Transoral endoscopic-assisted management of subcondylar fractures in 17 patients: an alternative to open reduction with rigid internal fixation and closed reduction with maxillomandibular fixation.

The authors evaluate the results of transoral endoscopic-assisted open reduction and miniplate fixation of subcondylar fractures. Seventeen patients were treated from August 2005 to April 2007. Inclusion criteria were: adult patients, inability to achieve adequate occlusion with closed reduction, dislocation of the condylar fragment between 10 and 45 degrees , and 2-mm inter-fragment overlapping. Regular panoramic radiographs were taken postoperatively. Transbuccal incisions were used to place the screws for fixation in 15 patients. Pure intraoral access and angulated drills and screwdrivers w

Nailing versus prosthesis for femoral-neck fractures. A critical review of long-term results in two hundred and thirty-nine consecutive private patients.

In a follow-up study of 239 consecutive patients with fractures of the femoral neck treated by six surgeons in one hospital by Pugh nailing, Austin Moore prosthesis, or Knowles pinning, 96 per cent could be followed for a minimum of two years or until a definitve result had been reached. The incidence of unsatisfactory results was 39.3 per cent after Pugh nailing, 18.2 per cent after prosthetic replacement, and 14.2 per cent after Knowles pinning (mostly undisplaced or impacted fractures). Thirty-five unsatisfactory results following Pugh nailing were due to: aseptic necrosis which was definit

Femoral varus derotation osteotomy for the treatment of habitual subluxation and dislocation of the pediatric hip in trisomy 21: a 10-year experience.

BACKGROUND: Habitual hip subluxation and dislocation are potentially disabling features of the trisomy 21 syndrome. We describe outcomes after a femoral varus derotation osteotomy to achieve and maintain hip stability and community ambulation. METHODS: All individuals with trisomy 21, who underwent hip surgery at our institution between 1998 and 2008, were searched using the hospital databases. The clinical notes and radiographs were reviewed from presentation to final follow-up. Nine children (16 hips) aged below 10 years, were identified. All had a femoral varus derotation osteotomy with a

Ultrasound-guided cross-pin technique for paediatric supracondylar humerus fractures: minimizing iatrogenic ulnar nerve injury.

BACKGROUND: Previous ultrasound-guided cross-pin techniques, which employ 90° elbow flexion, have demonstrated effectiveness; however, they may be associated with an elevated risk of iatrogenic ulnar nerve subluxation or dislocation. The aim of this study was to evaluate the efficacy and safety of a modified ultrasound-guided cross-pin technique for reducing the risk of iatrogenic ulnar nerve injury in paediatric patients with supracondylar humerus fractures. METHODS: This retrospective study was conducted from December 2017- October 2019 and included paediatric patients with displaced supraco

High Rates of Spacer Fracture in the Setting of Extended Trochanteric Osteotomy With a Specific Thin-Core Articulating Antibiotic Hip Spacer.

BACKGROUND: Two-stage revision remains the standard of care for prosthetic joint infection after total hip arthroplasty. However, there are substantial complications associated with articulating antibiotic hip spacers. Handmade and molded spacers have been shown to have higher rates of spacer fracture than antibiotic-coated prostheses (ACPs). The aim of this study is to review outcomes with an implant that is often categorized as an ACP spacer, the Zimmer-Biomet StageOne Select Femoral Spacer (ZBSO). METHODS: A retrospective review was performed of 63 patients who underwent placement of a ZBS

The gas bubble sign-a reliable indicator of laryngeal fractures in hanging on post-mortem CT.

OBJECTIVE: The purpose was to evaluate the presence of gas in the tissue adjacent to the laryngeal structures, "the gas bubble sign", in cases of hanging as a diagnostic indicator of neck trauma. METHODS: In this study, post-mortem CT (PMCT) scans and autopsies of 35 victims of hanging were examined to reveal age-dependent changes, laryngeal fracture, fracture location and the presence of gas. A matched group with cardiac arrest or intoxication was used as controls (n = 35). An autopsy was performed in each case. RESULTS: Incomplete suspension was the most common method in hanging. The thyro

Surgical evolution in the treatment of mandibular condyle fractures.

BACKGROUND: In Literature fractures of the mandible that involve the condyle ranges from 20% to 35% and various possible surgical options are described according to the varying pathological situations. Up to the present, numerous techniques have been used for the surgical treatment of condylar fractures. In this article we are proposing the combination of two surgical techniques as therapy for extra-capsular condylar fractures with dislocation. METHODS: From June 2003 to July 2007 30 patients were treated for condylar fractures with the application of a Rigid External Fixator under endoscopic

[Late results of central talus injuries].

From 1982 to 1992 we treated 66 talar injuries at our institution, 44 of which were central fractures (25 talar neck fractures, 17 corpus fractures, 1 comminuted fracture and 1 complete dislocation of the intact talus). In 77% of patients additional injuries were present; 34% were polytraumatized. In 41 patients (93%) clinical and radiological follow-up was possible after an average of 6.3 years. All displaced fractures were treated by open reduction (84%) and, usually, screw fixation, 41% of these within 6 h after the accident. Only 1 required an osteotomy of the medial malleolus, and in 4 a

[Combination of a Lisfranc injury and a dislocation of the proximal tibiofibular joint].

Lisfranc injuries and dislocations of the proximal tibiofibular joint are both rare and frequently missed on initial examination. However, delayed treatment can lead to chronic instability. This is a case report of a 26-year-old man, who received both of these injuries on the same extremity from a collision during a basketball game. The dislocation of the proximal tibiofibular joint was treated with closed reduction, however the Lisfranc injury required open reduction and internal fixation. At the follow-up after ten weeks, the patient was doing well and started rehabilitation.

The Volume Difference Along the External Surface of the Zygomatic Bone: A Novel Method of Measuring Zygomatic Bone Asymmetry.

This study introduced the volume difference along the external surface (VDAES) of the zygomatic bone as a novel approach to assess zygomatic bone asymmetry and was the first to describe a distinctive, 4-step method of measuring it. VDAES has a potential to be used as an objective tool to evaluate dislocation and can assist surgeons in predicting risks of long-term cosmetic complications in patients with zygomaticomaxillary complex fractures. After having measured 100 healthy study participants, the observed median VDAES was 1.48 cm3 for all study participants, 2.02 cm3 for males, and 1.09 cm3

Measuring Deltoid Insufficiency After Supination-External Rotation Ankle Fracture With Lateral Talar Subluxation on Gravity Stress View.

BACKGROUND: Stress-view radiographs are frequently obtained to evaluate supination-external rotation (SER) variant ankle fractures. Measurement of the ankle medial clear space (MCS) is a surrogate of medial structure integrity as part of overall ankle stability. Measurement of the lateral talus displacement with respect to the incisura may be a sensitive and specific method to assess joint subluxation. METHODS: Retrospective review of acute SER-variant isolated lateral malleolar fractures with gravity stress views (GSVs) were performed for 103 patients. GSV analysis was performed with standar

Morphologic Alterations of the Sternoclavicular Joint following Ipsilateral Clavicle Fractures.

RATIONALE: To our knowledge, no study has investigated concomitant injuries of the sternoclavicular joint (SCJ) in acute clavicle fractures. The purpose of this study was to determine the effect of an ipsilateral clavicle fracture on the SCJ in a systematic computer tomography (CT) morphologic evaluation. METHODS: CT scans in the axial and coronal plane of 45 consecutive patients with clavicle fractures were retrospectively analyzed. The scans were assessed regarding anatomic congruence of bilateral SCJs-joint space width (JSW); the position of bilateral medial clavicles (PC); and the non-fus
