The effect of intragastric balloon on plasma ghrelin, leptin, and adiponectin levels in patients with morbid obesity.

CONTEXT: Ghrelin and leptin are hormones regulating appetite and metabolic processes. Adiponectin plays an important role in the modulation of glucose and lipid metabolism. OBJECTIVE: The objective of the study was to evaluate the levels of plasma ghrelin, leptin, and adiponectin in obese subjects treated with bioenterics intragastric balloon (BIB), low-calorie diet (1500 kcal), and physical exercise. DESIGN: BIB was placed for 6 months in 21 subjects with body mass index 47.3 +/- 5.7. The control group consisted of 15 morbidly obese subjects treated with a low-calorie diet and physical effo

The myopathy of peripheral arterial occlusive disease: part 1. Functional and histomorphological changes and evidence for mitochondrial dysfunction.

In recent years, an increasing number of studies have demonstrated that a myopathy is present, contributes, and, to a certain extent, determines the pathogenesis of peripheral arterial occlusive disease (PAD). These works provide evidence that a state of repetitive cycles of exercise-induced ischemia followed by reperfusion at rest operates in PAD patients and mediates a large number of structural and metabolic changes in the muscle, resulting in reduced strength and function. The key players in this process appear to be defective mitochondria that, through multilevel failure in their roles as

Polycystic ovary syndrome and female reproduction.

Polycystic ovary syndrome (PCOS) is the most common endocrine disorder in women of reproductive age. The presentation is diverse. There is no single, unified definition of PCOS, no single diagnostic test and no consensus on the diagnostic criteria. Currently, PCOS is most commonly defined by the 2003 Rotterdam criteria, which requires at least two of three features for diagnosis--chronic anovulation, clinical and/or biochemical signs of hyperandrogenism, or polycystic ovaries. The aetiology is unknown, but the principal underlying disorder is thought to be reduced insulin sensitivity in periph

Bosentan treatment for pulmonary arterial hypertension related to connective tissue disease: a subgroup analysis of the pivotal clinical trials and their open-label extensions.

BACKGROUND: Endothelin-1 is considered to be a central pathogenic factor in connective tissue diseases (CTDs) such as systemic sclerosis (SSc), leading to vasoconstriction, fibrosis, hypertrophy and inflammation. A frequent complication of CTD is pulmonary arterial hypertension (PAH), which has a major effect on functioning and quality of life, and is associated with a particularly poor prognosis. OBJECTIVE: To present a subgroup analysis that summarises experiences from the pivotal studies and their open-label extensions with the oral dual endothelin-1 receptor antagonist bosentan in patient

Mitochondrial myopathy and complex III deficiency in a patient with a new stop-codon mutation (G339X) in the cytochrome b gene.

A 19-year-old woman complained of life-long exercise intolerance and had chronic lactic acidosis. Neurological examination was normal, but muscle biopsy showed cytochrome c oxidase-positive fibers and marked complex III deficiency. Sequence analysis showed a novel stop-codon mutation (G15761A) in the mitochondrial DNA (mtDNA)-encoded cytochrome b gene, resulting in loss of the last 41 amino acids of the protein. By PCR/restriction fragment-length polymorphism (RFLP) analysis, the G15761A mutation was very abundant (73%) in the patient's muscle, barely detectable (less than 1%) in her urine, an

Immunopathogenesis of fish allergy: identification of fish-allergic adults by skin test and radioallergosorbent test.

BACKGROUND: As the consumption of fish increases in the United States, the importance of allergic reactions to fish has become clear. Since most previous studies on fish allergy have focused on children reacting mainly to codfish, there is a need to investigate allergic reactions to other fish in adults. OBJECTIVE: To identify fish-allergic adults, and to assess cross-reactivity among different species of fish by RAST inhibition. METHODS: Thirty-nine individuals who reported fish allergy were selected for study; 32 (82%) were atopic as defined by two or more positive skin tests to common inh

[Enteral nutrition in burn patients].

UNLABELLED: Nutritional support plays an important role in the treatment of patients with burns. Due to the severe hypercatabolism that develops in these patients, oral support is insufficient in most cases, and this makes it essential to initiate artificial nutritional support (either enteral or parenteral). Enteral nutrition is more physiological than parenteral, and data exist which show that in patients with burns, enteral nutrition exercises a protective effect on the intestine and may even reduce the hypermetabolic response in these patients. The purpose of the study was to evaluate the

Effects and relationships of early physiotherapy on respiratory functions and functional capacity at discharge in patients undergoing open heart surgery.

BACKGROUND: Open heart surgery (OHS) often leads to declines in respiratory functions and functional capacity. Postoperative challenges, including changes in pulmonary function and respiratory muscle strength, can negatively impact patients’ recovery and quality of life. Early physiotherapy interventions aim to support the restoration of respiratory and physical functions during the critical discharge phase. The aim of this study was to investigate the changes in pulmonary function, respiratory muscle strength, and functional capacity, as well as the relationships among these markers, in patie

Hepatocyte growth factor for walking performance in peripheral artery disease.

BACKGROUND: VM202 is a plasmid encoding two isoforms of hepatocyte growth factor. In preclinical studies, hepatocyte growth factor stimulated angiogenesis and muscle regeneration. This preliminary clinical trial tested the hypothesis that VM202 injections in gastrocnemius muscle would improve walking performance in people with mild to moderate and symptomatic lower extremity peripheral artery disease (PAD). METHODS: In a double-blind clinical trial, patients with PAD were randomized to gastrocnemius muscle injections of either 4 mg of VM202 or placebo every 14 days for four treatments. The pr

Clinical impact of mean pulmonary arterial pressure after balloon pulmonary angioplasty for inoperable chronic thromboembolic pulmonary hypertension.

BACKGROUND: Balloon pulmonary angioplasty (BPA) improves haemodynamics in patients with inoperable chronic thromboembolic pulmonary hypertension (CTEPH). Previous studies on BPA have set the treatment objective to achieve a mean pulmonary arterial pressure (mPAP) of <30 mm Hg. However, the clinical impact of mPAP after BPA remains unclear. This study aimed to stratify patients according to their mPAP after BPA and evaluate its association with clinical status and long-term outcomes. METHODS: We retrospectively reviewed 304 patients with inoperable CTEPH (median age, 72 (61 to 79) years) who u

Exploring the prevalence of hyperglycemia and the link to physical fitness in young Danish women with overweight - A cross-sectional study.

BACKGROUND: Diagnosis of type 2 diabetes (T2D) at 20 years of age is associated with a reduction in life expectancy of 17.9 years. With an increasing prevalence of overweight among young people, we aimed to assess the prevalence of T2D and intermediate hyperglycaemia among young Danish women with overweight or obesity, who had not been previously diagnosed with T2D. Furthermore, we aimed to examine associations between markers of hyperglycaemia (glucose tolerance, fasting glucose, insulin, and HOMA-IR), body composition, physical fitness, and other lifestyle factors. METHODS: In this multicen

Optimised oxygenation improves functional capacity during daily activities in patients with COPD on long-term oxygen therapy: a randomised crossover trial.

BACKGROUND: Minimising hypoxaemia during submaximal walking tests has a positive effect on exercise capacity and dyspnoea in patients with chronic obstructive pulmonary disease (COPD) on long-term oxygen therapy (LTOT). However, the impact of optimising oxygenation during everyday tasks remains unexplored. Therefore, we investigated the effects of maintaining a target saturation on activities of daily living (ADL) using automated oxygen titration compared with conventional fixed oxygen flow. METHODS: In a double-blinded, randomised crossover trial, patients with COPD on LTOT performed two Gli

The relationship between symptoms and regurgitant severity in primary mitral regurgitation: a cardiovascular magnetic resonance study.

In the ACC/AHA guidelines, the presence of symptoms plays a central role in determining timing surgery in primary mitral regurgitation (MR). Studies have shown a disconnect between the severity of MR and symptoms. The purpose of this study is to assess risk factors for symptoms in patients with chronic primary MR. There were 430 patients with degenerative MR and preserved left ventricular function who underwent cardiovascular magnetic resonance (CMR). MR volume (MRV) and MR fraction (MRF) were categorized as per the ACC/AHA guidelines. Patients were divided into three groups based on category

Distal Ventricular Pacing for Drug-Refractory Mid-Cavity Obstructive Hypertrophic Cardiomyopathy: A Randomized, Placebo-Controlled Trial of Personalized Pacing.

BACKGROUND: Patients with refractory, symptomatic left ventricular (LV) mid-cavity obstructive (LVMCO) hypertrophic cardiomyopathy have few therapeutic options. Right ventricular pacing is associated with modest hemodynamic and symptomatic improvement, and LV pacing pilot data suggest therapeutic potential. We hypothesized that site-specific pacing would reduce LVMCO gradients and improve symptoms. METHODS: Patients with symptomatic-drug-refractory LVMCO were recruited for a randomized, blinded trial of personalized prescription of pacing (PPoP). Multiple LV and apical right ventricular pacin
