink, park, trail and street) in Melbourne and rural Victoria, and to compare local PPE with reported international use. METHOD: Unobtrusive observations of in-line skaters at four types of skate settings (rinks, parks, trails, street) in Melbourne and rural Victoria. RESULTS: Two-thirds of 490 observed in-line skaters (66.7%) wore none of the recommended PPE (wrist guards, elbow and knee pads and helmets). Only 2.2% wore all four pieces of PPE. Wrist guards were worn by 25.…

[Study of the proximal conduction of the arms using SEP and MEP].

The proximal conduction of the upper limb has been studied with the somatosensory evoked potentials (SEP) and motor evoked potentials (MEP) in 40 controls which provided normal data. The SEP allowed the sensory conduction study of the Erb/C7 segment with N9/N13 delay determination after the median nerve stimulation at the wrist, and after the ulnar nerve stimulation at the wrist (20 cases) or above the elbow (20 cases). The N9/N13 delay was 3.24 ms +/- 0.35 for the median nerve, 3.30 ms +/- 0.52 for the ulnar nerve after stimulation at the wrist and 3.30 ms +/- 0.40 after stimulation at the el…

Comminuted fractures of the proximal radius and ulna.

Forty-three comminuted fractures of the proximal radius and ulna in 34 patients were treated with operative stabilization using AO/ASIF techniques. The patients were divided into three groups, according to the type of injury: Group I, isolated comminuted fractures of the olecranon (18 patients); Group II, isolated fractures of the radial head (eight patients); Group III, combined olecranon and radial head fractures (eight patients). All fractures were followed until union. The average follow-up period was 18 months (range, 12-48 months). At the time of this review, the average limits of elbow …

[Examination and treatment of a loose medial coronoid process in dogs].

An ununited medial coronoid process (MCP) is the most common developmental disturbance in the elbow joint of young dogs of large and medium-sized breeds. The clinical and radiological symptoms of ununited medial coronoid processes in thirty-nine dogs are reported, the presence of an ununited medial coronoid process being established by arthrotomy in these cases. Lameness was observed for the first time in these dogs, mainly Rottweilers, Retrievers and Berner Sennenhunde, and more often male dogs than bitches, when they were from four to six months of age. They showed lameness ranging from slig…

Rehabilitation of burns of the upper limb.

The authors wish to document the experience of the Burns Unit, Singapore General Hospital in the management of burns of the upper limb. Fifty-six patients with post-burn deformity of the upper limb seen during a five year period (July 1978-July 1983) were reviewed. The cases were analysed and there were no significant differences in age, sex and race distribution. There were 8 axillary contractures, 9 elbow contractures, 10 wrist deformities and 56 hand deformities. Anterior axillary skin fold contracture was the commonest deformity occurring at the shoulder, whilst cubital contractures were m…

Does the ulnohumeral angle have a role in the aetiopathogenesis of lateral epicondylitis?

The pathophysiology of lateral epicondylitis (LE) remains not fully elucidated, as it involves a complex interaction of anatomical structures. The primary objective of the research is to identify a potential relationship between LE and the ulnohumeral angle (UHA), which demonstrates the coronal alignment of the elbow. Patients diagnosed with LE between September 1st, 2020, and September 1st, 2023, were retrospectively examined. Demographic information and UHA measurements of patients meeting the inclusion criteria and a control group with similar criteria were collected. Measurements were inde…

A Review of Workload-Monitoring Considerations for Baseball Pitchers.

Because of the unique demands of a pitch, baseball players have the greatest percentage of injuries resulting in surgery among high school athletes, with a majority of these injuries affecting the shoulder and elbow due to overuse from throwing. These injuries are believed to occur because of repeated microtrauma to soft tissues caused by the repetitive mechanical strain of throwing. Researchers and practitioners have suggested that baseball pitchers' workloads are a significant risk factor for injury in adolescent players, resulting in lost time and slowing of performance development. The pur…

Joint Contact Changes With Undersized Prosthetic Radial Heads.

OBJECTIVE: To evaluate the effect of intentional undersizing of prosthetic radial head implant diameters on joint contact pressures. METHODS: Eight fresh-frozen cadaveric elbows were aligned in neutral extension and loaded with 100 N using a custom testing apparatus. Radiocapitellar contact pressures were recorded using a Tekscan thin-film pressure sensor. Prosthetic radial head replacement was performed with 2 prostheses: the Anatomic Radial Head and the Evolve Proline Radial Head prostheses. Each design was sized according to the manufacturer's recommendations and then again using 2-mm smal…

Birth injuries to growth plates: A sheep in wolves' clothing.

Growth plate injuries (Salter-Harris type 1 or physeal fractures) of the long bones in the newborn are easily misdiagnosed as joint fractures with dislocations due to their nonossified epiphyses on plain radiographs. Diagnosis with musculoskeletal ultrasound (US) is advantageous due to its ability to visualize the nonossified epiphysis. We present two cases of humeral growth plate fractures in newborns, one at the shoulder (proximal humerus) and the other at the elbow (distal humerus). These cases emphasize the importance of quick and noninvasive diagnosis with US to avoid unnecessary interven…

Late presented case of distal humerus epiphyseal separation in a newborn.

The incidence of traumatic bone injuries occurring during birth is rare (1 per 1000 live births). Of all long bone fractures, the humerus is one of the commonest bones to be involved. However, distal humeral epiphyseal separation is rare and has seldom been written up in case reports and small case series. It warrants some critical assessment and appropriate attention. This injury is sometimes missed as it is difficult to diagnose at initial presentation. It may be mistaken for dislocation of the elbow, osteomyelitis, septic arthritis or brachial plexus injury, owing to lack of movement of the…
