An upsurge in Echovirus 30 (E30) infections, associated with meningitis/meningoencephalitis, has been observed in Denmark, Germany, the Netherlands, Norway and Sweden in the period April to September 2018, compared with 2015-2017. In total, 658 E30 infections among 4,537 enterovirus infections were detected in 15 countries between January and September 2018 and affected mainly newborns and 26-45 year-olds. National public health institutes are reminded to remain vigilant and inform clinicians of the ongoing epidemic.

We report preliminary findings of a large outbreak of human leptospirosis with 36 confirmed/probable and 583 suspected cases from June-August 2018, linked to contaminated water bodies in Northern Israel. There was a travel-associated case in Germany; additional cases are being investigated in other countries. The presumed chain of transmission, implicating wild boar and cattle, raises multiple challenges for risk assessment, risk management and risk communication currently being addressed by a public health response team.

States in south-eastern Brazil were recently affected by the largest Yellow Fever (YF) outbreak seen in a decade in Latin America. Here we provide a quantitative assessment of the risk of travel-related international spread of YF indicating that the United States, Argentina, Uruguay, Spain, Italy and Germany may have received at least one travel-related YF case capable of seeding local transmission. Mitigating the risk of imported YF cases seeding local transmission requires heightened surveillance globally.

The conventional surgical history of ligation of a patent ductus arteriosus (PDA) dates from August 26, 1938, when Robert E. Gross of Boston, Massachusetts, successfully ligated a PDA. It is largely unknown that in the same year and before Gross, Emil Karl Frey, a Surgeon at the Medizinische Akademie in Dusseldorf, Germany, already ligated a PDA successfully. Assuming that he would soon perform more ligations, Frey did not publish his findings, and this historic ductal operation escaped attention.

The authors compared the efficacy of three different doses (18.75, 37.5, and 75 MU per parotid gland) of botulinum toxin A (BTX-A; Dysport, Ipsen Pharma, Germany) injections vs vehicle in patients with sialorrhea (n = 32) using a single-center, prospective, double-blind, placebo-controlled dose-finding study. The primary endpoint was achieved with 75 MU BTX-A without treatment-related adverse events, suggesting BTX-A is a safe and effective treatment for patients with sialorrhea.

In about 10% of cases, tick-borne encephalitis (TBE) presents with additional myeloradiculitic features mimicking acute poliomyelitis, which can rarely appear as the sole symptom. We report on a 59-year-old man infected with TBE in Thuringia,Germany, who developed polyradiculitis with rapidly progressive, predominantly proximal tetraparesis and respiratory failure. We discuss the differential diagnosis and the epidemiological relevance in conjunction with a second typical case of TBE acquired in the same region and time period.

Despite a low incidence of botulism in the industrialized world some cases occasionally occur in Germany after eating contaminated food. Because botulism is rarely seen, most physicians are unfamiliar with its early recognition and treatment. However, immediate intensive care treatment is important. We report the case of a previously healthy 54-year-old female who developed signs of botulism after eating vacuum packed smoked fish and developed severe respiratory insufficiency with difficult carbon dioxide elimination in the days following.

Methicillin-resistant S. aureus with quinolone-resistance (QR-MRSA) isolated in Germany from three outbreaks of nosocomial infections and from sporadic nosocomial infections in five hospitals exhibited different SmaI-restriction patterns of their genomic DNA. Phage-typing and determination of plasmid profiles performed in parallel confirmed this differentiation, with one exception. These results indicate that there is obviously no overregional spread of one particular QR-MRSA clone and that quinolone resistance has developed independently in different MRSA.

Tropical mycoses are very rare diseases in this country. To date there have only been two published observations of paracoccidioidomycosis in the Federal Republic of Germany. In these rare cases the diagnosis is often delayed because paracoccidioidomycosis imitates frequently appearing and well-known diseases like tuberculosis or sarcoidosis. The diagnostic and therapeutic aspects are illustrated in a recently observed patient from Peru. Therapy consisted of ketoconazole 400 mg/day orally administered with good results and only transient side effects.

A few days after a spine of one of his indoor cacti had pricked the fourth finger of his left hand, a 56-year-old man developed multiple inflammatory suppurating nodules and infiltrates and a cord-like lymphangitis on the left forearm and upper arm. Nocardia brasiliensis was cultivated from aspirated pus. Subsequently, both Nocardia brasiliensis and Nocardia asteroides were isolated from the cactus earth. This is the first report of an infection by Nocardia brasiliensis acquired in Germany.

Tn unexplained, autochthone Trichophyton simii infection in Europe is reported. The patient was a 11-year-old caucasian girl, who had never been out of Germany, nor had contact with anyone from abroad. The girl's only pet, a guinea pig, was proven to be free of dermatophytes. Clinically the infection represented a somewhat persistent circumscribed lesion at the dorsum of one upper arm characterized mainly by small granulomata around hair follicles with only a slight erythematosquamous component.
