[Magnetic stimulation in patients with essential blepharospasm].

In 31 patients with an essential blepharospasm investigations were undertaken with transcranial cortical and cisternal magnetic stimulation of the facial nerve. Cortical stimulation was performed contralaterally, whereas cisternal stimulation was ipsilateral; the latencies and amplitudes of the potentials were determined. However, the extracranial part of the facial nerve was stimulated in the region of the stylomastoid foramen for neuromyography electrically. In most measurements the response potentials in the orbicularis oculi muscle and the levator labii muscle were determined. Cisternal st

Neuromodulation techniques for managing overactive bladder.

Overactive bladder (OAB) represents a significant burden and has a considerable impact on the quality of life (QoL) of affected patients. The treatment of this condition poses notable challenges due to its multifactorial pathophysiology and the frequent refractoriness to standard pharmacological therapies. In recent years, neuromodulation has gained increasing relevance as an effective and minimally invasive therapeutic strategy, offering promising results in selected patients. Among the currently available modalities, sacral neuromodulation is considered the gold standard, supported by long-t

Neuromodulatory strategies overcome multiple inevitable impairments of cerebral palsy.

Cerebral palsy (CP) is traditionally characterized as a disorder of motor impairment; however, growing evidence suggests it reflects broader dysregulation across multiple physiological systems. We present a longitudinal case study examining whether pairing transcutaneous spinal neuromodulation (TSN) with a proprioceptive-derived intervention (PDI) can engage systems-level functional adaptation in an adolescent with CP spastic diplegia, whose progress had plateaued after years of conventional therapy. Across 2 years of the intervention, structured clinical observations revealed progressive and

Cortical potentials evoked by stimulation of cervical vagus vs. auricular nerve: a comparative, parametric study in nonhuman primates.

Stimulation of sensory vagal pathways is typically delivered via invasive, cervical vagus nerve stimulation (cVNS) or noninvasive, trans-auricular nerve stimulation (taNS). While both methods are investigated therapeutically, their effects on brain physiology remain poorly understood, hindering mechanistic insights and stimulus optimization. In 6 awake nonhuman primates, we recorded cortical vagal-evoked potentials (VEPs) from subdural electrodes placed in prefrontal, sensorimotor and parietal cortical areas, in response to cVNS or taNS. Across 478 different taNS and cVNS protocols, we varied

Modulating neuroimmune function: The key in taVNS therapy for neurological and psychiatric disorders.

Numerous lines of evidence indicate that the inflammatory state of neural tissue plays a significant role in the pathogenesis of neurological and psychiatric disorders, with neuroinflammation contributing to the dysregulation of cellular immunity and heightened susceptibility to such disorders. Previous research has demonstrated that vagus nerve stimulation (VNS) can attenuate inflammatory responses in both central and peripheral tissues, thereby ameliorating certain neurological disorders and enhancing cognitive function in the brain. Transcutaneous auricular vagal nerve stimulation (taVNS) h

Causal influence of brainstem response to transcutaneous vagus nerve stimulation on cardiovagal outflow.

BACKGROUND: The autonomic response to transcutaneous auricular vagus nerve stimulation (taVNS) has been linked to the engagement of brainstem circuitry modulating autonomic outflow. However, the physiological mechanisms supporting such efferent vagal responses are not well understood, particularly in humans. HYPOTHESIS: We present a paradigm for estimating directional brain-heart interactions in response to taVNS. We propose that our approach is able to identify causal links between the activity of brainstem nuclei involved in autonomic control and cardiovagal outflow. METHODS: We adopt an a

A computational modelling framework for assessing information transmission with cochlear implants.

Computational models are useful tools to investigate scientific questions that would be complicated to address using an experimental approach. In the context of cochlear-implants (CIs), being able to simulate the neural activity evoked by these devices could help in understanding their limitations to provide natural hearing. Here, we present a computational modelling framework to quantify the transmission of information from sound to spikes in the auditory nerve of a CI user. The framework includes a model to simulate the electrical current waveform sensed by each auditory nerve fiber (electro

Feasibility and optimal choice of stimulation parameters for supramaximal stimulation of motor evoked potentials.

PURPOSE: The aim was to investigate the feasibility and optimal stimulation parameters for supramaximal stimulation of muscle recorded transcranial electrical stimulation motor evoked potentials (mTc-MEP). METHODS: Forty-seven consecutive patients that underwent scoliosis surgery were included. First, the feasibility of supramaximal stimulation was assessed for two settings (setting 1: pulse duration 0.075ms, interstimulus interval (ISI) 1.5ms; setting 2: pulse duration 0.300ms, ISI 3ms). Thereafter, three mTc-MEP parameters were considered for both settings; (1) elicitability, (2) amplitude,

Prefrontal cortex oxygenation and autonomic nervous system activity under transcutaneous auricular vagus nerve stimulation in adolescents.

INTRODUCTION: The Neurovisceral Integration Model (NIM) proposes a complex interplay of visceral and neural structures that are crucial for adaptive responses to environmental demands. The aim of the present study was to investigate this circuitry using experimental manipulation via transcutaneous auricular vagus nerve stimulation (tVNS), measures of peripheral autonomic nervous system (ANS) activity and prefrontal cortex (PFC) oxygenation, quantified using functional near-infrared spectroscopy (fNIRS). METHODS: In a sample of n = 30 adolescents (age 14-17 years), tVNS versus sham stimulation

The effect of non-invasive vagus nerve stimulation on memory recall in reading: A pilot study.

Expert reading acquisition is marked by fluent, effortless decoding, and adequate comprehension skills and is required for modern daily life. In spite of its importance, many individuals struggle with reading comprehension even when decoding skills are adequate. Unfortunately, effective reading comprehension interventions are limited, especially for adults. A growing body of research suggests that non-invasive transcutaneous stimulation of the auricular vagus nerve (taVNS) may drive neural plasticity for low-level reading skills such as speech sound perception and letter-sound learning, but it

Low-intensity transcutaneous auricular vagus nerve stimulation reduces postoperative ileus after laparoscopic radical resection of colorectal cancer: a randomized controlled trial.

BACKGROUND: Postoperative ileus (POI) is thought to result from a disrupted sympathetic/parasympathetic balance caused by trauma or surgery. Transcutaneous auricular vagus nerve stimulation (tVNS) is a non-invasive technique involving stimulation of the vagal auricular branch, leading to autonomic regulation and reduced inflammation. Here, the effects of low-intensity transcutaneous auricular vagal stimulation on POI after laparoscopic radical resection of colorectal cancer were investigated. METHODS: One hundred and thirty-four patients who received scheduled laparoscopic radical resection o

Feasibility study on transcutaneous auricular vagus nerve stimulation using millimeter waves.

Objective. Electrical stimulation of the auricular vagus nerve is a non-invasive neuromodulation technique that has been used for various conditions, including depression, epilepsy, headaches, and cerebral ischemia. However, unwanted non-vagal nerve stimulations can occur because of diffused stimulations. The objective of this study is to develop a region-specific non-invasive vagus nerve stimulation (VNS) technique using the millimeter wave (MMW) as a stimulus for the auricular branch of the vagus nerve (ABVN).Approach. A numerical simulation was conducted to ascertain whether the MMW could e

Vagus nerve stimulation activates nucleus of solitary tract neurons via supramedullary pathways.

Vagus nerve stimulation (VNS) treats patients with drug-resistant epilepsy, depression and heart failure, but the mechanisms responsible are uncertain. The mild stimulus intensities used in chronic VNS suggest activation of myelinated primary visceral afferents projecting to the nucleus of the solitary tract (NTS). Here, we monitored the activity of second and higher order NTS neurons in response to peripheral vagal activation using therapeutic VNS criteria. A bipolar stimulating electrode activated the left cervical vagus nerve, and stereotaxically placed single tungsten electrodes recorded u

Face-hand sensorimotor interactions revealed by afferent inhibition.

Reorganization of the sensorimotor cortex following permanent (e.g., amputation) or temporary (e.g., local anaesthesia) deafferentation of the hand has revealed large-scale plastic changes between the hand and face representations that are accompanied by perceptual correlates. The physiological mechanisms underlying this reorganization remain poorly understood. The aim of this study was to investigate sensorimotor interactions between the face and hand using an afferent inhibition transcranial magnetic stimulation protocol in which the motor evoked potential elicited by the magnetic pulse is i
